    Streszczenie angielskie: Tracheostomy is a life-saving procedure when pertbrmecl with an appropriate indication and surgical technique. There is a limited indication for tracheostomy procedures in paediatrie surgery. It is rarely applied to the paediatric patient because they can be kept intuhated for a longer period compared to adults. Indications for tracheostomy are prolonged intubation, subglotie stenosis, general body trauma, tracheomalasia, cervical tumour pressing the trachea and larynx, congenital diseases of the larynx, burn injuries of the trachea, and foreign body aspiration. In the last decade the number of cases with tracheotomy increased due to the development of new intensive care units, the use of mechanical ventilation, and the increasing number of patients needing prolonged ventilation support. Tracheotomy in children lias been associated with significant operative and post-operative complications. When long-term tracheotomy is needed, parent education, together with special equipment and environment may allow the return of the tracheotomised child to home in secure conditions.

    Streszczenie angielskie: Choanal atresia is a congenital abnormality often coexisting with other developmental abnormalities. The treatment of choice his condition is surgical restoration of choanae patency using a transnasal or transpalatal operative approach. After ;ical treatment, separators are fixed for several weeks in the posterior choanae and nasopharynx. Surgical transpalatal rvention and extended retention of separators in the posterior choanae and nasopharynx may cause auditive tube unction and otitis media with effusion. The aim of this work was the estimation of the occurence of otitis media with sion in children under post-operative treatment after choanal atresia depending on the surgical approach applied. immary of the period 1959-2002 in the Department of Paediatric Otorhinolaryngology in Warsaw shows that 175 operations s performed because of choanal atresia. A group of 11 children operated on choanal atresia between 1996-2002 was nined. Acoustic impedance audiometry was performed in all children. Bilateral choanal atresia was recognised in 4 children, unilateral choanal atresia in 7 children. A tympanomctry curve type B was obtained in 4 children, but a tympanometry curve type A was found in 7. In the examined group we did not find a dependence between the type of operative approach in surgical treatment of choanal atresia and otitis media with effusion occurrence.

    Streszczenie angielskie: Laryngeal papillomas in children constitute a disease of viral aetiology caused by Human Papilloma Virus. Even though the changes arc histologically benign, their exophytic growth may lead to a critical airway obstruction and the child's suffocation. Due to the tendency to recur, lack of causal treatment, and risk of malignancy the optimal method of treating this condition is still being sought. The study presents methods applied nowadays in the treatment of laryngeal papillomas; especially, it concentrates on a classical papillotomy and its main advantage: the diminished risk of scar formation leading to laryngeal stenosis and vocal fold injury with speech disorders.

    Streszczenie angielskie: Laryngeal papillomas are a frequent pathology in children and adolescents. The involvement of the respiratory system and spreading of the papillomas into the trachea and into the bronchi is a rare but severe complications. In teh group of 90 children diagnosed and treated in the Clinic of Otolaryngology papillomas in the trachea and in the bronchi were found 4 children. In the chest X-ray and in the CT of the chest in all four children small tumors and cysts of diameter beteen 5mm and 3 cm were found in the lungs. These lesions increased during the observation (8-16 years). The number and size of cystic lesions increased with age, with periodic occurrenceof inflammatory and atelectatic changes and progressive respiratory failure. One patient died in the age of 18, and teh clinical status of two patients is severe. The diagnosis of acquired cystic lesions in the lungs in the patients with laryngeal papillomas suggest the presence of papillomas in the trachea and in the bronchi with involvement of lung tissue.
