    Streszczenie angielskie: A series of four new potential renin inhibitors has been synthesized. The structure of the compounds was designed in such a way as to produce agents resistant to enzymatic degradation, metabolically stable, possibly potent and with improved oral absorption. All positions of the 8-13 fragment of the human angiotensinogen were occupied by unnatural units (two unnatural amino acids in position P3 and P2 and two pseudodipeptides in positions P1-P1, and P2-P3,). Both N- and C-terminal functions of the inhibitors were blocked with tert-Boc and ethyl ester groups. Their hydrophobicity evaluated as a log P value, calculated by a computer method, was 6.57 and 6.08 respectively. All peptides were obtained by the carbodiimide method in solution and purified by chromatography on the SiO2 column. Their resistance to enzymatic degradation was assayed by determination of stability against chymotrypsin activity. The potency was measured in vitro by a spectrofluorimetric method (assay of Leu-Val-Tyr-Ser released from the N-acetyltreadecapeptide substrate by renin in the presence of the inhibitor). All inhibitors were stable to chymotrypsin. Their IC50 (M/I) values were: 9.6 x 10**-4 (12), 1.6 x 10**-5 (17), 1.0 X 10**-5 (22) and 1.0 x 10**-5 (23) respectively.

    Streszczenie polskie: Przedstawiono 6-letniego chłopca, u którego w przebiegu plamicy Sch”nleina-Henocha wystąpiły powikłania neurologiczne w postaci utraty przytomności, uogólnionych drgawek i zaburzenia widzenia. Badanie MR wykazało obszary niedokrwienne w okolicy ciemieniowo-potylicznej mózgu, wskazujące na istnienie rozległego zapalenia drobnych naczyń (vasculitis).

    Streszczenie angielskie: We report 6-years old boy with Sch”nlein-Henoch purpura who presented neurologic manifestations: depressed state generalised covulsions, and cortical blindness. Sequential magnetic resonance imaging showed bilateral cerebral ischemic leisons in teh cortex and white matter of parieto-occipital lobes coused by vasculitis.

    Streszczenie angielskie: Background: Impairment in the functions of the lower urinary tract can be the cause of recurrent urinary tract infections (UTI) and vesicoureteral reflux (VUR) in children. The purpose of our research was to evalute the frequency of occurrence of bladder instability in children with UTI. Material/methods: The research involved 114 children (21 boys, 93 girls), ranging in age from 5 to 16. Group I consisted of 61 children with a history of recurrent UTI, while Gorup II included 53 children with recurrent UTI and VUR. Urodynamic tests (uroflowmetry and cystometry) were done on all the children, while in selected cases profilometry was also performed, using a Duet apparatus (Dantec Medical A/S). Results: Abonrmal functioning of the lower urinary tract was found in 45 children (74 p.c.) from Group I and 44 children (84 p.c.) grom Gorup II. The most common dysfunction was instability of the detrusor muscle, which was found in 52 children (45 p.c.), including 23 (38 p.c.) from Group I and 29 (55 p.c.) from Group II. In 19 children detrusor instability was accompanied by reduced bladder volume, and in 8 cases there as a lack of detrusor-sphincter coordination. In both groups ca. 20 p.c. of the children did not present with symptoms indicating urination dysfunctions. Ca. 80 p.c. reported various symptoms, of which the most common were nocturnal wetting and urinary urgency. In half of the children from Group I and one-fourth of the children from Group II there were several co-occurring symptoms; nocturnal and diurnal wetting, pollakiruria, and urinary urgency, or all three symptoms simultaneously. Conclusions: The most common disturbance of lower urinary tract functioning in these children with recurrent UTI was instaability of the detrusor muscle, which occurred more often in children with VUR.

    Streszczenie angielskie: The aim of the work was to assess the function of lower urinary tract in children with primary enuresis nocturna and recurrent urinary tract infections. Urodynamic examinations (uroflowmetry and cystometry) were done using Duet f. Dantec. The examinations were carried out in 87 children at the age from 5 to 16 years (mean age 8,6 year), who were divided in groups: I - 49 children without anatomical abnormalities in urinary tract, II - 38 children with vesicoureteral refluxes of II degree. The results showed disturbances of urinary tract in (73/87) children, more often in group II. The most frequent abnormality in both groups was detrusor instability (44/87), decreased cystometric capacity of bladder (18/87) and sphincter-detrusor dyssynergia (9/87). In children with primary enuresis nocturna and recurrent urinary tract infections, disturbances of urinary tract function appear in over 80 p.c.
