    Streszczenie polskie: Zespół Omenna jest rzadką, uwarunkowaną autosomalnie recesywnie chorobą, charakteryzującą się objawami ciężkiego złożonego niedoboru odporności skutkującego ciężkimi zakażeniami oraz erytrodermią, eozynofilią, hepatosplenomegalią, limfadenopatią, przedłużającą się biegunką, hypogammaglobulinemią i podwyższonym poziomem przeciwciał klasy IgE. Przedstawiony przypadek demonstruje trudności diagnostyczne, jakie towarzyszyły rozpoznaniu zespołu Omenna. Wynikały one przede wszystkim z niepełnego obrazu chorobowego w pierwszej fazie choroby. Podstawową manifestacją zespołu w początkowym okresie była przewlekająca się erytrodermia. Ponieważ występowanie zespołu jest b. rzadkie, a poszczególne objawy nie są dla niego patognomoniczne, dopiero ich łączne występowanie w połączeniu z charakterystycznymi zaburzeniami immunol. pozwoliło na ustalenie rozpoznania. Prezentując powyższy opis przypadku pragniemy zwrócić uwagę na konieczność uwzględniania zespołu Omenna w różnicowaniu przyczyn erytrodemii u noworodków i niemowląt.

    Streszczenie angielskie: Ommen's syndrome is a rare, autosomal recessive disease that is characterized by symptoms of a severe combined immune deficiency, severe infections, erythrodermia, hypereosinophilia, hepatosplenomegaly, lymphadenopathy, protracted diarrhoea, hypogammaglobulinemia and elevated serum IgE. The presented case demonstrates diagnostic problems in recognizing Omenn's syndrome. These resulted predominantly from the incomplete clinical picture of the disease in its early phase. Persistent erythrodermia was the basic manifestation of the syndrome in the initial phase. Sine the occurrence of the disease is extremely rare and particular symptoms are not pathognomonic, it is their co-occurrence in association with typical immunological disorders that enabled to estabilish the diagnosis. Through the presentation of the above case history we should like to point to the necessity of considering Omenn's syndrome in differentiating causes of erythrodermia in neonates and infants.

    Streszczenie angielskie: Background: Disorders of lymphocyte subpopulations counts and NK cells may not only contribute to impaired elimination of pathogens, but also be an important factor in the development of generalized inflammatory reaction. The aim of the paper was the analysis of subpopulations of lymphocytes and NK cells in children with sepsis and after recovery. Material/Methods: The study comprised 25 septic children (17 infants and 8 children aged from 1 to 3 years). The control group consisted of 25 healthy children. The study was conducted in the initial phase of sepsis and then repeated 2 to 3 months after recovery. In all the children, the percentage and absolute numbers of basic lymphocyte subsets and NK cells in blood serum were evaluated. Results: In the course of sepsis, the absolute number of activated lymphocytes T in the youngest children was higher in comparison with the control gorup. After revovery, the percentage and absolute counts of CD4 lymphocytes in all the children and CD3/CD4 lymhocytes ratio in the youngest children were considerably lower than in the control gorup. The percentage and absolute number of NK cells in the youngest children after recovery were higher in comparison with the control group. Conclusions: In children who underwent sepsis, depression traits of cellular immunity are observed. The occurrence of defensive system disorders after recovery may be an important factor in recurrent infections and creates the necessity of immune system monitoring in children with the hisotry of sepsis for the purpose of prevention.

    Streszczenie angielskie: The aim of the study was to assess the correlation between morphological and immunological features of lesions in the liver in children with chronic hepatitis C. In the study 11 children with chronic hepatitis C were enrolled. Histopathological and immunomorphological investigations included antigen marking CD45, CD45RA, CD45RO, CD20, CD3, CD8 in liver biopsy specimens. The immunomorphological assessment of the inflammatory infiltrate cells showed the presence of CD45 antigen on the vast majority of leukocytes in the portal tracts and in the lobular parenchyma in all the studied children. One fourth of the inflammatory cells were characterised by CD20 phenotype, 1/2 - by CD3 phenotype, 1/3 - by CD8phenotype, 1/3 - by CD45RA phenotype and 2/3 - by CD45RO phenotype. Conclusions: T and B lymphocytes were predominant in hepatic inflammatory infiltrates (T lymphocytes are twice as numerous as B lymphocytes). Among T lymphocytes, cytotoxic and suppressor cells were prevailing. Most T lymphocytes were characterized by CD45RO phenotype, which shows their activation. A positive correlation between most lymphoid cell markers, staging and grading suggests the role of these cells in liver injury.

    Streszczenie polskie: Celem pracy była ocena współzależności pomiędzy odsetkiem komórek NK (natural killer cells) w nacieku zapalnym wątroby a liczbą i aktywnością cytotoksyczną komórek NK we krwi obwodowej u dzieci z przewlekłym zapaleniem wątroby typu C. Badaniami objęto 25 dzieci z przewlekłym zapaleniem wątroby typu C. Grupę porównawczą stanowiło 10 dzieci z ujemnym wywiadem w kierunku chorób wątroby i z prawidłową aktywnością aminotransferaz. W grupie badanej dokonano oceny biopunktatów wątroby: histopatologicznej oraz immunomorfologicznej polegającej na typowaniu komórek NK w nacieku zapalneym wątroby. U wszystkich dzieci określono odsetek i bezwzględną liczbę komórek NK we krwi obwodowej metodą cytometrii przepływowej oraz aktywność komórek NK w stosunku do komórek linii białaczkowej erytroleukemii K-562 bez stymulacji i po stymulacji IL-2 (in vitro). Wyniki: W badanej grupie komórki NK stanowiły średnio 6,07 ń 3,92 proc. komórek nacieku zapalnego wątroby. We krwi obwodowej komórki NK stanowiły 11,68 ń 6,73 p.c. krwinek białych, a liczba bezwzględna komórek NK wynosiła 241,08 ń 128,56/ml. Aktywność cytotoksyczna komórek NK wynosiła w rozcieńczeniu E:T 25:1 - 91,8 ń 1,07 proc.; E:T 12,5:1 - 6,72ń 3,68 proc., a po stymulacji IL-2 odpowiednio 92,8 ń 1,01 proc. i 7,58 ń 3,95 proc.     Streszczenie angielskie: The aim of this study was to evaluate the correlation between NK (natural killer) cells in an inflammatory infiltration of the liver and in peripheral blood in children with chronic hepatitis type C. Material and methods: The study comprised 25 children with chronic hepatitis type C. The control group comprised 10 children with no liver disease in past medical history and normal acitivity of aminotransferases. In the study group, liver biopsy specimens were evaluated by histopathology and immunomorphology - the presence of NK cells present in the inflammatory infiltrate was determined. In all children the percentage and absolute count of NK cells in peripheral blood were evaluated by flow cytometry, as well as the activity of NK cells in relation to leukaemic cells of erythroleukemia K-562 without stimulation and after the stimulation with IL-2 (in vitro). Results: In the studied group of children NK cells constituted 6.07 ń 3.92 p.c. fo cells present in liver infiltrate. In peripheral blood NK cells constituted 11.68 ń 6.73 p.c. of white blood cells, and theri absolute number was 241.08 ń 128.56/ml. The cytotoxic activity of NK cells in dilution E:T 25:1 was - 91.8 ń 1.08 p.c. and in dilution E:T 12.5 - 6.72 ń 3.68 p.c.
