    Streszczenie polskie: W latach 1998 - 2000 w Katedrze i Klinice Ortopedii i Traumatologii AM w Lublinie leczono 15 chorych z powodu ogniskowego uszkodzenia chrząstki stawowej kłykci kości udowych. Wiek chorych wahał się od 20 do 72 lat, zaś okres obsewacji od 6 miesięcy do 2,5 lat. Przyczyną ubytku okrywy chrzęstnej 9 pacjentów była chrzęstno-kostna martwica oddzielająca, uszodzenia chrząstki stawowej dalszych 5 osób były skutkiem przewlekłej niestabilności więzadłowej kolan. U jednego chorego zwichnięcie rzepki spowodowało chrzęstne złamanie kłykcia bocznego kości udowej. Głębokość ubytków oceniano wg skali zaproponowanej przez ICRS. Uszkodzenia IVř odnotowano u 11 operowanych. IIIř - u pozostałych 4 osób. Ubyki chrzęstne kłykci kości udowych wypełniano autogennymi walcowatymi przeszczepami chrzęstno-kostnymi sposobem plastyki mozaikowej posługując się zestawem narzędzi OATS. Wyniki leczenia oceniano na podstawie prospektywnych badań porównując wartości punktowe przedoperacyjne i podczas badań kontrolnych wg skali HSS czynnościowej oceny kolana. Wartości przed zabiegami wahały się od 28 do 51 punktów, średnio 40,3. W badaniach odległych wynosiły od 76 do 91 punktów, średnio 84,7. Wszyscy pacjenci odzyskali pełny zakres ruchów, chociaż u jednego konieczna była artroskopowa mobilizacja kolana. Najniższe pooperacyjne wartości HSS uzyskały najstarsze osoby, u których stwierdzono również zmiany przedziału bocznego kolana i stawu rzepkowo-udowego.     Streszczenie angielskie: A series of 15 patients with focal chondral lesions of the femoral condyles were treated at the Othopaedics Department of the University of Medical Sciences of Lublin between 1998 - 2000. The patients' age varid from 20 to 72 years and the follow-up period from 6 months to 2.5 years. Osteochondritis dissecans was observed in 9 patients. Articular cartilage defects associated with chronic ligament instability was noted in 5 cases and 1 patient suffered from a chondral faracture of the lateral femoral condyle during a traumatic luxation of the patell. The depth of the lesions was graded according to ICRS scale. Grade IV was found in 11 cases and the reamaing 4 cases had grade III lesions. The defets were treated wtih osteochondral autografts using OATS instrumentation set. Results were prospectvely evaluated according to the HSS score. The preoperative value of HSS score ranged from 28 to 51 points (average 40.3). During follow-up the score values ranged from 76 to 91 points (average 84.7 points). All patients regained full range of moton in the knee, although in one case arthroscopic release of intraarticular adhesions was performed 6 months after chondroplasty. The lowest HSS values at follow-up were found in elderly patients with concomitant mild degenerative chages in the lateral knee compartment and in the patello-femoral joint.     Streszczenie angielskie: DNA pooling is a potential methodology for genetic loci with small effect contributing rapid preliminary screening of the genome for the allelic association with the most common class of polymorphic short tandem repeart markers. The methodology assumes as a common founder for the linked disease locus of interest and searches for a region of a chromosome sheared between affected individuals. The general theory of DNA pooling basically relies on the observed differences in the alletic distribution between pools from affected and unaffected individuals, including a reduction in the number of alleles in the affected pool, which indicate the sharing of a chromosomal region. The power od statistic for associated linkage mapping can be determined using two recently developed strategies, firstly, by measuring the differences of allelic image patterns produced by two DNA pools of extreme character and secondly, by measuring total allele content differences by comparing between two pools containing large numbers of DNA samples. These strategies have effectively been utilized to identify the shared chromosomal regions for linkage studies and to investigate the candidate disease locci for fine structure gene mapping using allelic association. This paper outlines the utilization of DNA pooling as a patential tool to locate the complex disease loci, statistical methods for accurate estimates of allelic frequencies from DNA pools, its advantagess, drawbacks and significance in associate linkage mapping using pooled DNA samples.
