    Streszczenie angielskie: Combined treatment with insulin and oral agents is implemented when insulin secretion is impaired. Administration of intermediate-acting insulin and oral drug breaks glucotoxicity and improves beta cell function. Another foal of combined treatment is a decrease of postprandial glycemia. In agreement with European guidelines for diagnosis and treatment of type 2 diabetes, recommended initial combined treatment in obese patients includes one daily intermediate-acting insulin and metformin (when contraindications are excluded). According to the European Diabetes Policy Group (EDPG) combined treatment with one insulin injection is implemented when HbA1c level exceeds 7.5 p.c. Combined treatment with two injections of premixed insulin or several injections (intensive insulin therapy) and oral agent aims to reduce insulin requiments by 30 p.c. to 50 p.c. by improvement of peripheral insulin action. This treatment is recommended particularly in obese individuals. In many clinical trials the effect of combined treatment is considered not only in terms of glycemic control and body weight but akso hypoglycemia, lipid metabolism, blood pressure, diabetes complications, liver and kidney function.
