Background. The management of patients with perisistent atrial fibrillation (AF) has not benn well established. Recurrences of AF following electrical cardioversion are common and the use of a single antiarrhythmic drug only is often ineffective. Aim. To assess the efficacy of sequential use of antiarrhythmic agents combined with electrical cardioversion for restoration and maintenence of sinus rhythm in patients with persistent AF. Methods. The study group consisted of 83 patients (27 females, 56 males, mean age 60.7 ń 8.3 years) who were selected for elective electrical cardioversion of persistent AF. After sinus rhythm restoration patients were started on antiarrhythmic treatment in an open-label manner with propafenone, sotalol or disopyramide. If there was a recurrence of AF, the electrical cardioversion was repeated and the patient was started on the other antiarrhythmic agent. In case of a further AF recurrence, amiodarone in the maintenance dose of 100 - 200 mg/day was continued. Patients in whom the initial cardioversion was not successful, were treated with amiodarone and underwent repeated cardioversion. The follow-up duration was 12 months. Results. The first electrical cardioversion was effective in 44 (53.0 p.c.) patients. After one year of follow-up, 23 (52.3 p.c.) of them maintained sinus rhythm. None of the patients who underwent second cardioversion and received a second antiarrhythmic drug, maintained sinus rhythm. Background. Focal ventricular arrhythmias (FVA) are relatively rare and account for 4 p.c. of all patients with ventricular arrhythmias. Because antiarrhythmic therapy often fails, other methods have gained considerable interest. They include radiofrequency ablation (RFA) and, less often, implantation of cardioverter-defibrillator (ICD), antiarrhythmic surgery or a combination of these methods. Aim. To assess the safety anf efficacy of RFA in patients with FVA. Methods. Between 1998-2001, twenty six patients (16 females, 10 males) aged 21-64 years (mean 43.5ń12.4 years) underwent RFA because of recurrent, symptomatic and drug-refractory FVA. All the patients underwent FRA based on the earliest ventricular activation during tachyarrhythmia and/or the concordance of the morphology of paced QRS during sinus rhythm with QRS morphology durting arrhythmia in all 12 ECG leads. Total efficacy of the procedure was defined as the complete elimination of both spontaneous and induced arrhythmia of the morphology identical with that of clinical arrhythmia. Partial success was defined when a substantial reduction in the frequency and quantity of arrhythmia was achieved becoming suppressible with antiarrhythmic treatment. Results. In total, 33 RFA sessions were performed. The number of 60-second RF current applications ranged from 8 to 35 (mean 17) and the fluoroscopy time varied between 18 and 79 minutes (mean 36.3 minutes). The long QT syndrome (LQTS) is a disorder of ventricular repolarization that exposes affected individuals to cardiac arrythmias and sudden death. The first gene for LQTS has been mapped to chromosome 11 p. 15.5 by genome-wide linkage analysis. This gene, originally named KVLQT1 (and later KCNQ1), is a novel potassium channel gene. Mutations in the human KVLQT1 gene, encoding the ŕ-subunit of the KVLQT1 channel, cause the long QT syndrome. In this work, we analysed the sequence of six KVLQT1 exons in patients with various heart pathologies. We describe 6 different mSSCP patterns with no disease-related SSCP conformes in any sample. Different sequencing of exons 2 to 7 confirmed the absence of mutations. This suggests that the analysed region of the KVLQT1 gene is not commonly involved in pathogenesis of the long QT syndrome.

Wstęp: Nadciśnienie tętnicze u większości chorych współistnieje z innymi schorzeniami nasilającymi jego wpływ na zaburzenia czynności układu krążenia. U chorych z czynnikami ryzyka sercowo-naczyniowego, takimi jak: cukrzyca, choroba wieńcowa czy niewydolność nerek, uzyskanie dobrej kontroli ciśnienia tętniczego ma szczególne znaczenie. Udowodniono, że stopień zmniejszenia ryzyka sercowo-naczyniowego w wyniku leczenia hipotensyjnego zależy od osiągniętych wartości ciśnienia tętniczego. Preparaty stanowiące kombinację leków hipotensyjnych w stałych, małych dawkach stworzyły nowe możliowści terapii nadciśnienia tętniczego. Głównym celem niniejszego badania była ocena skuteczności hipotensyjnej i tolerancji preparatu Noliprel (0,625 mg indapamidu i 2 mg peryndoprylu) w leczeniu chorych na nadciśnienie tętnicze łagodne lub umiarkowane ze współistniejącą cukrzycą i/lub chorobą wieńcową, i/lub niewydolnością nerek. Materiał i metody: Badanie przeprowadzono jako wieloośrodkową próbę, o zasięgu ogólnopolskim, w poradniach specjalistycznych. Leczenie trwało 12 tyg. i poprzedzone było 2-tyg. okresem selekcji. W okresie leczenia chorzy otrzymywali Noliprel w dawce 1 tabletka dziennie. U chorych, u których po 6 tyg. nie uzyskano dostatecznej kontroli ciśnienia tętniczego, dawkę Noliprelu zwiększono do 2 tabletek dziennie. Po 12 tyg. leczenia (wizyta W 12) badacze ocenili skuteczność i tolerancję zastosowanwej terapii hipotensyjnej, a dodatkowo poproszono chorych o dokonanie subiektywnej oceny tolerancji leczenia....

KCNQ1 (formerly called KVLQT1) is a Shaker-like voltage-gated potassium channel gene responsible for teh LQT1 sub-type of LQTS. In general, heterozygous mutations in KCNQ1 cause Romano-Ward syndrome (LQT1 only), while homozygous mutations cause JLNS (LQT1 and deafness). To date, more than 100 families with mutations in this gene have been reported, most with their own novel 'private' mutations. The majority of these mutations are missense. However, other types of mutations, such as deletions, frame-shifts and splice-donor errors have also been reported. There is one frequently reported mutated region (the 'hotspot'). KCNQ1 is now believed to be the most commonly mutated gene in LQTS. The combination of normal and mutan;t KCNQ1 ŕ-subunits has been found to form abnormal I KS channels, hence mutations associated with the KCNQ1 gene are also believed to act mainly through a dominant-negative mechanism (the mutatnt form interferes with the function of the normal wild-type form through a 'poison pill' type mechanism) or loss of function mechanism. Even in the case of carriers of teh same mutaion, it is currently unknown why there are significant clinical phenotype variations in LQT1 patients. This question could be answered by increasing the number of patient genotypes studied. LQT1 patients expereince a majority of their cardiac events (62 p.c.) during exercise, and only 3 p.c. occur during rest or sleep. Of the patients who experienced cardiac events while swimming, 99 p.c. were LQT1. Auditory stimuli are rare and occur in only 2 p.c. of patients. However, both lethal and non-lethal events followthe same pattern.
