Drug interactions are an important issue in medical therapy of hypertension. They can act on each phase of drug interference with the human organism. In the case of orally administered drugs, pharmacokinetic (drug absorption) and pharmacodynamic phases (drug - target tissue interaction) are of paramount importance. Drug interactions during absorption may decrease bio-availability, while distribution is strongly influenced by binding to serum proteins, which in turn is influenced by some other drugs. We present a review of literature concerning interactions of angiotensin converting enzyme inhibitors (ACEI) - a mainstay of antihypertensive therapy of today - with other agents. The pharmacokinetic interacitons of ACEI are rare. In practice we mostly find pharmacodynamic interactions, most of which (those between antihypertensive agents) are beneficial, and make a combined therapy feasible especially with smaller-than-usual doses. This in turn increases the efficacy of antihypertensive therapy.
