    Streszczenie angielskie: Ethanol may increase the production of reactive oxygen species (ROS) in the liver, which results in the development of alcoholic liver disease (ALD). Cytokine-acitvated blood leukocytes may also participate in this process. The aim of our study was to evaluate the oxidative stress level in the blood of patients with alcoholic liver cirrhosis. Blood neutrophils from 16 patients with compensated alcoholic liver cirrhosis and 28 patients with decompensated were evaluated for their ability to produce superoxide anion and hydrogen peroxide spontaneously and after PMA induction, in comparison to controls (16 healthy persons). Serum catalase (CAT), superoxide dismutase (SOD) and glutathione peroxidase (GPx) activity were also measured in both groups of patients and in the controls. The neutrophils from patients with compensated liver cirrhosis spontaneously produced a normal level of Oz- and a high level of H2O2, but exhibited a defect in PMA-induced O2- production. The neutrophils from patients with decompensated liver cirrhosis spontaneously produced more O2- adn H2O2 than the controls, but the PM response was weak. There were no changes in the expression of GPx, buth enhanced SOD activity was observed in the serum of patients with decompensated alcoholic liver cirrhosis. Increased CAT activity was detected only in serum from patients with compensated liver cirrhosis. These data point to oxidative stress in the blood of patients with decompensated alcoholic cirrhosis, since increased resting production of ROS in neutrophils was not accompanied by increased GPx and CAT activity in serum.

    Streszczenie polskie: Apoptoza i nekroza są morfologicznie różniącymi się formami śmierci komórki, które występują w wielu różnych chorobach wątroby. Artykuł ten stanowi przegląd wiedzy dotyczącej morfologicznych i biochemicznych zmian w komórkach ulegających apoptozie oraz mechanizmów regulujących przebieg apoptozy. Omówiono również mechanizmy uczestniczące w generowaniu sygnałów proapoptotycznych wewnątrz komórki pochodzących ze środowiska komórki wątrobowej. Cholangiocyty są nieustannie narażone na działanie czynników genotoksycznych, takich jak sole żółciowe, oraz często na czynniki towarzyszące stanowi zapalnemu. Z kolei indukcja izoenzymu P450 2E1 przez etanol prowadzi do wzrostu wytwarzania rodników tlenowych i peroksydacji lipidów stanowiących sygnał do apoptozy hepatocytów. W wirusowych, autoimmunologicznych i alkoholowych zapaleniach wątroby występuje apoptoza hepatocytów indukowana działaniem cytotoksycznych limfocytów. Także cytokiny, w tym TNF i TGF-á, wiążąc się ze swoistymi receptorami, mogą indukować apoptozę komórek wątrobowych.

    Streszczenie angielskie: Apoptosis and necrosis are morphologically distinct forms of cell death which occur in a wide variety of liver diseases. In this article we review the current knowledge concerning morphological and biochemical changes in cells undergoing apoptosis and intercellular pathways regulating apoptosis. Apoptosis occurs in the cholangiopathies, viral hepatitis and in alcoholic liver diseases. The mechanisms involved in generations of apoptosis signals inside and outside of liver cells are described. Cholangiocytes are continuously exposed to a variety of genotoxic insults, such as chronic inflammation and hydrophobic bile acids. Induction of cytochrome P450 2E1, the alcohol-inducible cytochrome P450, leads to increased release of reactive oxygen species and lipid peroxidation, which in turn may be a signal leading to cell apoptosis. In viral, autoimmune and alcohol-related hepatitis, cytotoxic lymphocyte-mediated pathways of apoptosis are activated. Moreover, cytokines such as tumour necrosis factor (TNF) and tumour growth factor beta (TGF-á) acting through their receptors can induce apoptosis in liver cells.
