    Streszczenie angielskie: Background. Of all the parameters reflecting the activity of the autonomic nervous system, only the R-R interval has been shown to influence some haemodynamic parameters of left ventricular (LV) filling. Aim. To assess the relationship between heart variability (HRV) indices and parameters of LV diastolic function in the various stages of arterial hypertension. Methods. The study gorup consisted of 74 patients (57 patients with untreated hypertension and 17 healthy volunteers) who were divided into four groups: group I - 17 healthy volunteers, group II - 11 patients with "white coat" hypertension, group III - 25 patients with established hypertension without LV hypertrophy, and group IV - 21 patients with established hypertension with LV hypertrophy. Doppler echocardiography was used to assess the mitral flow by caluculating the peakvelocity of early (E) and late (A) diastolic filling as well as E/A ratio. Frequency-domain HRV components (LF and HF) were calculated from the 5 min CG recordings obtained in a supine position Results. The combined analysis of groups I and II revealed a significant correlation between E and InHF (r = 0.44, p = 0.021) as well as E/A and InHF (r = 0.56, p = 0.002). In group III a significant inverse correlation between A and InLF (r = 0.8, p 0.0001), A and InFH (r = 0.065, p 0.000) and a positive correlation between E/A and InLF (r = 0.5, p = 01) were found.     Streszczenie polskie: Zmienność ciśnienia tętniczego krwi odgrywa ważną rolę jako czynnik prognostyczny rozwoju wtórnych zmian narządów w przebiegu nadciśnienia tętniczego. Stopień uszkodzeń jest związany ze zmiennością ciśnienia tętniczego krwi, niezależnie od średnich wartości ciśnienia, określonych w 24 godzinnych monitorowaniu ciśnienia tętniczego. Zmienność ciśnienia tętniczego krwi wyrażona odchyleniem standardowym należy traktować jako wskaźnik powikłań narządowych takich jak: zmiana geometrii lewej komory serca z upośledzeniem jej czynności, remodeling naczyniowy, powikłania nerkowe objawiające się w początkowym okresie mikroalbuminuria, nasilenie zmian miażdżycowych oraz zwiększona śmiertelność. Rozbieżne wyniki badań skłaniają do dalszych klinicznych badań oraz do dalszej dyskusji i obserwacji.

    Streszczenie angielskie: Arterial blond pressure variability plays an important role as a prognostic factor of the development of secondary organ changes in the course of arterial hypertension. The degree of damages is connected with arterial blond pressure variability independently of mean blond pressure values determined in 24-hour arterial blond pressure monitoring. Arterial blond pressure variability shown by the standard deviation should be treated as an index of organ complications such as the change of left ventricle geometry with the impairment of its function, vessel remodelling, renal complications appearing in the first stage as microalbuminuria, intensification of atherosclerosis changes and increased mortality. The divergent results of studies require further clinical frials and further discussion and observations.

    Streszczenie angielskie: Background. Patients with essential hypertension (E) and left ventricular hypertrophy (LVH) have an increased risk of cardio-vascular complciatons. Alterations in the autonomic nervous system (ANS) activity may play a role in the development of serious cardiac arrhythmias and mortality in these patients. Aim. To examine the activity of ANS in patients with established EH in relation to the presence of LVH. Methods. The study group consisted of 70 subjects: 50 patients with untreated EH (mean age 44.4 ń 12.7 years, 28 patients without LVH an 22 with LVH), adn 20 age-matched healthy volunteers. ANS reactivity was assessed using heart rate variability (HRV) analysed during tilt table testing at 60ř. The following 5-min time-periods were analysed: (A) before tilting (supine position), (B) initial period of tilting, (C) lst 5 min of tilting, and (D) immediately after completion of tilt test ater return to supine position). Resutls. Patients with EH withoud LVH had a higher increase of LF/FH values (period B versus period C) compared with controls (p 0.05). The change from tilt to supine position caused signifcantly lower change in InHF values in patients with EH and LVH than in controls (p 0.01) or patients with EH withoud LVH (p 0.005). In patients with EH and LVH a singificant correlation between HF and LV mass/neight (r = 0.5, p 0.01) was noted.
