    Streszczenie polskie: Cel pracy. Analiza biochemiczna szwów metalowych służących do zespoleń mostka po zabiegach kardiochirurgicznych. Materiał i metodyka. W naszej pracy przeprowadziliśmy badania biomechaniczne: szwu pojedynczego, szwu typu "Z" oraz splecionego i wielokrotnie skręconego (SWS). Oceniliśmy odkształcenie w fazie sprężystej (cykliczne obciążenie 100 N, 200 N), granicę plastyczności i granicę całkowitego zniszczenia szwu. Taki sam cykl badań przeprowadziliśmy na próbkach ze wzmocnionym miejscem skręcenia. Wyniki. W fazie sprężystej najmniejszemu przemieszczeniu podczas obciążenia ulegał szew pojedynczy, umiarkowanemu szew typu "Z", a największemu szew SWS. Różnice pomiędzy szwem pojedynczym i SWS były znamienne statystytycznie. Granica plastyczności i całkowitego zniszczenia zespolenia wynosiła kolejno: dla szwu pojedynczego 355 i 855 N, dla szwu typu "Z" 400 i 891 N, dla SWS 430 i 880 N. Znamiennie statystycznie różniła się tylko granica plastyczności szwu pojedynczego w porównaniu ze szewm typu SWS. Po wzmocnieniu miejsca skręcania poprawiły się granice plastyczności i całkowitego zniszczenia we wszystkich typach szwów. Relacje pomiędzy poszczególnymi rodzajami szwów pozostały niezmienione. Wnioski. Dla mostków o prawidłowej strukturze szwy pojedyncze zapewniają dobrą stabilność zespolenia, a ich nieco mniejsza wytrzymałość może być wyrównana poprzez zwiększenie liczby użytych szwów.     Streszczenie angielskie: Aim of the study. Biomechanical assessment of wire sutures for sternal closure in cardiothoracic surgery. Material and methods. The following wire closures were studied: straight, figure-of-8 and interlocking multi-twisted (IM) sutures. We determined displacement in the elastic region (repetitive loading 100 N, 200 N), plastic limit and load of closure rupture. Twisting wire ends were reinforced by glue and the same procedure was performed. Results. Straight wires had less displacement at each, figure-of-8 had medium and interlocking multitwisted presented most displacement within the elastic range of the specimen. Differences between straight and interlocking multi-twisted sutures achieved statistical significance. The plastic limit and rupture of closure were as follows: for straight closure 355 N and 855 N, for figure-of-8 400 N and 891 N, for IM 430 and 880 N, respectively. Plastic limit between straight interlocking multitwisted closure demonstrated statistical significance. After strengthening the twisting wire ends plalstic limit and rupture of closure parameters increased in case of all suture types. Suture relationship turned out to be similar. Conclusions. Straight wires provide good fixation stability for the durable sternum thus lesser strenght can be recompensated by an increase in the number of used sutures.     Streszczenie angielskie: Background. Vein graft failure occurs in 15 p.c. of patients during one year after coronary by-pass grafting. The mechanisms of graft occlusion and preventive methods have not yet been well established. Aim. To test the hypothesis that the use of originally developed extremal dacron mesh affects the proliferative and degenarative changes in vein grafts implanted to the cliterial circulation. Methods. In 21 sheep standard cephalic vein graft or hybrid graft consisting of cephalic vein covered with extermal darcon mesh, were implainted to the cartoid artery. The histological and immunocystochemical characteristics of the histologic specimens harvested at various time points from implaimentation were compared. Results. In both types of grafts a gradual intimal and medial thickening was observed. Five days after implantation the mean intimal thickness in the hybrid grafts was 19.2 ń 10.7 um vs 24.4 ń 6.5 um in the non-stented graft, after 9 days - 22.3 ń 5.8 um vs 26.2 ń 10 um, after 14 days - 22.6 ń 7.6 um vs 52.2 ń 15 um, after 6 weeks - 36.6 ń 20.7 um vs 89.9 ń 31 um, after 8 weeks - 56.6 ń 30.8 um vs 103.6 ń 28 um, after 10 weeks - 58.3 ń 20.6 um vs 132.5 ń 31.6, and 12 weeks - 72 ń 21.5 um vs 244 ń 99.8 um, respectively (p 0.001 for all comparisons). The histological assessment in the light microscopy revealed progressive degeneration of the vein wall markedly more pronouced in grafts without rather than extermal stenting. Conclusions. The use of external stenting in a form of dacron mesh covering vein graft implainted in the arterial circulation reduced graft wall hypertrophy, slows down the speed of intimal thickening and inhibits cellular proliferation in the graft wall.
