    Streszczenie angielskie: An aim of study was to evaluate an efficiacy of associated treatment: tracheobronchial tree lavage with natural surfactant solution (SLL) together with nitric Oxide inhalation (INO) in therapy of severe meconium aspiration syndrome (MAS) with persistent pulmonary hypertension (PPHN). Analysed material consisted of 11 neonates: group A (n = 6) - treated with SLL and INO, group B - control (n = 5) - treated with INO. Criteria of inclusion: maturity 35 weeks of gestation, presence of fluid containing meconium below vocal cords, respiratory insufficiency demanding mechanical ventilation (FiO2 4.0), radiological changes, echocardiographic features of pulmonary hypertension. Criteria of including into groups - time after delivery less than 6 hours. Method. SLL was performed with natural surfactant solution (Survanta) in concentration of 5 mg of phospholipids (1 ml 0.9 p.c. NaCl, a dose of 15 ml of solution per 1 kg). Initial NO dose was 20 ppm. Analysed parameters: IO, PaO2, (before treatmetn and after 1, 4, 12, 24 hrs), mean pulmonary artery pressure (before treatment and after 24 hrs), IMV and hospitalization times, number of deaths. Results. In group A (mean values ń SD) before treatment, IO - 24.3 (7.37), PaO2 - 50.4 mm Hg (14.0), FiO2 - 0.95 (0.1); after 1 h: IO - 8.3 (3.9), PaO2 - 117.6 mm Hg (44.5), FiO2 - 0.73 (0.2); after 24 h: IO - 4.6 (2.3), PaO2 - 75.5 mm Hg (17.7), FiO2 - 0.53 (0.24); IMV time - 4.8 ń 2.8 days; hospitalizaiton time - 12 ń 8.7 days.
