Streszczenie angielskie: In this paper authors present current opinions about the possibility of amniotic membrane transplantation (AMT) for severe corneal diseases treatment. We describe background and the pioneers, unique characteristic of amniotic membrane, indications for AMT, amniotic graft preparation and preservation AMT technique. The AMT can be helpfull in the traetment of corneal diseases in which all other conventional mangement has failed.

Streszczenie angielskie: Purpose: To determine the effect of amniotic membrane transplantation in severe corneal diseases. Material and methods. 11 patients underwent amniotic membrane transplantation with severe corneal diseases. The amniotic membrane was placed on the surface of the corneal with epithelial side up. Amniotic membrane was prepared in the International Eye Bank of Lublin from human placenta after elective caesarean section according to the standard protocol. After surgery, a bandage contact lens was applied. All cases were treated with antibiotioc eye-drops and topical corticosteroids. In the dry eye cases, arfificial tears or autologous serum drops were instilled. Results: The persistent corneal epithelial defects was healed after the amniotic membrane transplantation in 2-3 weeks after surgery. Improvement of visual function and condition of cornea after therapy was observed in about 80 p.c. Conclusions: The amniotic membrane transplantation can be helpful in the treatment of persistent corneal epithelial defects and sterile ulceration in which all other conventional management has failed.

Streszczenie angielskie: In this paper we present a case of 48-year old man M. M. with severe blunt trauma of his right eye made by car boot rubber. In a patient was found haemorrhage into the anterior chamber and to the vitreous, paralysis of the pupillary sphincter, corneal epithelial defect covering half of its surface, subluxation of the lens, traumatic oedema of the retina and rupture of the choroids with subretinal haemorrhage. On examination visual acuity was RE, hand motions. Patient underwent operation of subluxated lens with pars plana virectomy, and scleral fixation posterior chamber IOL implantation. Two weeks after the operation intraocular pressure (IOP) attained higher level but we managed to lower it by using anti glaucoma medicines applying topically. After eight weeks patient was again taken to hospital because despite of local and general treatment (IOP) did not normalize and also from several weeks there appeared big, unhealed corneal defect. Patient underwent simultaneously deep sclerectomy with implant and amniotic membrane transplantation on the surface of the cornea with epithelial side up. After the operation IOP normalized and after 2-3 weeks transplanted amniotic membrane became more transparent and the epithelial defect healed. Visual acuity was 5/16. Good anatomical and functional result achieved in the described case point to the suitability of applied operative method.
