    Streszczenie angielskie: Aim: (A). To delineate the trends in primary cesarean birth rates at our center and in the industrialized countries. (B) To summarize our experience with specific immediate and long term risks of repeat cesarean birth. Design: Shaare Zedek Medical Center perinatal database and national reports of primary cesarean birth rates (Israel, Europe and USA). Our center's experience (1990-2001) with the immediate and long-term risks of repeat cesarean: intensive care unit admissions, massive transfusion and peripartum hysterectomy. Descriptive statistics and comparisons with student t test, Mann Whitney's when appropriate and Pearson's corelation coefficient. Logistic regression model built for the significant risk factors; p 0.05 considered significant. Results: (A). Cesarean delivery rate has reached 16.3.0 p.c. in Israel and 19 p.c. in the UK. Shaare Zedek Medical Center has maintained a stable cesarean rate of 9.7 p.c. Southern Ireland is the only country in western Europe that reports a stable cesarean delivery rate of 9 p.c. In the USA between 1989-1996, cesarean rate declined by 8 p.c. (from 22.8 p.c. to 20.7 p.c.) after which a continuous rise occurred to 22.9 p.c. Despite the rising trend of cesarean delivery there has been no improvement in maternal and perinatal mortality. (B). From 61,682 deliveries, we identified 48 admissions to the intensive care unit (ICU) (0.8:1000 live births): 34 (71 p.c.) were due to obstetrical complications; 94 p.c. of the patients admitted to the ICU had had at least one previous cesarean section; more than two thirds (70.6 p.c.) that had had a cesarean delivery in the index pregnancy required additional attention...
