Psychological processes of learning may lead to biological changes in brain synapses, as was shown by Eric Kandel. Nobel laureate for 2000. Factors connected with learning (for instance, psychotherapeutic procedures) can modify the expression of genes occurring in brain cells. Current neurobiological research make it possible to explain some theoretical concepts and mechanisms od action of psychotherapy on the ground of basic medical sciences. Experimental and clinical studies point to a prominent role for early untoward life experiences in brain development and vulnerability for psychiatric disturbances. Neuroimaging studies demonstrated the existence of unconscious processes in both cognitive and emotional functions. In several studies, changes in brain function or in the biology of whole organisms were measured under the influence of psychotherapy, It was found that in such illnesses as depresion or obesive-compulsive disorder, similar changes in brain function, as measured with neuroimaging methods, were observed either aftr pharmacotherapy or psychotherapy. The attempts have been also made to interpret psychotherapeutic mechanisms in the light of current concepts of memory processes, functioning of brain circuits or neurobiological theories of character and temperament.

In the article, clinical symptoms of psychiatric complications occuring during treatment with glucocorticoids were described, and therapeutic guidelines were suggested. Psychiatric symptoms are observed in approximately 25 p.c. of patients receiving glucocorticoid therapy. Depression, mania and mixed states are the most frequent, psychotic symptoms and cognitive impairments (including delirium) are but less frequent. Associations between clinical factors (i.e. age, sex, past psychiatric story, medical condition, the dose of glucocorticoids) and the frequency and the severity of psychiatric complications were discussed. When psychiatric disturbances occur, the reduction or discontinuation of glucocorticoids should be taken into account. Treatment of psychiatric symptoms is needed when psychiatric disorders is severe or the patient is suicidal or agitated. In such cases the use of neuroleptics, antidepressants, normothymic and other drugs, as well as electroconvulsive therapy was discussed. The outcome is generally good, the majority of patients make a good recovery, over 90 p.c. of patients restored to health within 6 weeks of the onset of treatment.

In recent years a number of obsessive-compulsive disorders and tics following streptococcal infection have been distinguished on the grounds of clinical observations. In these cases various psychopathological symptoms develop following interaction of autoantibodies with neurons of basal ganglia in the brain. The article presents new data concerning etiopathogenesis, clinical symptoms, and treatment of these disorders.

Wstęp: Niejasny pozostje wpływ czynników klinicznych na poprawę w zakresie biochemicznych wykładników funkcji wątroby po okresie nadużywania alkoholu. Cel pracy: Ocena wpływu tych czynników na zmiany wartości testów wątrobowych po 4 tygodniach abstynencji. Materiał i metody: Zbadano 160 mężczyzn z zespołem zależności alkoholowej (wg ICD-10). Na początku obserwacji po 4 tygodniach kontrolowanej abstynencji oznaczano u nich stężenie bilirubiny i poszczególnych frakcji białkowych osocza oraz aktywności GGTP, AspAT i AIAT. Wyniki: Po 4 tygodniach obserwacji odnotowano znamienny spadek średnich poziomów bilirubiny, GGTP, AspAT, AIAT i wskaźnika DeRitisa. Stwierdzono, że zarówno wartości badanych testów wątrobowych na początku okresu obserwacji, jak i ich poprawa po 4 tygodniach abstynencji wykazywały związek ze stężeniem tyreotropiny (TSH) na początku obserwacji, częstotliwością spożywania alkoholu w ciągu 90 dni przed włączeniem do badania, z występowaniem uzależnienia alkoholowego w wywiadzie rodzinnym, a także z wiekiem początku uzależnienia od alkoholu, czasem jego trwania i jego głębokością, ocenianą na podstawie występowania padaczki alkoholowej i majaczenia drżennego w wywiadzie, spadku tolerancji alkoholu, spożywania "niekonsumpcyjnych" napojów alkoholowych oraz punktacji w skali SADD i MAST. Background: The effect of clinical factors on the improevement in liver function tests during alcohol abastinence period is still not clear. The aim of the study was to determine factors affecting the changes in liver function tests values after four weeks of alcohol abstinence period. Material and methods: 160 alcohol-dependent male patients (according to ICD-10) were studied. In all patients at the beginning of the study and after 4 weeks of controlled abstinence the estimation of bilirubin and plasma protein fracations as well as ç-glutamyltranspeptidase (GTP), aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities was made. Results: After 4 weeks of abstinence a significant decrease of bilirubin, GTP, AST, ALT and DeRitis index values was found. The values of liver function tests at the study beginning and their changes after 4 weeks of abstinence had relation to the duration of abstinence period, thyreotropin (TSH) concentration at the study bewginning, the frequency of alcohol consumption during 90 days before the study, family history of alcoholism, age of alcohol dependence onset, time of alcohol dependence duration and its severity estimated on the basis fo the presence of withdrawal epilepsy and delirium tremens in anamnesis, decrease in alcohol tolerance, "non-consumptive" alcohol beverages drinking, as well as the results of Short Alcohol Dependence Data (SADD) scale and Polish version of Michigan Alcoholism Screning Test (MAST). ...

Investigations of cerebral blood flow and metabolism provide interesting data concerning specific brain regions involved in depression. The objective of this paper is to present to Polish readers results of studies using PET in patients with depression. Provocation of transient sadness in healthy subjects during clinical experiments was associated with increase of metabolism in limbic and paralimbic regions and decreased metabolism in dorsolateral prefrontal cortex. During depression metabolism of dorsolateral prefrontal cortex (DLPF) was decreased but metabolism of subgenual cingulate was increased. After recovery, normalisation of metabolism, that is increase in DLPF was observed in patients treated with antidepressive drugs as well as psychoterapy. These data point to the role of frontal-thalamic-limbic circuits in regulation of emotions and pathogenesis of depression.

The substantial importance of genetic factors in the etyiology of schizophrenia was demonstrated in many studies. The complex model of disease inheritance seems to be the most probable, involving epistatic interaction of many genes. Association studies are based on the analysis of the so-called candidate genes, which are coding for neurotransmitter receptors, neurotransmitter transportes and enzymes involved in their metabolism. In the present work the results of an association study of VNTR polymorphism of dopamine transporter gene (DAT) in schizophrenia are presented. This polymorphism is characterised by a different number of tandem repeats (VNTR) within the 3'-untranslated region of gene. In Caucasians the 40 base pair motif has 3 to 11 repeates, the most common (about 90 p.c.) are alleles containing 9 and 10 repeats. In the present study no association between the polymorphism studied and schizophrenia was found.

The aim of the study was to assess the impact of some clinical factors such as family history of psychiatric illness, obstetric complications and duration of the illnes on the effect of treatment with riseridone on psychopathological symptoms and deficits of working memory in patients with schizophrenia. The study involved 50 patients with schizophrenia, aged 16-50 years (mean 28 years), with duration of the illness 1-15 years (mean 4 years). The clinical and neuropsychological evaluation was performed before and after 3 months of risperidone treatment in dose 2-6 mg (mean 4,2 mg/day). Before treatment, a significantly higher intensity of psychopathological symptoms and of some cognitive dysfunctions was found in schizophrenic patients with family history of psychiatric illness and in those with a history of obstetric complications. After 3 months of risperidone treatment, no difference in the intensity of psychopathological symptoms or the performance on neuropsychological tests was observed among these groups. Patients with longer duration of the illness had less improvement in positive symptoms. The results of this study show that treatment with riseridone may lead to better compensation of psychopathology and of neuropsychological deficits originating in genetic predisposition and obstetric complications. Better effect of risperidone on psychotic symptoms is likely with the administration of the drug early in the course of the illness.

The aim of this study was to assess the effect of sertraline on psychopathological symptoms and cognitive functions and in patients with obsessive compulsive disorder (OCD). The investigated group consisted of 25 patients with OCD (12 male, 13 female) aged 17-47 (mean 29 ń 9) years, duration of illness was 1-15 (mean 4 ń 2) years. After treatmenmt with sertraline, a significant improvement in OCD symptoms measured by YBOCS and in neuropsychological "frontal" tests were observed. Litle correlation was found between the effect of sertraline on OCD symptoms and on cognitive dysfunctions. This may suggest that these two effects may be connected with different pharmacological properties of the drug. The effect of sertraline on OCD symptoms, similarly like other drugs inhibiting serotonin transporter (clomipramine, other SSRI) is associated with its influence on serotonergic system. On the other hand, the effect of sertraline on dopaminergic neurotransmission may be related to its favorable action on cognitive functions connected with the activity of frontal lobe.

The purpose of the study was to evaluate the efficacy of lithium prophylaxis in patients with bipolar affective disorder and to determine clinical differences between lithium responders and non-responders. Clinical and demographic data of 103 patients treated with lithium for at least 10 years were analysed. 24 patients had no episodes on lithium (excellent responders - ER), in 48 - at least 50 p.c. reduction of episode index (number of episodes/year) was observed, and the remaining 31 did not benefit from lithium treatment. Duration of illness before lithium introduction was significantly longer in lithium non-responders. Lithium responders had significantly higher episode index before lithium. Percantage of excellent responders diminished during treatment. The trend toward higher lithium plasma level in excellent responders was observed. Non-responders had significantly more depressions during first year of lithium prophylaxis. Depressive episodes during first year of treatment may be a predictor of nonresponse.

Background: It has been proposed that the Borna Disease Virus (BDV) plays a role in the etiopathogenesis of psychiatric disorders. We assessed BDV seropositivity in Polish psychiatric patients and healthy controls, and the relationship between seropositivity and selected diagnostic and clinical variables. Material/Methods: Serum samples from 946 psychiatric patients with different diagnoses (ICD-10) and 407 psychiatrically healthy controls were assayed. The ECLIA method was used, which enables the assessment of two anti-BDV antibodies: anti-p24 and anti p-40. Data were also collected on diagnosis, age, age at onset, and place of residence. Results: Only anti-p24 antibodies were found. The seropositivity rates were: 2.4 p.c. and 1.0 p.c., respectively, in patients and controls (p = 0.1). A significant difference between patients and controls was observed in mental retardation and affective-anxiety spectrum disorders. Seropositivity did not show any association with demographic variables, but it was elevated in patients with recent onset of disease vs. remote onset of disease (10.2 p.c. vs. 1.6 p.c.; p = 0.0003). Conclusions: Significantly higher anti-BDV seropositivity was found in Polish psychiatric patients with affective-anxiety spectrum disorders and mental retardation than in controls. The association between recent onset disorders and higher anti-BDV response, in the light of recent reports on circulating immune complexes of BDV antigens and antibodies, warrants furhter studies on the longitudinal course of humoral response to BDV.

Recently a functional 30 bp variable number of tandem repeat polymorphism (VNTR) in the promoter region of the MAO-A gene was identified, which was shown to be associated with MAO-A transcriptional activity. Variation in the number of repeats (3 - 5) displayed a 2.7 - 4.8 fold increased transcription activity for allele containing four or five repetitive elements than allele containing three repectitive elements. In the present study we investigated this polymorphism in 78 male and 122 female patients with bipolar and unipolar affective disorders, and in 221 control subjects. There were significant differences in the alleles and genotypes frequencies between the group of females with bipolar affective illess compared to the controls and compared to the unipolar female subjects. No differences were found in genotype distribution between unipolar female patients and control group and between bipolar and unipolar male subjects comparing to the controls. We suggest that the association may exist between uVNTR polymorphism in MAO A gene and bipolar affective disorders in women.

There is evidence for an association between allelic wariants of the monoamine transporter genes and the temperamental personality traits. Recent findings show that also an interaction of the different genes may contribute to the personality factors. Character dimensions of personality, which theoretically depend mainly on the ontogenetic development, were suggested to be under the genetic influence as well. We studied the association between personality dimensions measured with Temperament and Character Inventory (TCI) and polymorphism of the dopamine (DAT), norepinephrine (NET) and serotonin (5-HTT) transporter genes. The sample studied consisted of 128 healthy volunteers: 76 females and 52 males, mean age 23.8 ń 1.7 years, derived from two cities in the western part of Poland. We found statistically significant association between character dimension of Self-Directedness (SD) and NET polymorphism. There was a statistically significant effect of the interaction of NET and DAT polymorphisms on the dimension of Self-Transcedence (ST). The temperamental dimension of Reward Dependence (RD) was associated with interaction of 5-HTT and DAT polymorphisms. Character dimensions of personality, Self-Directedness and Self-Transcendence were associated with genetic differences. Interaction effect of the transporter polymorphisms was demonstrated in the Self-Transcendence and Reward Dependence dimensions. These findings warrant further study on the interplay of the different genes contributing to the complex traits.

Aim: Several studies have shown that depression is associated with cognitive deficits, however, it is not clear if cognitive impairment persists upon recovery. The authors' goal was to examine a neuropsychological performance in, unipolar depressive female patients during remission. Methods: The Wisconsin Card Sorting Test (WCST), the Verbal Fluency Test (VFT) and the Giessen Maze Test were administered to 21 remitted female patients and matched 17 control subjects. Al patients had been for at least 6 months on maintenance treatment with antidepressants before the study. Results: Patients achieved worse results than the control group on the Giessen Maze Test. No differences between both groups in WCST and VFT were noticed. Conclusions: These results point to deficits of visuospatical cognitive functions mostly related to the right hemisphere in unipolar patients which persisted despite of long-term remission.
