    Streszczenie angielskie: Cellular immune responses can destory cancer cells, achieving the cure of experimental malignancies. An exapanding wealth of knowledge on the molecular basis of how to prime and amplify a T cell response has fueled a number of strategies successful at treating estabilished tumors (rather than merely preventing tumor grafting). The most efficacious approaches operate at different stages, including: 1) priming the immune response using tumor antigen-expressing dendritic cells or tumor cells transfected with genes that render them immunogenic, 2) sustaining and amplifying immunity using agonistic monoclonal antibodies against costimulatory molecules or immune-potentiating cytokines, and 3) eliminating mechanisms that self-regulate the strenght of the immune response, such as inhibitory receptors or regulatory T cells. A rational combination of such approaches holds great hope for cumulative and synergistic effects, but there is also evidence that they can open the flood-gates for unwanted inflammatory reactions. The next decade can be envisioned as the time when the first reproducibly efficacious combination regimes for cancer immunotherapy will become available and videly used in the clinic, as clinicians learn the best strategies and try to harness their potentially damaging effects.

    Streszczenie angielskie: To face alcohol-related problems at a Primary Care setting isn't easy because of many barriers. Among them, an insufficient training negatively influences the perceived self-efficacy and the therapeutic commitment. This article recalls the WHO recommendations for its adoption by faculties for general practitioners and suggests the key components (knowledge, skills and attitudes) and strategies (didactic lectures, practical training and promotion of attitudes) for training programmes aimed at general practitioners. Some support materials and the promotion of an easy "4-A plan" (ask, assess, advice, assistanticipate) are suggested as contents of the training programme, whereas the learner-centred principle and the "training trainers" model are strongly recommended for the programme's design.

    Streszczenie angielskie: Background: Primary neuroendocrine tumors of the lung are uncommon neoplasms. Of those found, the vast majority occur in the distal trachea and proximal larger airways. The risk of metastasis depends on the histologic subtype of the tumor. Case report: Our patient is a 46-year-old woman who developed a cough and presented with hemoptysis. Further evaluation revealed a two-centimetr lung mass in her left lower lobe. The mediastinum was radiographically negative. A lung resection was performed that revealed a well-differentiated typical carcinoid tumor. Given its size and nodal status the tumor was assigned a T1NoMx class or Stage 1 malignancy. Conclusions: The atipical location of this uncommon pulmonary neoplasm raises sailent issuies regarding the appropriate evaluation of indeterminant pulmonary nodules. The routine use percutaneous lung biopsy with its small sample size could lead to an error in diagnosis, as differentiation between small cell lung cancer and neuroedocrine tumors can often be difficult. We belive the optimal approach to treatment of a solitary pulmonary nodule in a patient with a radiographically negative mediastinum should be flexible bronchoscop, and if not contraindicated, open or VATS resection of the nodule for definitive diagnosis and treatment.
