    Streszczenie angielskie: Essential features of endometriosis involve interactions with extracellular matrix (ECM). Recent data emphasize the important role of ECM proteins in the regulation of T cell function. The aim of this study was to determine activated T cell adhesion to ECM proteins in infertile women with endometriosis. Nine women with endometriosis diagnosed by laparoscopy according to the Revised American Fertility Society classification and ten normal healthy women with a previous successful pregnancy outcome were studied. We investigated phorbol acetate myristate (PMA) or phytohemaglutinin (PHA) activated peripheral blood T cell adhesion to the following proteins of ECM: collagen IV (C-IV), elastin (E) and fibronectin (Fn). In addition, CD4, CD8, CD29, CD45RO exspression on peripheral CD3+ T cells were studied using flow cytometry. We determine that PHA-activated T cell adhesion to C-IV and Fn are significantly higher in infertile women with endometriosis when compared to normal healthy women (P 0.05). No significant differences were noted in T cell's surface antigens expression between study groups. Our data suggest the existence of disturbed T cell-ECM interactions in infertile women with endometriosis. Further studies are needed to determine the role of these abnormalities in the pathogenesis of endometriosis.

    Streszczenie polskie: Karboksyterminalny propeptyd kolagenu typu I (PICP-carboxy-terminal propeptide of type I procollagen) i stabilizowany pirydynoliną karoboksyterminalny telopeptyd kolagenu typu I (ICTP - cross-linked carboxy-terminal telopeptide of type I collagen) są markerami odpowiednio rozpadu i synetzy kolagenu typu I. Ich stężenia w surowicy krwi odzwierciedlają tempo przemian tego białka w organizmie. Oznaczono stężenia PICP i ICTP w surowicy kobiet przyjmujących doustne środki antykoncepcyjne zawierające 20 ćg etinylestradiolu i 150 ćg desogestrelu (Mercilon) lub 30 ćg etinylestradiolu i 150ćg desogestrelu (Marvelon) przez 6 miesięcy. Stwierdzono znamiennie niższe średnie stężenie PICP po 3 miesiącach przyjmowania preparatu zawierającego 20 ćg etinylestradiolu i 150 ćg desogestrelu, które jednak po 6 miesiącach nie różniło się istotnie od wartości poczatkowej. U kobiet przyjmujących preparat zawierający 30 ćg etinylestradiolu i 150 ćg desogestrelu nie zaobserwowano znaczących zmian stężenia PICP. Średnie stężenia ICTP nie uległy istotnym zmianom w obu grupach. Wyniki wskazują na przemijające zmniejszenie syntezy kolagenu typu I u kobiet przyjmujących środki antykoncepcyjne zawierające 20 ćg etinylestradiolu. Żaden z obu badanych preparatów nie wpływa znacząco na degradację kolagenu typu I w organizmie kobiet podczas pierwszych sześciu miesięcy przyjmowania.

    Streszczenie angielskie: Objectives: To investigate serum concentrations of the carboxy-terminal of type I procollagen (PICP) and cross-linked carboxy-terminal telopeptide of type I collagen (ICTP), reflecting the rate of synthesis and degradation of the parent collagen respectively, in women using monophasic oral contraceptives. Study design: PICP and ICTP were estimated in 60 women taking 20 ćg of ethinyl estradiol (EE) + 150 ćg of desogestrel (DSG) or 30 ćg of EE + 150 ćg DSG over six months. Results: Mean PICP concentration decreased in women receiving 20 ćg of EE + 150 ćg of DSG after three cycles of administration. However, after six months this value returned to a leel comparable to the initial one. In women taking 30 ćg of EE + 150 ćg of DSG the mean concentrations of PICP did not change significantly throughout the period studied. None of the oral contraceptives noticeably influenced the concentrations of ICTP. Conclusion: Investigated oral contraceptives do not significantly affect the metabolism of type I collagen, however a transient decrease in its biosynthesis may be expected during the use of formulations containing 20 ćg of EE.

    Streszczenie polskie: Analiza kliniczna wyników 100 zabiegów metodą TVT wykazała skuteczność operacji mierzoną całkowitym ustąpieniem dolegliwości u 95 proc. pacjentek. Poprawę odnotowano u 4 proc. operowanych zaś u 1 pacjentki nie stwierdzono żadnej poprawy. Śródoperacyjne uszkodzenie pęcherza moczowego odnotowano w 9 przypadkach. Zastosowanie profilaktyki antybiotykowej było skuteczne w zwalczaniu klinicznych objawów infekcji dróg moczowych u 96 proc. leczonych.

    Streszczenie angielskie: Rationale: Since its clinical debut in 1996 TVT procedure has been offering an excellent clinical tool both for patients and surgeons to treat SUI suffering women. Since the learning curve is an important factor influencing the rate of surgical complications as well as the clinical outcome of any new surgical technique we decided to analyse the first 100 cases of SUI women treated with TVT technique. Aim of the study: To analyse clinical effectiveness and complications among first 100 consecutive cases of TVT procedures. Matertial and methods: On hundred women aged from 30 to 80 years (old mean 55,6) were hospitalised in our Department from September 1999 to November 200 because of SUI as confirmed by the complete urogynecologic assessment including history and physical examinations, catheterised residual volume determination, and multichanel urodynamic testing. TVT procedure was performed as originally described by Ulmsten et al. except the fact that 43 operations were performed not under local but epidural anesthesia. For 80 patients TVT was offered as the primary surgery for SUI treatment whereas for 14 it was the second and for 6 the third or more attempt. Results: Operation was performed without any technical difficulties in all cases (mean time 25 min). Bladder injury (treated by 1 or 2 days catheterization) occurred in 9 cases, urinary retention in 13 (in 6 cases after 10-14 days tape was cut). Urinary tract infections occurred in 4 women despite intraoperative 2,0 g i,v. piperacillin prophylactics and fosfomycin trometamol 3,0 g p.o. in first postoperative day.     Streszczenie angielskie: Objective: The assessment of relationship between pubocervical collagen content and clinical results of surgical treatment of genuine stress urinary incontinence (GSUI) in women. Methods: Twenty-four women treated for genuine stress urinary inontinence were included into the study. All women underwent the same surgical procedure. The samples of pubocervical fasia were taken at the time of surgery. The contents of acid soluble, pepsin soluble, insoluble fraction of collagen, total collagen and collagen crosslinks were measured. The study of pubocervical fascia collagen metabolism included also estimation of collagen activity. At follow-up done 5 years following surgery, 20 patients reported symptoms of GSUI (study group). Four women were still without symptoms of urine leakage (control group). The biochemical parameters of pubocervical fascia did not show, statistically significant differences between cimpared groups. Conclusions: The pubocervical fascia collagen metabolism does not have impact on the results of anti-incontinence surgery.

    Streszczenie angielskie: Several studies revealed that postmenopausal decrease in skin thickness and elasticity, uterine prolapse as well as stress urinary incontinence are, at least in part, results of impaired collagen turnover. This study aimed at the evaluation of collagen content in the rectus sheath of premenopausal and postmenopausal women. Samples of the anterior layer of the rectus sheath were taken from 16 premenopausal and 7 postmenopausal women aged 31-51 years and 51-75 years, respectively, undergoing surgery due to benign diseases of the uterus or gall bladder. Collagen was extracted with acetic acid and subsequently with pepsin. The concentartion of collagen (all types) was measured using Sicrol Collagen Assay (Biocolor Ltd., Belfast, N. Irland). Significantly less collagen was extraced from the rectus sheath derived from postmenopausal as compared to premenopausal women (11.0 ń 5.5 (g/gram of wet tissue versus 21.5 ń 10.4 (g/gramof wet tissue; p=0.0033). Moreover, a negative correlation between collagen content and age of the subjects was observed (r = -0.47; p = 0.026). However, when the analysis was carried out separately for each group of women an association between the decrease in collagen content and aage was observed only in women after the menopause. Results of the study indicate that the content ofcollagen in the rectus sheath declines afetr the menopause. This effect seems to be dependant on hormonal changes related to menopause rather than on the process of ageing. The decrease in collagen content may cause faulty wound healing as well as predispose to abdominal hernias.

    Streszczenie angielskie: Objective: The oryginal tension-free vaginal tape (TVT) method, described by Ulmsten et al., routinely uses local anaesthesia during the procedure. Since the anaesthetic effect after local application of lidocaine hydrochloride was not always satisfactory we decided to introduce the spinal anaesthesia during this operation. The aim of the present study was to compare local and spinal anaesthesia with respect to their efficacy and safety in the TVT procedure. Methods: 103 women, with objectively confirmed stress urinary incontinence, were randomised into the study. Sixty-seven women were anaesthetised locally and 36 patients spinally. All TVT procedures were performed as originally described. Objective assessment of the influence of anaesthesia on intra-abdominal pressure at rest and during the cough test was done using a rectal catheter and a central venous pressure manometer. The efficacy of the TVT procedure was based on a gynaecological examination with a cough test and a three-degree subjective scale: complete cure, improvement or failure. Results: The success of the TVT procedure performed under local anaesthesia is a comparable with hat achieved under spinal analgesia (p = 0.42). The number of complications that occurred in the two groups does not differ significantly (p = 0.57). Conclusions: Spinal anaesthesia impairs the ability to cough effectively during the TVT procedure. However, the efficacy and safety of the operations performed under this type of anaesthesia are comparable with the efficacy and safety of operatins done under local anaesthesia.

    Streszczenie polskie: Cel pracy. Ocena efektywności klinicznej oraz powikłań metody TVT zastosowanej w leczeniu wysiłkowego nietrzymania moczu u kobiet. Materiałi metoda. U stu dwudziestu kobiet, z potwierdzonym obiektywnie rozpoznaniem (badanie kliniczne, kwestionariusz Gaudenza oraz badanie urodynamiczne obejmujące cystometrię i profilometrię cewkową) wysiłkowego nietrzymania moczu, przeprowadzono operację korekcyjną sposobem TVT. Zabieg TVT polega na podcewkowym umieszczeniu taśmy prolenowej przy zastosowaniu minimalnie inwazyjnej techniki. Wprowadzenie znieczulenia miejscowego (czy też podpajęczynówkowego), umożliwiło śródoperacyjne sprawdzenie poprawności wykonania operacji. Wyniki. Spośród 120 leczonych kobiet u 109 (90,8 proc.) uzyskano wyleczenie, u dziewięciu (7,5 proc.) poprawę, u dwóch zaś (1,7 proc.) operacja nie dała pozytywnego rezultatu. Nie wystąpiły żadne poważne powikłania śród- i pooperacyjne. Perforacja ściany pęcherza moczowego, która wystapiła u 9 pacjentek, nie miała negatywnego wpływu na wynik operacji. U 6 pacjentek wystąpiła pooperacyjna niestabilność wypieracza. Wnioski. Blisko dwuletnie, własne doświadczenia kliniczne z metodą TVT pozwalają stwierdzić, że ta mało inwazyjna i niezwykle prosta technicznie oepracja jest wysoce efektywna w leczeniu wysiłkowego nietrzymania moczu u kobiet. W naszej opinii powinna być roważana jako technika z wyboru do leczenia WNM u kobiet.

    Streszczenie angielskie: Objective. To evaluate clinical efficacy of TVT procedure as the treatment of stress urinary incontinence (SUI) in women. Moreover, intraoperativ as well as postoperative complications were critically analysed. Patients and methods. One hundred and twenty female patients suffering from SUI were included and underwent clinical evaluation with Gaudenz questionnaire and urodynamic tests (cystometry and urethral profilometry). TVT procedure implies the insertion of prolene tape around mid-urethra via a minimal vaginal incision. Results. Among 120 treated women 109 (90,80 p.c.) were fully recovered, 5 (75 p.c.) were improved and in 2 cases (1,7 p.c.) operation failed. No major intra-or postoperative complications occurred. Nine cases of bladder perforations were treated with catheterisations and had no negative impact on clinical outcome. Six patients developed detrusor instability de novo. Conclusions. After almost two years of clinical experiences we consider TVT procedure as a safe and effective surgical procedure for females with the SUI. Because of very high effectiveness and reproducibility accompanied by its technical simplicity TVT procedure should be considered as the opration of choice for female SUI treatment.
