Background. Angiotensin II type 1 receptor gene is one of the candidate genes in the pathogenesis of myocardial infarction (MI). Aim. To asses the association of the A1166C polymorphism in the gene locus with MI in the Polish population. Material and methods. Polymorphism was detected using polymerase chain reaction and restriction fragment length polymorphism. 166 patients who survived MI before the age of 50 were compared with 166 control subjects matched according to sex and age. Results. Callele frequency among MI survivors was higher than in the control group. Genotypes containing C1166 variant (AC+CC) occurred more frequently in MI patients (OR 3.05; 95 p.c. CI 1.91-4.85; p = 0.0028). In the study populations independent risk factors for MI were the presnece of C1166 variant of the AT1R gene and MI. This may account for increased risk of MI as well as for an increased chance to survive it's acute phase in C1166 variant bearers. However, only prospective studies may reveal the real meaning of this association.

Wczesna ekstubacja noworodków z bardzo niską urodzeniową masą ciała leczonych z zastosowaniem przerywanej wentylacji obowiązkowej (IMV) z powodu niewydolności oddychania niesie potencjalne korzyści wynikające z obniżenia częstości mechanicznego uszkodzenia krtani i tchawicy, zmniejszenia zachorowalności na wewnątrzszpitalne odoskrzelowe zapalenie płuc i posocznicę oraz zmniejszenie częstości występowania i ciężkości dysplazji oskrzelowo-płucnej. Z drugiej strony u tych noworodków relatywnie często dochodzi do niepowodzeń w odstawianiu wentylacji mechanicznej i ekstubacji wskutek częstych ciężkich bezdechów, hipowentylacji i niedodmy. W praktyce klinicznej podjęto próbę wentylacjii obowiązkowej zmniejszając jej inwazyjność przez rezygnację z intubowania tchawicy i stosowania IMV donosowo, co było bardzo łatwe w przypadku noworodków z stosowanym n-CPAP zwłaszcza w układzie respiratora, w którym wentylowano noworodka przed ekstubacją. Celem pracy była ocena skuteczności nosowej nieinwazyjnej wentylacji obowiązkowej (n-IMV) jako metody postępowania u noworodków w przypadkach, kiedy dochodzi do desaturacji wskutek zaburzeń wentylacji wynikających z zaburzeń generacji rytmu oddechowego. Atrakcyjność n-IMV dla klinicysty stoi w opozycji do obaw o napełnianie powietrzem przewodu pokarmowego, które przy świadomości naturalnej w tym okresie niewydolności wpustu żołądka przy dużym napięciu odźwiernika mogłoby być niebezpiecznym powikłaniem. Streszczenie angielskie:
Early extubation of ELBW and VLBW premature infants treated with IMV results in decreased incidence of tracheal and laryngeal injury, lowers the risk of nosocomial infection, decreases the severity and frequency of bronchopulmonary dysplasia (BPD). Due to prematurity this group of patients is especially susceptible to extubation failure because of apnoe, hypoventylation and atelectasis. In clinical practice attempt was made to provide adequate noninvasive ventilation by the use of nasal intermittent mandatory ventillation in the case of apnoe of prematurity. Advantages of noninvasive nasal IMV oppose the risk of stomach distension and regurgitation due to high tension of pylorus combined with inadequate caedia tension. The aim of the study was to evaluation of non invasive nasal IMV effectivennes along with a risk of abdominal distension caused by air trapping. 32 patients were examined during one year of studies. In all but one the use of nasal intermittent mandatory ventillation resulted in decreased incidence of apnoe of prematurity. Satisfactory levels of SaO2 and pCO2 were achieved without endotracheal tube placement, avoiding the risks of nosocomial pneumonia and bronchopulmonary dysplasia.

Celem pracy było określenie związku polimorfizmu ludzkiego genu śródbłonkowej syntazy tlenku azotu z występowaniem zawału mięśnia sercowego. Badaniem objęto 178 chorych ze świeżym lub przebytym zawałem serca oraz 136 zdrowych. W niniejszej pracy do określenia polimorfizmu badanego genu eNOS zastosowano technikę amplifikacji DNA w łańcuchowej reakcji polimerazy PCR i polimorfizmu długości fragmentów restrykcyjnych Ban II (RFLP). Obserwowano częstsze występowanie allela T i genotypów zawierających allel T w grupie badanej w wieku poniżej 50 lat w stosunku do grupy kontrolnej. Nie obserwowano podobnej zależności dla grupy badanej w wieku powyżej 50 lat. Istnieje możliwość wykorzystania nosicielstwa allela T jako genetycznego markera zagrożenia zawałem serca u ludzi przed ukończeniem 50 r.ż.

The aim of this study was an assessment of association of human endothelial nitric oxide synthase gene Ban II polymorphism with the myocardial infarction in 178 patients and 136 healthy individuals. These polymorphism were studied using polymerase chain reaction (PCR) and Ban II restriction fragment length polymorphism (RFLP). It was found that TT and GT genotypes and allele T were associated with myocardial infarction patients under 50 years. We did not observed this association for older myocardial infarction patients. Allele T might be used as a genetic marker in the prophylaxis of myocardial infarction among people under 50 years.

W badaniach analizowano związek polimorfizmu Nsil genu ludzkiego receptora insuliny z zawałem serca u 100 chorych i w 140-osobowej grupie kontrolnej. Polimorfizm badano przy użyciu reakcji łańcuchowej amplifikacji (PCR). Stwierdzono związek między allelem N2 i zawałem serca, zwłaszcza u chorych z zawałem serca i prawidłową masą ciała i/lub pozytywnym wywiadem rodzinnym.

The aim of this study was the assessment of the association of human insulin receptor gene Nsil polymorphism with myocardial infarction in 100 patients and 140 healthy individuals. The polymorphism was studied using polymerase chain reaction. An association was found between N2 allele and myocardial infarction, particularly in patients with normal body weight and/or positive family history.

Background: Diabetes mellitus type 2 results from a complex of hreditary and environmental factors. Genes encoding elements of the G-protein system are candidate genes in hypertension and obesity. Since insulin uses G-sensitive mechanisms to enhance tissue glucose uptake and vasodilatation, the GNB3 gene may be a candidate gene in type 2 diabetes. The goal of our research was to determine if the C825T polymorphism at the G-protein á3 subunit gene locus is associated wtih type 2 diabetes. Material/Methods: The study population consisted of 172 Polish patients with type 2 diabetes and 172 healthy, age- and sex-matched controls. The C825T polymorphism was detected by PCR and RFLP. Results: A higher frequency of genotypes containing the mutation (CT + TT) was observed among the diabetics than in the controls. The T825 variant occurred more often among hypertensive daibeatics (71 p.c.) than among diabetics with normal blood pressure (42.5 p.c.). The OR for hypertension in diabetic subjects bearing CT + TT genotypes was higher than in patients with the CC genotype. Overweight and obesity were not associated with teh T825 variant in either the experimental subjects or the controls. Conclusions: In this population, the T825 variant of the GNB3 gene was not associated withtype 2 diabetes itself, nor with overweight and obesity, but was associated with diabaetic hypertension. Upon confirmation of our results this variant may be useful as a genetic marker of susceptibility to hypertension and vascular complications in type 2 diabetes.
