    Streszczenie angielskie: Background: Long-term safety and efficacy of Exelon (rivastigmine) was evaluated in a multi-center open label study of 62 patients with probable mild to moderated Alzheimer's disease living in community setting. Material/Methods: The patients started treatment with 1.5 mg bid (3 mg/day) Exelon and were scheduled to receive doses of 1.5 mg bid Exelon escalating on a biweekly basis. The patients were maintained on the highest tolerated dose within the assigned dose range 1.5 - 60.0 mg bid (3 - 12 mg/day) for the rest of the study. Evaluations were scheduled at biweekly intervals for the first 8 weeks and subsequently at study weeks 12, 18 and 26. Effects of Exelon on cognition were evaluated using the mini-mental state examination (MMSE) and selected items of Alzhemier's disease assessment scale (ADAS-cog) and the staging ot the disease was measured using the global deterioration scale (GDS). Safety was monitored by physical examinations, vital signs, laboratory tests, ECG recording and by the assessment of adverse events. Resutls: 55 patients completed the study (89 p.c.). Patients treated for 26 weeks showed the mean MMSE, ADAS-cog and GDS scores close to baseline values (p = NS) with no improvement and no deterioration. Exelon was generally well tolerated with 11 p.c. of patients withdrawing due to adverse events. The most frequently reported adverse events related to the gastrointestinal tract. Conclusions: In conclusion, the study data indicate that treatment with Exelon is safe, generally well tolerated and inhibits the progression of cognitive decline in patients wtih mild to moderate Alzheimer's disease over 26 weeks of treatment.

    Streszczenie polskie: Zespoły depresyjne występujące u osób w podeszłym wieku są uwarunkowane polietiologicznie, co wpływa na szczególną charakterystykę kliniczną. Leczenie depresji jest trudne z powodu znacznej oporności na stosowane leki, dużej liczby objawów niepożądanych i powikłań. Aktualnie uważa się, że w związku z niekorzystnym profilem objawów niepożądanych trójpierścieniowe leki przeciwdepresyjne powinno się stosować jak najrzadziej. Zaleca się natomiast - jako bezpieczniejsze - podawanie inhibitorów serotoniny oraz leków działających na dwa układy neuroprzekaźnikowe.

    Streszczenie angielskie: Depressions in old age are conditioned by a huge number of different factors, which are responsible for clinical picture. Treatment of depression is difficult due to resistance to treatment, wide unwanted effects and complications. Due to unfavourable profile, TCA are not recomended. SSRI's, NaSSA, SNRI's are between much more safe drugs.

    Streszczenie angielskie: The community survey of the Warsaw district Mokotów estimated the prevalence of dementia in a random, population-based sample, stratified for age and sex. The sample of 1000 elderly (65-84 years) was screened at home with the MMSE test. All elderly under the cut-off 24-25/30 was assigned a diagnosis via the CAMDEX and the DSM-IIIR. The prevalence of dementia was estimated at 5.7 p.c. The following age-specific prevalence of dementia was found in the age groups 65-69, 70-74, 75-79, 80-84: 1.9, 5.8, 8.6 and 16.5 p.c. Rates for VD - 2.7 were higher then those for DAT - 2.3, mixed dementia- 0.5, and secondary dementia - 0.2 p.c. In the younger subgroups (65-74 years) VD was the most frequent and in the older subgroups (75-84 years) the most frequent was DAT. The rates of dementia were quite similar to those found in other European studies.
