    Streszczenie angielskie: Risk factors for deep vein thrombosis and pulmonary embolism together with pharmacological and nonpharmacological methods of their prevention were analysed. There are a lot of raports about thromoembolytic complications as well as about their prevention in perioperative period. One can however hardly find any information regarding this subject about their occurrence in ophthalmic surgery. The same refers to perioperative bleeding in a group of patients undergoing ophthalmic surgery once on thromboprophylaxis. This problem is underestimated by ophthalmologiost. New shorter surgical techniques (phacoemulsification), faster patients mobilisation, regional anaesthetic techniques are linked to lower risk of thrombosis. Nevertheless elder, frequently obese patients suffering from varices of lower extremities and circulatory diseases, operated under general anaesthesia need antithrombotic prophylaxis. The low-molecule-weight heparins seem to be the most effective. We recommend Enoxoparin (Clexane) 20 mg, Fraxiparin 7500 IU or Delteparin (Fragmin) 2500 IU supplied subcutaneously once a day. First dose should be given one day before procedure. The therapy should be continued for 2 - 3 days after operation.

    Streszczenie angielskie: Advantages and disadvantages of anticoagulant and antiplated therapy in patients undergoing elective ophtalmic operations were analysed. Giving up a long-term anticoagulant therapy is linked to increase of thromboembolic systemic complications (e.g. cerebral accidents, exacerbation of coronary artery disease, vein or artery thrombosis). On the other hand, continuation of therapy is linked to increasing of frequency of ophtalmological haemorrhagic complicatuions (e.g. retrobulbar haemorrhage, vitreous haemorrhage, chemosis, subconjunctival haemorrhage, specially when retrobulbar or peribulbar block was performed. Two solutions are proposed - either continuation of anticoagulation and application of anaesthetic methods carrying the minimal risk of haemorrhage (general anaesthesia, topical anaesthesia or sub-Tenon's block) or dicontinuation of oral anticoagulants 2 - 3 days before operation with changing treatment to low-molecular-weight heparins. Acetilosalicylic acid doesn't increase hemorrhage complications, so it may be continued in perioperative period.
