    Streszczenie polskie: Wstęp. Częstym następstwem urazu głowy jest występowanie tzw. zespołu czołowego, manifestującego się charakterystycznmi zaburzeniami neurobehawioralnymi. Brak jest jednak ekologicznego narzędzia badawczego, które wskazywałoby jednoznacznie na obecność tego zespołu. Celem pracy jest ocena autoryzowanej wersji polskiej Kwestionariusza Zachowań Osoby z Zespołem Czołowym (ang. Frontal Behavioral Inventory, FBInv), opracowanego w celu różnicowania otępienia czołowo-skroniowego od innych rodzajów otępienia. Materiał i metoda. W badaniach wzięło udział 95 pacjentów leczoncyh w ośrodkach reprezentowanych przez autorów. Badani zostali podzieleni na 3 grupy: grupa CHI, która obejmowała 39 pacjentów z pourazowym uszkodzeniem płatów czołowych, grupa FTD, która obejmowała 28 pacjentów z otępieniem czołowo-skroniowym oraz grupa osób z depresją pourazową, obejmująca 28 pacjentów, u których nie stwierdzono uszkodzenia płatów czołowych. Wyniki opracowano w oparciu o dane uzyskane od opiekunów w zakresie 24 kategorii zachowań pacjentów zawartcych w Kwestionariuszu Zachowań Osoby z Zespołem Czołowym (FBInv). Wyniki. Uzyskane wyniki wskazują na istotne różnice pomiędzy pacjentami z zespołem czołowym z grupy CHI oraz FTD a pacjentami z depresją pourazową pod względem całkowitej liczby przyznanych punktów, jak również pomiędzy pacjentami z grupy FTD oraz CHI pod względem wyników w poszczególnych pozycjach kwestionariusza.     Streszczenie angielskie: Background. A common sequella of head injury is "frontal syndrome", consisting in characteristic neurobehavioral disturbances. However, there is no ecologically valid research tool that would clearly indicate the presence of this sydrome. The goal of this article is to evaluate the authorized Polish version of the Frontal Behavioral Inventory (FBInv), used to differentiate fronto-temporal dementia (FTD) from other dementias. Material and methods. The research involved 95 patients treated at the centers represented by the authors, divided into 3 groups CHI, consisting of 39 patients with traumatic frontal lobe injuries; FTD, consisting of 28 patients with fronto-temporal dementia; and a control group of persons with post-traumatic depression without injury to the frontal lobes. The results were based on data obtained from caregivers in 24 categories of patient behavior covered by the FBInv. Resutls. We found important differences in total scores between patients with frontal syndrome from gorups CHI and FTD, as against patients with post-traumatic depression. There are also noticeable differences between patients in group FTD and group CHI in terms of scores on paticular test items. Conclusions. The FBInv in the authorized Polish version is both sensitive and specific in measuring neurobehavioral disturbances occuring in patients with post-traumatic damage to the frontal lobes.     Streszczenie angielskie: This article describes the life and accomplishements of Prof. Mieczysław Skulimowski (1930-1982), who among his many achievements was the inventor of subterraneotherapy, i.e. the utilization of the chemcial and physical properties of the air in underground environments to promote the health of patients with various kinds of ailments, including especially respiratory diseases. He designed and operated for many years an underground sanatorium in an abandoned run of the historic Wieliczka salt mine near Cracow. This facility provided the model for similar underground facilities in caverns and mines throughout Europe Prof. Skulimowski was an indefatigable ortganizer who played a vital role in establishing an important series of National Rehabilitation Symposia, the precursors of the Interantional Congresses of the Polish Rehabilitation Society. He also wrote a standard textbook on physiotherapy for medical students. He advocated a comphrehensive approach to rehabilitation, encompassing all available forms of non-invasive treataemtn, including psychotherapy.

    Streszczenie angielskie: Background: This article presents a case study of a 32-year-old Polish male patient clincally diagnosed with the MELAS syndrome, the second such patient reported in this country. Case report: The patient presents with a long history of neurological episodes, consisting of acute neurological deficitis suggestive of CVA with no evidence of focal lesion and very rapid remission of symptoms. These episodes began when the patient was 19 years old and have recurred since that time at intervals of 1-5 years. A recent psychiatric episode with unusual visual hallucinations brought the case to the attention of the present authors, who initiated further diagnostic testing. A muscle biopsy revealed ragged red fibers. Conclusions: The features in this case meet the diagnostic criteria for a clincial diagnosis of MELAS, pending confirmation of a pathogenic mutation.

    Streszczenie angielskie: Background: Executive dysfunction is one of the most destrucive sequelae of closed head injuries (CHI), often impeding or even preventing the patient's return to normal functioning. On the basis of extensive clinical testing of patients with neurobehavioral disturbances resulting from CHI, the authors propose a new typology of executive dysfunction based on the primary behavioral distinction between active ('acting without thinking') and passive ('thinking without acting') forms of executive function disorder. Material/Methods: Two patients were selected for detailed presentation. Both present with mild to moderate motor and cognitive symptoms resulting from closed head injury. The medical histories of the two patients are similar (educated professionals, mid-40s, married with children, injuries suffered in a traffic accident, 2 months in coma) except for the location of focal injuries. Results: Despite considerable progress in rehabilitation, the extent of functional disorder is disproportionately large in comparison to the degree of objective disability measured by standard instruments. It is suggested that the reason for this disparity lies in executive dysfunction. In particular, a model for executive functioning will be presented to explain why and low selective destruction of particular anatomical/functional components leads to the behvioral consequences known as 'executive dysfunction'. Conclusions: executive dysfunction is a distinct clinical syndrome which occurs in at least two distinguishable varieties, active and passive.

    Streszczenie angielskie: Background: Apraxia, broadly understood as the acquired inability to perform learned, skilled movements not caused by paresis or incomprehension, occurs with greater frequency among patients' recovering from prolonged post-trauamtic coma than has previously been reported. The present article prevents an evaluation of the effectiveness of a rehabilitation program called Semantic Priming, aimed at rebuilding praxis in the patients. Material/Methods: Out of a consective series of 51 patients with post-traumatic coma lasting longer than 4 weeks treated at the centers represented by the authors, 24 were found to have pathological scores on the Boston Test of Praxis. These patients were divided into two matched groups, of wom the experimental group received the Semantic Priming program (involving the systematic use of verbal cues to prime movement activation patterns), and the control group did not. Progress was measured by the Boston Test of Praxis and a test developed by the authors on the basis of Luria's procedure for testing praxis. Results: After 5 weeks of therapy the patients in the experimental gorup showed significantly greater improvement in all measured parameters (p 0.001). Conclusions: Apraxia in the broader sense is a more common problem among post-coma patients than heretofore recognized, and causes concrete problems in rehabilitation and adjestment to daily living. Our program based on the use of semantic priming of activation proved to be highly succesful in remediating apraxia in these patients.

    Streszczenie polskie: Celem pracy jest stwierdzenie, czy badania neuropsychologiczne dotyczące pamięci autobiograficznej i behawioralnej oraz funkcji wykonawczych mogą być podstawą do klinicznego różnicowania otępienia typu Alzhaimerowskiego (DAT) z otępieniem z ciałami Lewy'ego (LBD). W badaniach wzięło udział 34 pacjentów leczonych w ośrodkach autorów. Grupa DAT obejmowała 17 osób z klinicznymi objawami otępienia typu Alzheimerowskiego, natomiast grupa LBD to 17 pacjentów z klinicznymi objawami otępienia z ciałami Lewy'ego. Pacjentów dobierano parami pod względem wieku i płci. W badaniach oprócz obserwacji klinicznej zastosowano testy neuropsychologiczne ukierunkowane na pamięć autobiograficzną i behawioralną oraz funkcje wykonawcze. U pacjentów z grupy DAT wystąpiła znaczna dezorganizacja pamięci autobiograficznej, natomiast u pacjentów z grupu LBD zaburzenia te nie były tak duże. Łączna liczba punktów uzyskana w Teście Pamięci Zachowania Rivermead przez pacjentów z grupy LDB była ponad 2-krotnie większa niż u pacjentów z grupy DAT. Wyniki Klinicznego Testu Funkcji Wykonawczych były odwrotne: 2-krotnie większą liczbę punktów uzyskali pacjenci z grupy DAT niż pacjenci z grupy LBD. U chorych z grupy DAT oraz LBD stwierdzono zaburzenia pamięci, przy czym były one znacznie głębsze u pacjentów z grupy DAT.     Streszczenie angielskie: The purpose of our research was to determine whether neuropsychological testing in the area of autobiographical memory, behavioral memory, and executive functions can provide a basis for the clinical differentiation of dementia of the Alzheimer type (DAT) and Lewy body dementia (LBD). The reseach involved 34 patients treated at the centers represented by the authors. Group DAT consisted of 17 persons with dementia of the Alzheimer type, while Group LBD consisted of 17 patients, matched by age sex, with the clinical features of Lewy body dementia. The patients were examined using clinical observation and neuropsychological tests focused on autobiographical memory, behavioral memory, and executive functions. The DAT patients showed considerable disorganization of autobiographical memory, while the disturbances found in the LBD patients were not as drastic. The total scores earned by the LBD patients on the Rivermead Behavioral Memory Test were more than twice those of the DAT group. The results from the Clinical Test of Executive Functions were the opposite: the DAT patients earned twice as many points as the LBD. Memory disturbances were found in patients from both groups, but they were much worse in the DAT patients.
