    Streszczenie angielskie: Objective: The aim of the study was to compare therapeutic methods of Meconium Aspiration Syndrome treatment: exogenous surfactant supplementation versus broncho-pulmonary surfactat solution lavage. Material and methods: Between 1993 and 2001 15 neonates were treated at Research Institute of Polish Mother's Hospital due to Meconium Aspiration Syndrome. Studied population was divided in two groups according to treatment's method. Group I - consisted of 8 infants born in 1993 -1998. Neonates received natural surfactant supplementation. Mean GA was 39 weeks (range 38-40) and mean birth weight was 3497 g (range 2550-4100 g). Group II - included 8 infants born after 1998, who were treated with broncho-alveolar natural surfactant solution lavage (5 mg phospholipids/ml; 15 ml/BW). Mean gestational age was 39.4 weeks (range 38-40) and mean birth weight was 3287 g (range 1700-3800 g). In bothe groups we compared: alveolar-to-arterial oxygen gradient (A - aDO2) and oxygen index (OI) measured every 6 hours during first day of life and every 24 hours till 7 day of life. The total ventilation time, duration of oxygen supplementation and the time of hospitalization were also analyzed. Results: In the lavage group shorter ventilation time; 3.1 (0.5 - 7) vs 7.9 (4=-13) days as well as total oxygen supplementation period; 7.8 (0.5 - 14) vs 12.3 (5-23) and time of hospitalization; 16.1 (6-41) vs. 27 (18-40) were observed.
