    Streszczenie angielskie: Both positive and negative effects of the reform of the health care financing system are noted. Low prices offered by Sickness Funds for particular services (a bed-day, a visit) should be regarded as a negative effect of the reform. Particularly insufficient were the prices of services in some specialised psychiatric wards and in outpatient clinics. Prices in many community-based psychiatric facilities were also considerably understimated. Undoubtedly, the reform has led to positive changes in the organization of inpatient care. These changes include: further reduction of beds in large hospitals organisational structure as well marked increase in the number of psychiatric wards at general hospitals, which should be the key units of psychiatric inpatient care. Increasse in the number of day hospitals is another positive effect of the reform. The programme of psychiatric care transformation is presented mostly in the Mental Health programme. The main goal of this programme is to ensure apropriate care for the mentaly disordered people, namely comprehensive and accessible health care as well as other forms of help and support necessary for living in family and in society. This goal will be accomplished by health care and other forms of help mentioned in the. Mental Health Act and in teh Social Help Act. Community-based model of psychiatric care is teh key element of this system. Also, the Programme ststes desired accessibility rates for staff, number of beds and number of particular forms of psychiatric and alcohol treatment care. Separate rates for adult and children/youth population have been elaborated.

    Streszczenie polskie: Chemioterapia wysokodawkowa i autologiczny przeszczep szpiku (BMT) lub komórek macierzystych hemopoezy izolowanych z krwi obwodowej (PBSCT), staje się coraz częstszą opcją terapeutyczną w leczeniu ratującym u pacjentów z opornymi na leczenie lub nawrotowymi guzami zarodkowymi (GCT). Leczenie takie może dać ok. 26-57 proc. trwałych, całkowitych remisji, co stanowi pewien postęp w stosunku do tradycyjnej chemioterapii ratującej, która jest w stanie wyleczyć zaledwie 25 proc. takich pacjentów. Również w chemioterapii pierwszego rzutu, u chorych z grupy o złym rokowaniu, zastosowanie takiego intensywnego leczenia powinno dawać nawet o 20 proc. więcej wyleczeń. Takie przynajmniej są wnioski z badań klinicznych II fazy i retrospektywnych analiz porównujących pacjentów leczonych obydwiema metodami (matched-pair analysis). Jednak badania kliniczne III fazy, jak do tej pory nie potwierdziły tej przewagi - ani w przypadku leczenia ratującego, ani chemioterapii pierwszego rzutu. Jakie jest więc w tej chwili miejsce chemioterapii wysokodawkowej i autotransplantacji w leczeniu guzów zarodkowych? Autorzy starają się odpowiedzieć na to pytanie, analizując wyniki opublikowanych badań klinicznych - od pierwszych prób przeprowadzonych jeszcze w latach 70., po zakończone niedawno prospektywne, randomizowane międzynarodowe badanie III fazy IT94. Przedstawiają także własny dorobek - wyniki leczenia chemioterapią wysokodawkową u 15 pacjentów ze źle rokującymi guzami zarodkowymi, którzy ...

    Streszczenie angielskie: High dose chemotherapy supported with autologous bone marrow (BM) or peripheral blood stem cells transplantation (PBSCT) is becoming an increasing considered option for salvage treatment in patients with diesease resistant to platinium-based chemotherapy regimens or recurrent germ cell tumours (GCT). According to published data coming from retrospective matched-pair analyses or pahse I-II clinical trials such approach can produce approximately 26-57 p.c. of durable complete responses. It is a strong promise of certain progress that is going to be achieved in the closest future as we are able to cure inly about 25 p.c. of such patients with standard-dose salvage chemotherapy schedules. The next emerging problem is the treatment of so-called poor prognosis patients who are suffering from non-seminomatous GCT with: mediastinal primary tumour location and/or non-pulmonary visceral metastases and/or with defined elevation in b-HCG/AFP/LDH levels. It seems that the introduction of high dose chemotherapy as the first line treatment for this group of patients should result in a 20 p.c. Increase of cure-rate, when compared to the standard treatment. Although these conclusions can already be drawn from phases II trials, there is a strong need to support them with randomised multi-center phase III clinical trials.
