    Streszczenie angielskie: Objective. To assess whether organ-confined prostate cancer not detected on routine transrectal ultrasonography (TRUS) can be detected using threedimensional (3-D) power Doppler methods. Patients and methods. A preliminary trial of the use of TRUS with 3-D power Doppler image reconstruction was conducted in 116 patients being evaluated for lower urinary tract symptoms. Using a microbubble enhancing agent, a adequate horizontal plane allowed the simultaneous visualization and assessment of the symmetry of echogenicity and vascularization of the peripheral area of both prostate lobes, which is not possible in standard TRUS. The peripheral zone vascular asymmetry in the horizontal plane was also assessed retrospectively n those patients with pT2a,b prostate carcinoma. Results. Stage pT2a,b prostate carcinoma was confirmed in 26 patients; in this group, two independent investigators confirmed the vascular asymmetry between the left and right peripheral zone, in the horizontal plane, in 22 patients, giving a sensitivity of 85 p.c. Conclusions. Vascular asymmetry allows guided biopsy in lesions that are invisible using standard TRUS. Microbubble agents are an effective method of increasing the acoustic signals from small vessels of the prostate.

    Streszczenie polskie: Wstęp. Wraz ze wzrostem liczby urazów wielonarządowych zaobserwowano wzrost liczby urazów nerek. Celem pracy była analiza retrospektywna urazów nerek dla optymalizacji sposobów diagnostyki i leczenia. Materiał i metoda. Przeanalizowano dane dotyczące 887 urazów nerek, uzyskane na podstawie ankiet otrzymanych z 61 ośrodków urologicznych w Polsce za okres od 1995 do 1999 roku. Wyniki. Urazy tępe stanowiły 97 proc. przypadków urazów nerki. Diagnozę urazu nerki postawiono na podstawie urografii u 80 proc. chorych, a tylko w 20 proc. na podstawie badania TK. U 687 chorych rozpoznano stłuczenie nerki lub uszkodzenie kory nerki miernego stopnia. 234 chorych zakwalifikowano do leczenia operacyjnego (26 proc.). Wsród operowanych chorych u 170 wykonano zabieg usunięcia uszkodzonej nerki (73 proc. operowanych). Powikłania po leczeniu zachowawczym urazów nerki wystąpiły u 9 proc. chorych i u 5 proc. po leczeniu operacyjnym. Wnioski. Wśród wszystkich urazów nerki najczęściej występują urazy tępe (97 proc. chorych). TK z kontrastem dożylnym powinna być obecnie traktowana jako metoda diagnostyczna z wyboru, szczególnie u chorych z urazami wielonarządowymi. Chorzy z urazem nerki zbyt często kwalifikowani są do leczenia operacyjnego, a wśród operowanych najczęściej stosowaną metodą postępowania jest nefrektomia, co odbiega od standardów światowych.

    Streszczenie angielskie: Introduction. Kidney injures are frequently part of multiorgan trauma. The aim of the study was retrospective analyses of kidney injuries in order to optimize diagnostic and treatment methods. Material and method. The subject of the analyses was the survey for assessment of kidney injures treated in the years 1995 - 1999 gathered from 61 urological departments. Results. 887 kidneys injure cases were analysed. 97 p.c. were cases of blunt trauma. Most of the cases were classified as renal contusion and small parenchymal, damages in 687 cases. IVU was performed in 80 p.c. of cases and CT only in 20 p.c. Surgery was carried out in 234 patients (26 p.c.). Nephrectomy turned to be the most frequent surgical treatment in Poland. It was performed in 170 cases, i.e. 73 p.c. of operated patients. Complication ration occurred in 9 p.c. of patients after conservative treatment and in 5 p.c. after surgery. Conclusion. Blunt kidney injuries occured in 97.8 p.c. of all kidneys injures in Poland. In Poland patients with blunt kidney injuries are often treated surgically, and nephrectomy was found to be the most frequent surgical procedure. CT with intravenous administration of contrast should be considered as a diagnostic method of choice in cases of kidney injury, especially for patients with multiorgan trauma.

    Streszczenie polskie: Poddano analizie przyczyny, metody diagnostyczne i sposoby zaopatrywania moczowodów, na podstawie wyników ankiet z zakresu traumatologii urologicznej, jakie rozesłano do wszystkich oddziałów urologii w Polsce. Odpowiedzi otrzymano od 61 ordynatorów. Ankieta dotyczyła lat 1995 - 1999. Stwierdzono 45 urazy moczowodów, a wśród nich 31 urazów otwartych (7 proc.), 81 zamkniętych (18 proc.) i 340 (75 proc.) jatrogennych. Uszkodzenia jatrogenne najczęściej spowodowane były operacjami ginekologicznymi - u 72,6 proc. chorych. Urazom moczowodów najczęściej towarzyszyły urazy: nerek, jelita cienkiego, jelita grubego i pęcherza moczowego. Jatrogenne uszkodzenie moczowodu rozpoznano na stole operacyjnym jedynie u 31 proc. chorych. U 47,1 proc. chorych wykonano urecystoneostomię, operację Boareigo wykonano u 25 proc. chorych, a zespolenie "koniec do końca" - u 20,4 proc. chorych. Autorzy proponują, aby w przypadkach wątpliwych przyjąć jako zasadę przedoperacyjną konsultację urologiczną z zasondowaniem moczowodów, wychodząc z założenia, że profilaktyka jest korzystniejsza od leczenia.

    Streszczenie angielskie: We prepared a special questionnaire with: diagnostic methods, and causes of uretral injury in years 1995 - 1999. We sent this questionnaire to all urological words in Poland. We received replies from 61 urological wards. We had 452 ureteral injureis among them 31 open injuries (7 p.c.), 81 closed injuries (18 p.c.), 340 iatrogenic injury. Most of them were caused by gynaecological procedures (72 p.c.). Ureteral injuries were often complicated with injury of kidneys, intestines and urinary bladder. During the treatment we had: 47.1 p.c. ureterocystoneostomy, Boari method - 25.05 p.c., end to end junction 20.4 p.c. The authors suggest: that in doubtful cases urological consultation with catheterisation of the ureters should be undertaken, because - prophylaxis is preferable treatment.

    Streszczenie polskie: Wstęp. Uszkodzenia pęcherza moczowego towarzyszą najczęściej urazom miednicy lub są wywołane urazami jatrogennymi. Materiał i metoda. Analizie poddano wyniki ankiety, która dotyczyła oceny urazów pęcherza moczowego, leczonych w latach 1995 - 1999 w 61 oddziałach urologicznych w Polsce. Wyniki. W okresie 5 lat zarejestrowano 512 osób z uszkodzeniami pęcherza moczowego. U 210 (41,0 proc.) przyczyną uszkodzenia pęcherza były urazy komunikacyjne, u ośmiu (1,6 proc.) przysypanie lub zgniecenia, u 40 (7,8 proc.) upadek z wysokości, u 251 (49,0 proc.) uraz jatrogenny i u trzech chorych (0,6 proc.) rana postrzałowa. Spośród 261 niejatrogennych urazów pęcherza 41 (15,8 proc.) było wywołanych złamaniami kości miednicy. Równoczesne uszkodzenie pęcherza i cewki tylnej odnotowano u 36 osób, co stanowi 13,7 proc. tych urazów, a 7 proc. wszystkich urazów. Urazy jatrogenne powstały u 98 chorych (39,0 proc.) na oddziale urologicznym, u 130 (51,8 proc.) kobiet w oddziale ginekologicznym i u 23 osób (9,2 proc.) na oddziale chirurgicznym. Do urazu otwartego doszło u 102 chorych (20,0 proc.), a zamkniętego u 372 (72,6 proc.). Stłuczenia pęcherza moczowego zarejestrowano u 38 chorych (7,4 proc.). Uszkodzenia śródotrzewnowe stanowiły 44,0 proc. (225 osób), a zewnątrzotrzewnowe 56,0 proc. (287 chorych). W diagnostyce urazów wykonano 455 (88,8 proc.) badań ultrasonograficznych jamy brzusznej, 266 (51,9 proc.) urografii, 388 (75,7 proc.) cystografii, 15 (2,9 proc.) tomografii komputerowych. ...

    Streszczenie angielskie: Introduction. Urinary bladder injuries accompany most often pelvis trauma and are evoked by iatrogenic trauma. Material and method. The results of questionnaire, received from 61 urological departments, concerned with urinary bladder injuries treated between the years 1995 - 1999, were analysed. Results. During the 5 year period 512 patients with urinary bladder trauma injuries were noted. In 210 (41.0 p.c.) patients the injury was caused by traffic accidents, 8 (1.6 p.c.) patients were buried or squashed within the limits of pelvic bones, in 40 (7.8 p.c.) patients the cause was fall from hights, in 251 (49 p.c.) was noted the injury of iatrogenic origin and in 3 patients (0.6 p.c.) gunshot wound. Among 261 non-iatrogenic urinary bladder injuries, 41 (15.8 p.c.) were connected with pelvic bones trauma. In 36 patients were noted simultaneous injury of urinary bladder and urethra posterior which constitute 13.7 p.c. of such injuries and 7 p.c. of all trauma cases. The injuries of iatrogenic origin occurrred in 98 patients (39 p.c.) at urological department, in 130 (51.8 p.c.) women at gynecological department and in 23 patients (9.2 p.c.) at surgical ward. There was open injury in 102 patients (20.0 p.c.) and the closed in 372 (72.6 p.c.) Urinary bladder contusion was noted in 38 patients (7.4 p.c.). Intraperitoneal injuries constituted 44.0 p.c. Intravenous urography was used in 80 p.c. of the patients and computed tomography in only 20 p.c. In all, 234 patients (26 p.c.) underwent surgery; nephrectomy was the most common surgical treatment, in 170 patients (73 p.c. of those undergoing surgery). Complications occured in 9 p.c. of patients after conservative treatment and in 5 p.c. after surgery. Of the 452 ureteric injuries, 340 (75 p.c.) were iatrogenic, 81 (18 p.c.) blunt injuries and 31 (7 p.c.) open injuries. Of the iatrogenic injuries 73 p.c.occurred during gynaecological procedures, 14 p.c. in general surgery and 14 p.c. in urological procedures. The most frequent diagnostic method was intravenous urography (244 cases), with retrograde pyelography (98) and ureteric catheterization in 125. The diagnosis was estabilished immediately during surgery in 104 patients. The most frequent surgical treatment was uretero-neocystostomy (213, 47 p.c.), the others being a Boari flap (113, 25 p.c.), end-to-end anastomosis (92, 20 p.c.), reconstruction with an ileal loop (30, 7 p.c.) and autotransplantation (four, 1 p.c.)...

Materials and methods: The records and details of urinary bladder injuries treated between 1995 and 1999 were analysed for 61 urological departments in Poland. Results: During the 5-year period 512 patients had urinry bladder injuries: in 210 (41 p.c.) the injury was caused by a road traffic accident, in eight (2 p.c.) by compression (crushing injury) within the limits of the pelvic bones, 40 (8 p.c.) by a fall from a height, in three by a gunshot wound and in teh remaining 251 (49 p.c.) the injury was iatrogenic. Among the 261 bladder injuries that were not iatrogenic, 41 (16 p.c.) were associated with pelvic bone trauma. In 36 patients there was simultaneous injury of the urinary bladder and posterior urethra, constituting 14 p.c. of such injuries and 7 p.c. of all trauma cases. The iatrogenic injuries were in 98 patients (39 p.c.) in urological depatrments, in 130 (52 p.c.) women in gynaecological departments and in 23 (9 p.c.) on surgical wards. The injury was open in 102 patients (20 p.c.) and closed in 372 (73 p.c.0; there was bladder contusion in 38 patients (7 p.c.). The injuries were intraperitoneal in 225 patients (44 p.c.) and extraperitoneal in 287 (56 p.c.).
