The nonmelanoma skin cancer (NMSC) risk associated with the combustion of arsenic-rich coal to generate power in the district of Prievidza, Slovakia, was investigated as part of a European Union-funded project, EXPosure to ArSenic and CANcer in Central and Eastern Europe (EXPASCAN). We conducted a population-based case-control study with 264 cases and 286 controls. A questionnaire was used to obtain exposure and confounder information. Arsenic was measured in urine, soil, house and dust. DNA was extracted from buccal swaps to analyze sequence variations in the repair gene ERCC2 among other candidate genes. Historical arsenic pollution was modeled, and current arsenic exposure in environmental and biological specimen - although much lower than in the past - still shows a significant variation by distance to the power plant. Exposure of the study subjects to environmental arsenic was assessed with the residential hitory and annual arsenic emissions (Asres), further with nutritional habits taking additionally into account the consumption of home-grown food (Asnut). High exposure to environmental arsenic was associated with an excess risk for both Asres (odds ratio (OR) 1.90, 95 p.c. confidence interval (CI) 1.38-2.62) and Asnut (OR 1.83, 95 p.c. CI 0.98-3.43). Among other potential risk factors, skin characteristics and DNA repair were analyzed in more detail. Blonde or red hair was associated with an excess risk (oR 22.64, 95 p.c. CI 1.34-5.19).
