    Streszczenie angielskie: Bakcground: The aim of our study was to analyze drug-induced liver disease over a 3-year period in one gastroenterological department. Material/Methods: International consensus standard definitions and criteria for assessing causality of adverse drug reactions were applied to all patients with abnormal hepatic test results. Results: Drugs were implicated in hepatic injury in 14 patietns (8 females) in whom causal relationship between drug and liver disease was definite or highly probable. The drugs responsible were amoxicillin with clavulanic acid (3 cases), fluvastatin and pravastatin (3 cases), antituberculous drugs (2 cases), estrogens, roxithromycin, asacol, satolol, enalapril and thiamazol. A total of 78.6 p.c. (11 cases) were classified as hepatocellular or mixed hepatitis, while cholestatic injury was found in 21.4 p.c. (3 cases). There were no lethal or severe (prothrombin 50 p.c.) hepatic drug reactions. In 13 patietns the course of liver disease after with drawal of the offensive drug was either acute or protracted, while in one patient there was chronic cholestasis ( 3 years) resulting from injury to interlobular bile ducts by amoxicllin with clavulanic acid. Conclusion: A thorough history of drug intake should be taken in all patients presenting with abnormal hepatic test results. Amoxicillin & clavulanic acid, cholesterol-lowering and antituberculin drugs were the most frequent hepatotoxic factors in our patients. In a majority of cases the liver injury was not severe, and resolved after prompt withdrawal of the responsible drug.

    Streszczenie polskie: Wstęp: Spośród wielu metod stosowanych w leczeniu ostrego krwotoku z żylaków przełyku u chorych na marskość wątroby najistotniejsze znaczenie mają metody endoskopowe, takie jak EVS i EVL, których skuteczność w opanowaniu ostrego krwawienia ocenia się na 75-95 proc. Celem przedstawionej poniżej analizy własnego materiału była ocena skuteczności EVS w leczeniu krwotoków z żylaków przełyku i ich eradykacji w odniesieniu do systematycznej kontroli endoskopowej w okresie follow-up. Materiał i metody: W okresie styczeń 1991 - grudzień 2000 EVS wykonano u 271 chorych na marskość wątroby (etiologia marskości: wirusowa - 63 proc., alkoholowa - 26 proc., AIH - 3,7 proc., PBC - 3 proc., kryptogenna - 3,7 proc.). Terapie podzielono na dwa etapy, pierwszy obejmował leczenie ostrego krwotoku z następowymi zabiegami EVS powtarzanymi co 3 dni, aż do eradykacji i/lub pojawienia się powikłań EVS, nie więcej jednak niż 10 sesji. W razie braku eradykacji ponawiano EVS w ramach drugiego etapu, nie więcej niż 10 sesji sklerotyzacyjnych, a następnie kwalifikowano chorych do 48-miesięcznego okresu follow-up, w czasie którego co 2 miesiące wykonywano endoskopię i powtarzano EVS w przypadku nawrotu żylaków. Wyniki: Spośród 271 chorych krwawienie zahamowano u 259 (95,6 proc.). Sześciu chorych zmarło w pierwszych 48 godzinach hospitalizacji. Stu trzydziestu jeden chorych zgłaszało się do kontroli endoskopowych regularnie co około 2 miesiące, a 101 - nieregularnie, los 27 chorych był nieznany. ...

    Streszczenie angielskie: Background: Among many therapeutic regimens EVS (endoscopic variceal sclerotherapy) and EVL (endoscopic variceal ligation) are known to be the most effective treatments in variceal haemorrhage (75 p.c. to 95 p.c.) caused by liver cirrhosis. The aim of this study was to determine the EVS efficacy in the eradication and treatment of bleeding oesophageal varices in patients who underwent endoscopic control in two months during follow-up. Material and methods: Between 1991 and 2000 a total number of 271 patients of different cirrhosis aetiology (viral 63 p.c., alcoholic 26 p.c., AIH 3.7 p.c., PBC 3 p.c., cryptogenic 3.7 p.c.) underwent EVS procedure in two parts. In the first, EVS was done in 3-day intervals up to variceal eradication (no more than 10 procedures) or up to EVS complications. In the cases of no variceal eradication, the second part of treatment was introduced (no more then 10 sclerotherapies). After that, the patients were endoscopically followed up for 48 months in two-month intervals. Retreatment using EVS was introduced if variceal recurrence was found. Results: Acute haemostasis was achieved in 259 out of 271 patients (95.6 p.c.). Six patients died within the first 48 hours of hospitalisation. 131 patients were endoscopically followed up for 48 months regularly while the remaining 101 patients irregularly. 27 patients were lost, there was no information about them.
