    Streszczenie angielskie: There is a considerable disagreement on differentiated thyroid cancer (DTC) treatment in children. In most centres standard treatment for DTC comprises total thyroidectomy, radioiodine treatment and TSH suppressive therapy. However, not all agree on such radical therapy due to the minor agressivity of differentiated thyroid cancer in children. The opponents question both the indications for total thyroidectomy and complementary radioiodine treatment. The aim of the present study is a retrospective analysis of treatment results in children treated or followed up in our centre. The study included 205 children with DTC (age 4 to 18) observed up to 27 years post initial treatment (median: 4 years). There were 48 children with follicular ca and 156 with papillary ca diagnosed. The median age at diagnosis was 14 years. The treatment comprised total thyroidectomy in 176 cases (86 p.c.), radioiodine therapy in 80 p.c. and TSH suppressive therapy with levothyroxine in all patients. The actuarial survival rate was 100 p.c. and recurrence rate 10,7 p.c. The 5 and 10 - year actuarial disease free survival was 76 p.c. and 74 p.c. respectively. The results were much poorer in children who were not primarily treated by a radical approach. It is to be concluded that total thyroidectomy and radioiodine treatment significantly improve recurrence free survival in children and should be routinely applied even in young children as the primary treatment of differentiated thyroid carcinoma.
