The aim of the study was the clinical and histopathological evaluation of the oral mucosa changes in patients with renal graft and active HPV infection and also epidemiological analysis of the papilloma cases in patients undergoing treatment at the Department of Oral Surgery in 1999-2000. The study group consists of 10 patients (8 women, 2 men), age 23-58, 2 to 18 years after renal transplantation, with confirmed HPV infection in anogenital area. Every patient undergoes immunosuppressive treatment, consisting of Encorton, Azathioprine and Cyclosporine A. During dental examination and treatment the samples of the oral mucosa were taken for further histopathological diagnostics. Typical papillomatosous changes of the oral mucosa were rare. However the histopathological examinations revealed characteristic signs of HPV infection: koilocystosis, dyskeratosis, parakeratosis. In one case high degree dysplasia was found. Our results give evidence that even in lack of clinical changes of the oral mucosa we always should consider the possible existence of subclinical HPV infection. It is important, because patients undergoing immunosuppression have high risk of carcinogenesis caused by oncogenic types of HPV.

The aim of this study was to compare the oral cavity status with the species composition of microorganisms colonizing the mounth of patients with or without systemic diseases. Seventy-seven men and women, 19 to 65-years-old, of four groups: hemodialyzed patients (HD), kidney allograft recipients (Tx), congenitally disordered (Cd) and control (C) patients (without systemic diseases) were examined clinically for the oral cavity status and the microorganisms occurring in the mounth were identified. Swabs and samples taken from dental plaques, pockets and periodontium were used for light and electron microscopical studies as well as for protozoan, bacterial and fungal cultures. Entamoeba gingivalis and Trichomonas tenax were found in the HD, Tx, Cd and C groups (top prevalence 14, 14, 87.5 and 25 p.c. respectively); free-living amoebae (with some features of Acanthamoeba sp.) also occurred in several cases. Staphylococcus aureus bacteria were found in 66 p.c. of Cd patients infected with the protozoans. Numerous fecal bacteria and/or fungi Candida albicans have been observed in 41-51-year-old Cd and HD patients. In all patient groups, it was the 41-51-year-olds who showed the highest prevalence of protozoans, bacteria and fungi. The results indicate that metabolic disabilities favour pathological changes in periodontal tissues and may influence the species composition of mixed protozoan, bacterial and/or fungal infections in various ways, in patients with different systemic diseases.

Jednym z najistotniejszych czynników decydujących o możliwości podjęcia rehabilitacji implantologicznej pacjenta jest odpowiednia ilość oraz gęstość tkanki kostnej. Dla prawidłowj oceny gęstości żuchw pacjentów z osteodystrofią nerkową stworzono model densytometryczny dla różnych stref anatomicznych na podstawie cyfrowych zdjęć pantomograficznych w systemie Planmeca oceniane przy wykorzystaniu programu Wix Vin 2000. W oparciu o stworzony cyfrowy model densytometryczny żuchw zdrowych mężczyzn, stwierdzono obniżoną gęstość optyczną kości żuchw u mężczyzn po przeszczepieniu nerki, najsilniej zaznaczoną w trzonie i kącie. Autorzy podjęli próbę dyskusji nad możliwością zawężenia przeciwwskazań do rehabilitacji implantologicznej u chorych z osteodystrofią nerkową.
