    Streszczenie angielskie: The hallmarks of rheumatoid arthritis (RA) are leukocytic infiltratin of the synovium and expansiveness of fibroblast-like synovicytes (FLS). The abnormal proliferation of FLS and their resistance to apoptosis is mediated, at least in part, bay present in RA joints proinflammatory cytokines and growth factors. Because IL-15 exerts properties of antiapoptotic and growth facators, and is produced by RA FLS, we hypothesized that IL-15 participates in RA FLA activation. To test this hypothesis, we first examined whether RA FLS express chains required for high affinity functional IL-15R. Indeed, RA FLS express IL-15Rŕ at mRNA and protein levels. Moreover, we confirmed the presence of IL-2Rá and common ç-chains. Interstingly, TNF-ŕ or IL-1á triggered significant elevation of IL-15Rŕ chanin at mRNA and protein levels. Next, we investigated the effects of exogenous or endogenous IL-15 on Bcl-2 andBcl-XL expression, FLS proliferation, and apoptosis. Exogenous IL-15 enhanced RA FLS proliferation and increased the level of mRNA-encoding Bcl-XL. To test the role of endogenous IL-15 in the activation of RA FLS, an IL-15 mutant/Fcç2a protein exerting properties of specific antagonist to the IL-15Rŕ chain was used. We found that blocking IL-15 biological activities using this protein substantially reduced endogenous expression of Bcl-2 and Bcl-XL, and RA FLA proliferation that was reflected by increased apoptosis. Thus, we havae demonstrated that a distinctive phenotype of RA FLS, i.e., persistent activation, proliferation, and resistance to apoptosis, is related to the autocrine activation of IL-15Rs by FLS-derived IL-15.

    Streszczenie angielskie: In the European countries the health surveillance may be provided as a part of the national health system intervention. In Italy, the legislative Decree (626/94) makes the health surveillance compulsory for all those workers who are exposed to occupational risks. The aim of this study was to describe the introduction of preventive and protective measures, according to the new regulations, in the teaching hospital of the University of Modena. The population examined in 2000 included 1523 workers. Specific health surveillance protocols were prepared on the grounds of the risk characteristics based on the scientific evidence and on the risk perception. The intervention was oriented towards health promotion. The subjects were classified into 10 groups according to the risk characteristics. The percentage of workers ranged from 4 p.c. to 42 p.c., depending on the exposure-related health changes. Moreover, the study explored some of the health surveillance benefits: an improvement in worker's satisfaction, an improvement in relationship between stakeholders, an early detection of health changes and a sickness absence reduction after the influenza vaccination program.

    Streszczenie angielskie: Background: The objective of the present study is to compare the frequency and nature of expected 'whiplash' symptoms in Lithuania (a country where the late whiplash syndrome is rare or unknown) with that in Canada. Material/Methods: A symptom checklist was administered to 2 subject groups selected from local companies in Kaunas, Lithuania, and Edmonton, Canada, respectively. Subjects were asked to imagine having suffered a neck sprain (whiplash injry) with no loss of consciousness in a motor vehicle accident, and to check which, of a variety of symptoms, they would expect might arise from the injury. For symptoms they anticipated, they were asked to select the period of time they expected those symptoms to persist. Results: In both the Lithuanian and Edmontonian groups, the pattern of symptoms anticipated closely resembled the acute symptoms commonly reported by accident victims with acute neck sprain, but while up to 50 p.c. of Edmontonians also anticipated symptoms to last months or years, very fe Lithuanian subjects selected any symptoms as likely to persist. Conclusions: In Lithuania, despite the documented occurrence of neck sprain symptoms in some 50 p.c. of individuals following motor vehicle accidents, there is a very low rate of expectation of any sequelae from this injury. What current or previous aspects of society that underlie this remain uncertain. This lack of expectation of chronicity in Lithuania may, in part, determine the low prevalence of the late whiplash syndrome there. Further sutdies of symptom expectation as an etiologic factor in the late whiplash syndrome are needed.
