    Streszczenie angielskie: Neuropathic pain is subsequent to the pathological foci which arise within the central and peripheral nervous system, and the aim of therapy, including pharmacological, is to diminish or cease their activity through using with the diferent mechanisms of action. The way to optimise pharmacological management of patients with chronic pain, especially those with neuropathic pain consist in, for example, selecting the effective and available drug combination supported by known mechanisms of action or identification of pain-generating mechanisms based on analysis of the effectiveness of the drugs used. Multidirectional management is indicated and includes not only anesthesiological, surgical or pharmacological methods but also rehabilitative programs, cognitive, behavioral therapies or adjunctive techniques for the purpose of better functioning even if the pain perceptions still continue.

    Streszczenie angielskie: Pain is one of the basic symptoms of various diseases occurring in the elderly. However many factors e.g. degenerative processes in various organs and the psychological status of elderly patients affect perception of pain in this group. The article briefly reviews the pathophysiology o fpain in ageing patietns. An outline of pain management concerning special features of this particular populaiton is presented. mechanisms of action of basic drugs used in pain management are discussed. THe role of psychotherapy and combined treatment is stressed.

    Streszczenie angielskie: The aim of this study was to assess the influence of iv tramadol on opioid requirement in the early postoperative period. The subjects were 90 patients scheduled for colon surgery (hemicolectomy) who received general anesthesia using the (N2O/O2) isoflurane technique. Thirty patients (group I) were administered 100 mg of tramadol iv before induction of general anesthesia (preemptive analgesia). Group II (30 patients) was administered 100 mg of tramadol iv immediately after peritoneal closure (preventive analgesia) and control group (30 patients) received 100 mg of tramadol iv immediately after operation. Following the operation, all patients were administered tramadol in the PCA-iv mode in order to treat postoperative pain. In the postoperative period, the following parameters were measured: pain intensity (using VAS), total consumption of tramadol, time until the first PCA activation, and frequency of side effects (drowsiness, nausea, vomiting). In patients of groups I and II who had received preemptive or preventive analgesia, a significantly lower total consumption of tramadol, as compared with control group, was observed in the early postoperative period. However, the time until the first PCA activation was significantly shorter in group I as compared to the other two groups. No significant differences between the groups were found regarding pain intensity and frequency of side effects.

    Streszczenie polskie: Przedstawiono historię 74-letniego pacjenta, cierpiącego na ostre, napadowe, pourazowe bóle prawej strony karku, promieniujące wzdłuż przyśrodkowego brzegu łopatki. U chorego stwierdzono zanik mięśnia czworobocznego prawostronnie, punkt spustowy bólu w bliźnie na wysokości wyjścia n. XI spod mięśnia mostowo-obojczykowo-sutkowego oraz obiektywnie potwierdzono neuropatię nerwu dodatkowego badaniem EMG. Jednocześnie współistniały objawy radikulopatii na poziomie C2-C4. Istnieją przesłanki anatomiczne, by uważać, że bolesna neuropatia n. XI istnieje i jest uwarunkowana złożonym mechanizmem patologicznym.

    Streszczenie angielskie: The story of the 74-years old male patient is presented, suffering from sharp, spontaneous, posttraumatic pains of the right side of the neck, radiating down along the medial edge of the right shoulder-blade. On his examination it was found wasting of the right trapezius muscle, the trigger point over a scar, lying on the level of the emerging right XI nerve from under the sterno-cleido-mastoid muscle, and by means of EMG, the lesion of this nerve was proved. At the same time, there were signs of radiculopathy C2-C4. Taking under consideration the anatomy, it might be said, that the painful neuropathy of the accessory nerve exists and depends on a complex pathology.
