    Streszczenie polskie: Przezoskrzelowa biopsja płuca (PBP) jest jedną z metod inwazyjnych stosowanych w diagnostyce zmian litych i rozsianych. Zabieg wykonywany jest pod kontrolą rentgenoskopii, przy użyciu bronchofiberoskopu. Powikłania PBP występuja stosunkowo rzadko. Należą do nich krwawienie, zwykle niewielkie oraz odma opłucnowa, w niektórych przypadkach wymagająca drenażu. W okresie od maja 2000 do maja 2001 r. w Klinice Ftyzjopneumonologii Akademii Medycznej w Poznaniu wykonano biopsje kleszczowe u 39 chorych (32 mężczyzn i 7 kobiet) w wieku od 30 do 78 lat z podejrzeniem procesu nowotworowego. Materiał tkankowy poddano ocenie histopatologicznej. U 27 chorych (69,2 proc.) rozpoznano raka płuca, włóknienie śródmiąższowe u 2 chorych, u 1 chorego gruźlicę i u 1 chorego pylicę krzemową. Nie uzyskano potwierdzenia histologicznego procesu chorobowego u 8 pacjentów poddanych biopsji (20,5 proc.). Nie odnotowano żadnego przypadku odmy opłucnowej. Powikłaniem występującym po każdej biopsji kleszczowej było krwawienie, na ogół niewielkie, jedynie w 7 przypadkach wymagało podania leków przeciwkrwotocznych.

    Streszczenie angielskie: Transbronchial lung biopsy (TBLB) is an invasive method of diagnosis of both solid pulmonary lesion and disseminated pulmonary changes. During the procedure forceps biopsy of extra-bronchial changes is performed. Correct positioning of a fiberoptic bronchoscope and forceps is assurend and guided by chest fluoroscopy. The complications of TBLB are rare and include: haemophysis and pneumothorax. rarely either of these requires intensive treatment. The aim of this study was to assess the usefulness of fluoroscopy guided transbronchial lung biopsy in the diagnosis of possible lung malignancies. Between May 2000 and May 2001 in the Department of Pulmonary Diseases University of Medical Sciences in Poznań lung biopsies were done in 39 patients suspected of pulmonary malignancies. They were 32 males and 7 females and their age varied from 30-78 years. There were different imaging techniques used in the assessment of lung change character and localisation. All the patients had conventional chest radiographs (both posteroanterior and lateral fimls) performed, some tomograms or computed tomography of the chest. The procedures were performed in radiology laboratory after the patient was premedicated. Fiveroptic bronchoscope was inserted through nasal passage, and upper airways and bronchial tree was evaluated. Then forceps were introduced through the bronchoscope to the region with change. The position of biopsy forceps was controlled with the use of fluorosocpy both in patient's supine and lateral position...

    Streszczenie polskie: Biopsja płuca przez ścianę klatki piersiowej stanowi jedną z metod inwazyjnych, stosowanych w diagnostyce zmian nowotworowych położonych obwodowo. Zastosowanie igły tnącej umożliwia uzyskiwanie stosunkowo dużych wycinków poddawanych ocenie histopatologicznej. W niektórych przypadkach może to mieć znaczenie w diagnostyce różnicowej zmian o etiologii nienowotworowej w układzie oddechowym, gdzie do psotawienia rozpoznania niezbędna jest ocena materiału tkankowego (sarkoidoza i inne). Choroby te mogą w swoim przebiegu budzić podejrzenie zmian nowotworowych. Badania przeprowadzano w Klinice Ftyzjopneumonologii w Poznaniu od kwietnia 2000 r. do września 2001 r. u 20 chorych (15 mężczyzn i 5 kobiet) w wieku 36-82 lat. Zabiegi wykonywane były pod kontrolą rentgenoskopii, przy użyciu igły tnącej. Materiał tkankowy poddano ocenie histopatologicznej. Potwierdzono występowanie raka płuca u 14 chorych (70 proc.). Rozpoznano po jednym przypadku twardziny układowej, szpiczaka mnogiego i ropnia płuca (łącznie 15 proc.). Nie uzyskano potwierdzenia histologicznego procesu chorobowego u 3 pacjentów poddanych biopsji (15 proc.). w 4 przypadkach wystąpiła odma opłucnowa bez potrzeby drenażu.

    Streszczenie angielskie: Transthoracic lung biopsy (TTLB) is an invasive method used in diagnosis of neoplastic lung changes of peripheral localisation. The use of cutting needle enables to obtain relative big samples for histopatology assessment. TTLB is especially helpful, when tissue assessment is required for differential diagnosis of neoplastic lung changes. The reported sensitivity of TTLB depends on the size of the lesion and is usually higher than 70 p.c. Pneumothorax is the most frequent complication but usually only requires drainage in a minority. The risk of pneumothorax appears to be influenced by variables related to the patient, the lesion localisation and its diameter, the technique performance and the experience of the physician. The aim of this study was to assess the usefulness of fluoroscopy guided transthoracic lung biopsy in the diagnosis of possible lung malignances. Between April 2000 and September 2001 in Department of Pulmonary Diseases University of Medical Sciences in Poznań lung biopsies were done in 20 patietns (15 male and 5 female) and their age varied from 36-82 years.     Streszczenie angielskie: Purpose: Brachytherapy is one of the most efficient methods of overcoming endobronchial obstruction in palliative treatment of lung cancer. In single cases, brachytherapy is performed as radical treatment, however in most of cases, due to advanced clinical stage it has a paliative aim. In the absence of clear consensus regarding the value of doses used in brachytherapy different fraction doses are used in clinical treatment. The aim of this work is to compare results of palliative high dose rate brachtherapy using various treatment protocols with the view to analysing differences in survival and diminishing breathing difficulties. Material and methods: Between May 1999 and February 2000 at the Greatpoland Cancer Center, 69 patients with advanced lung cancer were treated by high dose rate brachytherapy. They were disqualified from radical treatment due to advanced clinical stage. The age of the patients ranged from 39 to 76 years (average 53.2 years). Fofty-one patients received a total dose of 22.5 Gy in 3 fractions once a week, 18 patients received one single fraction of 10 Gy. All the patients were divided into two groups according to their clinical stage and the Karnofsky score - those with the Karnofsky score lower than 50 were qualified for a single fraction treatment. They were under clinical and endobronchial observation as regards survival rates, local remission and subsiding dyspnoea, breathing, cough and haemoptysis in the first, third, sixth and twelveth month of observation.
