Streszczenie angielskie: Hepatocellular carcinoma (HCC) is known to be a tumour of older children and adolescence and its closely related to metabolic liver diseases, hepatitis and post HBV/HCV liver cirrhosis. Cirrhosis, multifocal origin of the disease and poor response to chemotherapy are also known limitations in the treatment of HCC. Both traditional surgery and liver transplantation have been applied in the treatment of HCC but the experience in children is still very limited. Indications for each treatment strategies are also not clear in children as the many paediatric patients demonstrate the lack of underlining liver cirrhosis. Possibilities and limitations in the treatment of HCC have been studied in the group of 19 patients treated in the single centre, to assess the results and treatment strategies. The common type of HCC was diagnosed in 14 children and the fibrolamellar variant (FL-HCC) in 5 patients. In 13 children HCC developed secondary to liver cirrhosis and in 11 children history of hepatitis mainly post HBV had been documented. The patients with liver cirrhosis and HCC demonstrated elevated serum level of alfa fotoprotein (AFP), whereas serum AFP of the patients with FL-HCC was within the normal ranges. Eight of 19 children with HCC presented very advanced stage of disease, making surgery impossible. Partial liver resection have been performed in 7 patients. In 5 children with HCC, of whom 4 had liver cirrhosis, orthotopic liver transplantation had been done. Six of 11 operated children are alive. Better prognosis for patients with FL-HCC is suggested. The prognosis for common type of HCC, especially in advanced stages and resulting from liver cirrhosis, despite different approaches remains poor.
