    Streszczenie angielskie: One of the first paediatric ENT centres in the world was the research group managed by Jan Danielewicz in Warsaw. It was organised on the basis of progress clinics and paediatric ENT wards, and a programme of specialisation was established (1, 2). Almost simultaneously, in the sixties of the twentieth century, centres for paediatric ENT were established in Prague, Berne, Bratislava, Budapest, Bucharest and Sophia. In the nineteen-seventies these were in touch with the centre for paediatric ENT in Italy, which was the pioneer in this speciality in West Europe. The effect of this contact was the creation of the European Federation of Oto-Rhino-Laryngology Societies, and the International Federation of Oto-Rhino-Laryngological Societies (IFOS), and further dynamic progress in this specialisation.

    Streszczenie angielskie: Tracheostomy is a life-saving procedure when pertbrmecl with an appropriate indication and surgical technique. There is a limited indication for tracheostomy procedures in paediatrie surgery. It is rarely applied to the paediatric patient because they can be kept intuhated for a longer period compared to adults. Indications for tracheostomy are prolonged intubation, subglotie stenosis, general body trauma, tracheomalasia, cervical tumour pressing the trachea and larynx, congenital diseases of the larynx, burn injuries of the trachea, and foreign body aspiration. In the last decade the number of cases with tracheotomy increased due to the development of new intensive care units, the use of mechanical ventilation, and the increasing number of patients needing prolonged ventilation support. Tracheotomy in children lias been associated with significant operative and post-operative complications. When long-term tracheotomy is needed, parent education, together with special equipment and environment may allow the return of the tracheotomised child to home in secure conditions.

    Streszczenie angielskie: Plastic surgery of the lateral nasal wall in children has been performed in Warsaw Paediatric ENT Clinic since 1995. There have been 121 operations. They consisted of conservative reconstruction of congenital or acquired defects in this area. In most eases a decrease in the number of upper airways infections and permanent improvement of nasal patency was attained. None of the cases lias shown any inhibitory influence from these procedures on craniofacial development. The principles of accomplishing conservative plastic surgery of the nasal mucous membrane are defined.

    Streszczenie angielskie: Choanal atresia is a congenital abnormality often coexisting with other developmental abnormalities. The treatment of choice his condition is surgical restoration of choanae patency using a transnasal or transpalatal operative approach. After ;ical treatment, separators are fixed for several weeks in the posterior choanae and nasopharynx. Surgical transpalatal rvention and extended retention of separators in the posterior choanae and nasopharynx may cause auditive tube unction and otitis media with effusion. The aim of this work was the estimation of the occurence of otitis media with sion in children under post-operative treatment after choanal atresia depending on the surgical approach applied. immary of the period 1959-2002 in the Department of Paediatric Otorhinolaryngology in Warsaw shows that 175 operations s performed because of choanal atresia. A group of 11 children operated on choanal atresia between 1996-2002 was nined. Acoustic impedance audiometry was performed in all children. Bilateral choanal atresia was recognised in 4 children, unilateral choanal atresia in 7 children. A tympanomctry curve type B was obtained in 4 children, but a tympanometry curve type A was found in 7. In the examined group we did not find a dependence between the type of operative approach in surgical treatment of choanal atresia and otitis media with effusion occurrence.

    Streszczenie angielskie: Adenoidectomy, tonsillectomy and tonsillotomy were and still are the most common surgical procedures performed in Paediatric Otorhinolaryngology. The aim of this study is to justify the use of these methods in particular cases, and also specifying the complications that might occur in the postoperative period. The indications for the procedures are divided into definite and relative, while the complications are separated into two groups, early and late. The information given in the text is based on the experience of the Paediatric ENT Clinic of the Medical University of Warsaw, and the available literature.

    Streszczenie angielskie: Laryngeal papillomas in children constitute a disease of viral aetiology caused by Human Papilloma Virus. Even though the changes arc histologically benign, their exophytic growth may lead to a critical airway obstruction and the child's suffocation. Due to the tendency to recur, lack of causal treatment, and risk of malignancy the optimal method of treating this condition is still being sought. The study presents methods applied nowadays in the treatment of laryngeal papillomas; especially, it concentrates on a classical papillotomy and its main advantage: the diminished risk of scar formation leading to laryngeal stenosis and vocal fold injury with speech disorders.

    Streszczenie angielskie: The purpose of the study was to review clinical presentation, diagnostic methods, and conditions causing vertigo and dizziness in children. The authors report a retrospective review of 70 children diagnosed in the ENT Department or Outpatient Clinic in the Department of Paediatric Neurology. The differential diagnosis of dizziness and vertigo in children requires a multidisciplinary approach. Close co-operation between the otolaryngologist, paediatrician, neurologist and ophtalmologist allows the best chance of accurate diagnosis. From an analysis of the results, a diagnostic approach to the dizzy child is suggested, and the most frequent conditions causing vertigo in children are discussed.

    Streszczenie angielskie: The goal of our study was to assess suitability of FNA biopsy material as a source of samples (cell suspension) for DNA ploidy assessment in neoplastic tumors using flow cytometry. DNA ploidy is an established prognostic factor in many types of cancers. Aneuploid breast tumors are characterized by increased aggressiveness which manifests itself through rapid local progression and metastatic spread. Investigated specimens were breast cancer FNA biopsy cell susupensions. Measurements were performed using flow cytometry. Material studied comprised 143 cases analyzed in 1999-2000. We found in this group 101 carinoma cases with aneuploid typ and 42 cases of primary breast carcinoma with diploid type of cell cycle. Immunocytochemical assessment of estrogen receptor and progesterone receptor status was performed in group of 105 cases DNA ploidy was compared to receptor status of the invetigated cells. DNA aneuploidy correlated wtih weak or no reaction for the presence of estrogen and progesterone receptors. Our study demonstrates the suitability of DNA ploidy assessment method applied to cytological material from FNA biopsies.

    Streszczenie angielskie: Ovarian steroids are one of the strongest risk factors for breast cancer. Sex hormone-binding globulin (SHBG) binds and transports sex steroids in the blood, regulating their bioavailable fraction and access to target cells. It can also inhibit the estradiol-induced proliferation of breast cancer cells through its membrane receptor. Three coding-region polymorphisms, which oead to an amino acid change, have been reported. We studied the influence of these three polymorphisms on breast cancer risk in three different populations: polish familial breast cancer cases, 27 p.c. of them carrying a BRCA1/BRCA2 mutation, Nordic familial and sporadic breast cancer cases. The reported G to A polymorphism in exon 1 was not found in the 423 analyzed samples. Instead, we found a C to T transition causing an arg to cys amino acid change within the same codon in one polish breast cancer patient and her daughter. Both of them were heterozygotes for the exon 8 to A polymorphism as well. They were diagnosed for bilateral breast cancer and carried a BRCA1 mutation (5382insC). Analysis of the tumor samples showed that they had lost the wild-type allele both at exons 1 and 8 of the SHBG gene. Analysis of the other Polish samples showed no correlation of the exon 8 polymorphism to breast cancer, bilateral breast cancer, BRCA1/BRCA2 mutations or age at diagnosis. No association of the exon 8 polymorphism with breast cancer in the Nordic familial or sporadic cases was found. The C to T polymorphism located in exon 4 was rare in all the studied populations (overal allele frequency 0.011)...
