    Streszczenie angielskie: Background. The management of patients with perisistent atrial fibrillation (AF) has not benn well established. Recurrences of AF following electrical cardioversion are common and the use of a single antiarrhythmic drug only is often ineffective. Aim. To assess the efficacy of sequential use of antiarrhythmic agents combined with electrical cardioversion for restoration and maintenence of sinus rhythm in patients with persistent AF. Methods. The study group consisted of 83 patients (27 females, 56 males, mean age 60.7 ń 8.3 years) who were selected for elective electrical cardioversion of persistent AF. After sinus rhythm restoration patients were started on antiarrhythmic treatment in an open-label manner with propafenone, sotalol or disopyramide. If there was a recurrence of AF, the electrical cardioversion was repeated and the patient was started on the other antiarrhythmic agent. In case of a further AF recurrence, amiodarone in the maintenance dose of 100 - 200 mg/day was continued. Patients in whom the initial cardioversion was not successful, were treated with amiodarone and underwent repeated cardioversion. The follow-up duration was 12 months. Results. The first electrical cardioversion was effective in 44 (53.0 p.c.) patients. After one year of follow-up, 23 (52.3 p.c.) of them maintained sinus rhythm. None of the patients who underwent second cardioversion and received a second antiarrhythmic drug, maintained sinus rhythm.
