    Streszczenie angielskie: The anticoconvulsive potential of classical antiepileptics co-administered with á-adrenergic receptor antagonists against generalized tonic-clonic seizures was evaluated in the model of maximal electroshock (MES)-induced convulsions. Propranolol, acebutolol, metoprolol and atenolol were tested in the doses not affecting the electroconvulsive threshold. Propranolol and metoprolol lowered the ED50 of valproate and diazepam. Acebutolol reduced valproate's but not diazepam's ED50 value. In contrast, hydrophilic atenolol, not penetrating via blood-brain barrier, affected neither the action of valproate nor diazepam. None of the studied drugs changed the protective activity of carbamazepine and phenytoin against MES. á-blokers per se did not alter the motor performance of mice. Moreover, propranolol and metoprolol did not influence diazepam-evoked impairment of locomotor activity. The free plasma and brain levels of antiepileptic drugs were not affected by á-blokers. In conclusion, the use of certain á-adrenoceptor antagonists, such as propranolol and metroprolol, might improve the antiepileptic potential of valproate and diazepam.

    Streszczenie polskie: W województwie krakowskim w 1998 roku zarejestrowano 4382 nowe zachorowania na nowotwory złośliwe, w tym 2154 u mężczyzn i 2228 u kobiet. U mężczyzn najczęściej rejestrowany był nowotwór złośliwy płuca (26,1 proc.), gruczołu krokowego (8,2 proc.) i żołądka (6,7 proc.), a następnie skóry (5,6 proc. i pęcherza moczowego (5,5 proc.). Nowotworami złośliwymi najczęściej rejestrowanymi u kobiet tyły nowotwory złośliwe piersi (20,1 proc.), szyjki macicy (7,7 proc.), płuca (7,4 proc.) a także jajnika (7,0 proc.) i skóry (6,0 proc.). W 1998 roku z powodu nowotworów złośliwych zmarły w województwie krakowskim 2793 osoby, w tym 1558 mężczyzn i 1235 kobiet. U mężczyzn najczęstszą przyczyną zgonów był nowotwór złośliwy płuca (33,2 proc.), żołądka (8,9 proc.), gruczołu krokowego (7,2 proc.), pęcherza moczowego (5,1 proc.) i okrężnicy (4,6 proc.) Największy odsetek zgonów na nowotwory złośliwe u kobiet stanowiły nowotwory piersi (13,8 proc.), płuca (10,3 proc.), okrężnicy (7,0 proc.), szyjki macicy (6,8 proc.) i jajnika (6,6 proc.). Prognoza wartości standaryzowanych współczynników zachorowalności na nowotwory złośliwe u mężczyzn i u kobiet do roku 2003 wykazała dalszy wzrost współczynników u obu płci, przy czym wzrost ten jest wyraźniejszy u kobiet.

    Streszczenie angielskie: There were registered 4382 cancer cases in the Cracow Region in 1998, 2154 in males and 2228 in females. For men the most frequently registered cancer cases were: cancer of the lung (26.1 p.c.), prostate (8.2 p.c.), stomach (6.7 p.c.), skin (5.6 p.c.), urinary bladder (5.5 p.c.). The most common in women were: cancer of the breast (20.1 p.c.), cervix uteri (7.7 p.c.), lung (7.4 p.c.), ovary (7.0 p.c.), skin (6.0 p.c.). In 1998, 2793 persons died of cancer, of which 1558 were males, and 1235 females. Lung cancer was the most frequent case of cancer death in males (33.2 p.c.). Cancer of the stomach (8.9 p.c.), prostate (7.2 p.c.), urinary bladder (5.5 p.c.) and colon (4.6 p.c.) were also a major cause of cancer death. Most of the women died from breast (13.8 p.c.) and lung cancer (10.3 p.c.), the next frequent sites of cancer were: colon (7.0 p.c.), cervix uteri (6.8 p.c.). The prognosis of standardised cancer incidence values in men and women by the year 2003, showed further growth in incidence rates for both sexes, but this growth is more evident in women.

    Streszczenie angielskie: The aim of the present study was to evaluate the effect of mitochondrial inhibitors, 1-methyl-4-phenylpyridinium (MPP+) and 3-nitropropionic acid (3-NPA), on the brain production of endogenous glutamate antagonist, kynurenic acid (KYNA). MPP+ and 3-NPA dose-dependently impaired the synthesis of KYNA in rat cortical slices. Enzymatic studies revealed the MPP+ inhibits in a concentration-dependent manner the activity of kynurenine aminotransferase II (KAT II), but not the activity of kynurenine aminotransferase I (KAT I). 3-NPA impaired the activity of both enzymes, KAT I and KAT II. Thus, MPP+ - and 3-NPA-evoked neurotoxicity may be at least partially associated with the depletion of KYNA.

    Streszczenie polskie: Wprowadzenie: Choroba refluksowa przełyku może objawiać się poza typowymi dolegliwościami, takimi jak zgaga, dysfagia czy zwracanie treści żołądkowej również objawami otolaryngologicznymi (nawracająca chrypka, zaleganie wydzieliny w jamie ustnej, uporczywy kaszel). Cel pracy: Celem pracy była ocena znaczenia badań diagnostycznych w rozpoznawaniu choroby refluksowej u pacjentów z przewlekłą chrypką. Materiał i metody: Badaniami objęto 28 osób w wieku 17-40 lat leczonych bez uzyskania poprawy w Klinice Otolaryngologii z powodu nawracającej chrypki. Poza badaniem otolaryngologicznym wzbogaconym o pełne badanie foniatryczne u chorych wykonywano badanie endoskopowe (wraz z oceną histopatologiczną) oraz badanie pH-metryczne przełyku. Wyniki: U 86 proc. badanych stwierdzono obecność kwaśnego refluksu, a u 58 proc. spośród chorych z zarzucaniem od przełyku wykazano również obecność zmian endoskopowych.

    Streszczenie angielskie: Introduction: Gastroesophageal reflux disease (GERD) may manifest, besides typical complaints such as pyrosis, dysphagia or regurgitation, also with otolaryngological symptoms (recurrent hoarseness, secretion in the mouth, persistent cought). Aim: Evaluation of the significance of different methods in the diagnosis of GERD in patients with chronic hoarseness. Material and methods: 28 patients (17-40 years) treated without improvement because of chronic hoarseness were examined. Besides laryngological and complete foniatric examination, oesophagogastroscopy with biopsy and histopathological examination and 24-hour pH-metry were perforemd. Results: Acid reflux was observed in 86 p.c. of patients, and in 58 p.c. endoscopic changes were demonstrated.

    Streszczenie angielskie: Rat aortic slices produced and liberated the endogenous antagonist of glutamate receptors, kynurenic acid, upon exposure to L-kynurenine. Endothelium-denuded slices did not synthesize any measurable amount of kynurenic acid, indicating its endothelial origin. Aortic kynurenic acid production was diminished by modification of the ionic milieu, hypoxia and hypoglycemia, as well as by L-glutamate and L-aspartate, endogenous glutamate receptor agonists, and aminooxyacetic acid, a non-selective inhibitor of aminotransferase and mitochondrial respiration. These data pave the way for future research aimed to clarify to the role of kynurenic acid in the physiology and pathology of the endothelium and vasculature.

    Streszczenie polskie: Polimorfizm N363S receptora glikokortykosteroidów (GCR) jest związany ze zróżnicowaną wrażliwością na glikokortykosteroidy. Z drugiej strony odpowiedź na kortykoterapię jest istotnym czynnikiem prognostycznym u dzieci z idiopatycznym zespołem nerczycowym (IZN). Stąd też rodzi się pytanie czy polimorfizm N363S genu GCR może być związany z podatnością na zachorowania lub z przebiegiem klinicznym IZN. Badania przeprowadzono u 73 dzieci z IZN i 68 dzieci zdrowych stanowiących grupę kontrolną. Genomowy DNA z leukocytów krwi obwodowej amplifikowano metodą PCR z primerami flankującymi obszar polimorficzny. W pobliżu końca 3' primera sensownego został wprowadzony błędny nukleotyd w celu stworzenia miejsca restrykcyjengo dla enzymu Mun I w allelu N363. Allel S363 był identyfikowany na podstawie utraty miejsca restrykcyjnego Mun I. Nie stwierdzono istotnych różnić w częstości genotypów między grupami (80,8 proc. NN i 19,2 proc. NS w grupie IZN, a 91,2 proc. NN i 8,8 proc. NS w grupie kontrolnej, p=0,08). Dodatkowo, w grupie IZN nie stwierdzono znamiennego powiązania między genotypami GCR, a płcią, wiekiem ujawnienia się i nawrotowością IZN, stężeniami kreatyniny i cholesterolu w surowicy oraz wysokością ciśnienia tętniczego. Wyniki naszych wstępnych badań sugerują brak związku między polimorfizmem N363S receptora GCR, a predyspozycją do zachorowania i rokowaniem u polskich dzieci z IZN.

    Streszczenie angielskie: THe N363S polymorphism of the glucocorticoid receptor (GCR) has been associated with a different sensitivity to glucocrticoids. On the other hand, the response to steroid treatment significantly predicts the prognosis in children with idiopathic nephrotic syndrome (ISN). Therefore, it raises the question whether N363S polymorphism of GCR gene may be associated either with the susceptibility to INS or with the clinical course of INS. The study group consited of 73 children with INS and 68 healthy children as controls. Genomic DNA from peripheral blood leukocytes was amplified by PCR method with primers flanking the polymorphic region. The mismatch near to 3'-end of upstream primer was itnroduced to create a restriction site for Mun I enzyme in N363 allele. The S363 allele was identified by loss of mun I restriction site. No significant difference in genotype frequency has been observed between both groups (80.8 p.c. NN and 19.2 p.c. NS in INS versus 91.2 p.c. NN and 8.8 p.c. NS in control group, p = 0.08). In addition, no assocaition between GCR genotypes and gender, age of INS onset, INS recurrences, serum concentrations of creatinine and cholesterol as well as values of blood pressure was observed in INS group. The results of our preliminary study suggest the lack of association between the N363S polymorphism of the glucocorticoid receptor and both the predisposition to INS and the prognosis in Polish children with idiopathic nephrotic syndrome.

    Streszczenie polskie: Wyciągi z korzeni Rhodiola rosea L. zebranych ze środowiska naturalnego (Babia Góra, Karpaty) oraz Ogrodu Roślin Leczniczych Instytutu Roślin i Przetworów Zielarkich w Poznaniu, jak również wyciągi z kalusa in vitro, zmniejszały wzrost trzech szczepów Staphylococcus aureus: ATCC 6538 P, ATCC 25923 i NCTC 4163. Najbardziej wrażliwy był S. aureus NCTC 4163. Wyciągi z korzeni były bardziej aktywne. Powodowały większą strefę zahamowania wzrostu.

    Streszczenie angielskie: Extracts of Rhodiola rosea obtained from intact plant roots harvested from the Medicinal Plants Garden of the Institute of Medicinal Plants in Poznań, from plants growing in the wild (Babia Góra in the Carpathian Mountains) and from callus cultivated in vitro reduced the growth of the three Staphylococcus aureus strains: ATCC 6538 P, ATCC 25923 and NCTC 4163. S. Aureus NCTC 4163 was the most sensitive to their action. The extracts of intact plant roots produced a higher growth inhibition zone than callus extracts.

    Streszczenie angielskie: Spectroscopic, photochemical and biological properties of indocyanine green (ICG) are presented. Light over 800 nm is effectively absorbed by ICG. This property as well as photochemical behaviour of ICG make it a very suitable dye for photodynamic treatment of melanoma cells. Cytotoxicity of ICG itself and the effect of photodynamic therapy (PDT) were evaluated by following the growth of human (SKMEL 188) and mouse (S91) melanoma cells. The surviving fraction of the cells irradiated (lambda ex = 830nm) vs non-irradiated, treated with the same dose of ICG, is significantly decreased (5- to 10-fold). These results show that ICG is a very promising dye for photodynamic therapy of melanomas.

    Streszczenie angielskie: The study includes data of 109 people who were examined by psychiatrists and psychologists of their pension demands concerning statement for the Regional Court in Lublin, in 1999-2001 period. The period of treatment and the frequency of calls to PZP bave been taken into consideration. The number of hospitalizations has also been taken into account. Then, pharmacological treatment was examined. Some discrepancies have been found among algorithms of pharmacotherapy and the practice of treatment. Insufficient and inconsistent application of psychotropic medicine has been proved. It turned out that 1/3 of men and 1/5 of women of the examined group weren't patients of PZP. They were treated by physician (family doctors) or neurologists.

    Streszczenie angielskie: Plasma cell leukemia (PCL) is a very rare variant of multiple myeloma (MM) occuring in about 2 p.c. of newly diagnosed patients. Plasma cell leukemia may develop during the course of MM (secondary PCL) or it can occur without any prior sign of MM (primary PCL). We report a case of aggressive primary PCL with unusual clinical, cytogenetic and molecular features. A 36-year-old male patient was first seen because of fever and bone pain. On the skin of his chest, back, abdomen, and palpebras, there were nodular infiltrations resembling urticaria. White blood cell count was 10.8x10**9/1 with 41 p.c. plasmacytes. Bone marrow aspiration was hypercellular, 93.5 p.c. of cells were atypical plasmacytes and plasmablasts. The cytogenetic analysis of G-banded chromosomes in bone marrow cells yielded the trisomy 8. The skin biopsy specimen showed intensive infiltrates of uninucleated blastic cells similar to those found in the bone marrow. Immunophenotyping of bone marrow and skin neoplastic cells showed CD45+, CD45R0+, CD68+, CD38+ and cytoplasmic kappa light chain+. The neoplasic cells stained negatively for lambda light chain, CD3, CD20, CD30, EMA, CD15, CD34, CD56 and factor VIII. The pattern of IgL genes rearrangement in the bone marrow aspirate, peripheral blood mononuclear cells, and skin specimens was examined by PCR analysis. All studied specimens showed three different Igkappa gene configurations suggesting that the neoplastic cells originated as a result of oligoclonal lymphoproliferation process. The patient received two courses of VAD (vincristine, doxorubicin, dexamethasone)...

    Streszczenie angielskie: Aplastic anemia (AA) may sometimes precede the diagnosis of acute lymphoblastic leukemia (ALL) in children. Such presentation of ALL is externally rare in adults and until now only few such cases have been reported. We present a 40-year-old male with ALL common type, which developed 14 months after the diagnosis of severe AA, successfully treated with corticosteroids. ALL was treated with standard induction chemotherapy but remission has not been achieved. The patient died 6 weeks after the diagnosis of ALL because of central nervous system bleeding. The pattern of IgH gene rearrangement analyzed with PCR method in bone marrow from the period of AA diagnosis and in peripheral blood mononuclear cells from ALL diagnosis showed two different monoclonal IgH configurations as the results of biallelic monoclonal rearrangement of IgH genes. The observed bands in both specimens were identical and indicated that leukemic cells originated from B-cell progenitor were also present in the bone marrow when AA was diagnosed. We suggest that molecular analysis of monoclonality in patients with AA may be important for proper selection of the rare cases of ALL first presenting as marrow aplasia.

    Streszczenie angielskie: Gaucher-like cells have occasionally been described in various haematological malignancies including Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma (MM) and chronic myelogenous leukemia (CML). A special type of this phenomenon is crystal-storing histocytosis or the so-called pseudo-pseudo Gaucher cells (PPGC) in which crystalline protein storage in macrophages is induced by paraproteinemia. Here we describe a 54-year-old man with an initial suspicion of Gaucher disease and monoclonal IgA gammopathy in whom a correct diagnosis of lymphoplasmacytic lymphoma (LPL) with massive infiltration of bone marrow and spleen by PPGC was confirmed by immunological, ultrastructural and molecular characterisation. The activity of leukocyte beta-glucocerebrosidase was only slightly elevated (7.3 nmol/mg protein/1h) which ruled out the diagnosis of classic Gaucher's disease. The patient received two courses of CHOP without improvement and anti-CD20 monoclonal antibody (rituximab) with only temporary stabilisation. Subsequently, he underwent splenectomy because of prolonged severe pancytopenia and a suspicion of hypersplenism. After splenectomy significant haematological improvement was observed. Following anti-CD20 therapy, changes in immunoprofile and morphology of tumour cells were evident. Before treatment the population of LPL was more divergent, with expression of LCA, CD20, CD38 and CD138. However, after the treatment, there were more mature plasma cells which no longer expressed CD20 antigen-this picture was more consistent with the diagnosis of plasma cell myeloma...
