    Streszczenie angielskie: Several animal experimental studies have shown that autologous skeletal myoblast transplantation into the area of postinfarction left ventricular injury results in an increase in segmental contractile performance related to contraction of cells differentated from transplanted myoblasts. We report the first case in Poland of the use of autologous myoblast transplantation in the treatment of postinfarction heart failure. Lack of major perioperative complications and expected benefits from myocardial regeneration in patietns with postinfarction heart failure justify the initiation of phase one clinical trial evaluating this method.

    Streszczenie angielskie: Background. In order to provide a tissue autotransplant to the post-infarcted heart, an in vitro culture of human myoblasts with the highest possible number of non-differentiated muscle-sperm precursors in a time frame between 3-4 weeks is required. Aim. To test the different medium supplements such as FCS (Fetal calf serum), HS (Human serum) and HSA (Human serum albumin) for the optimisation of in vitro culture of human myoblasts. Methods. A fragment of tissue from m. vastus lateralis was collected, then myoblast cells were isolaated and cultured in vitro. In order to isolate satellite cells, the combination of enzymatic digestion (collagenase, type 1) with mechanical agitation was used. The obtained cell suspension was distributed to the culture flasks covered with gelatine. Incubation was carried out in DMEM medium (Dulbecco's Modified Eagle Medium) for 2 weeks. Cells after the first passage were equally divided and placed to the media with different supplements (20 p.c. FCS, 20 p.c. HS, 3 p.c. HSA). After 1 week of culture the cell concentration, viability and stage of differentiation were evaluated. Results. After culture in medium with 20 p.c. FCS, cells demonstrated normal morphology and a myoblast monolayer was attached to the flask surface; no myocytes were observed. In a culture with medium containing 20 p.c. HS high numbers of the non-attached cells with a significant (19 p.c.) proportion of dead cells were present. HSA caused the cell differentiation from myoblasts into myocytes.     Streszczenie angielskie: Background. In-stent restenosis after percutaneous coronary intervention (PCI) is due to the proliferation of intima. Supplementation with L-arginine has been shown to improve endothelial function and decrease neointima proliferation in experimental animal model of restnosis. Aim. To assess the effects of L-arginine supplementation on neointima proliferation and endothelial markers as well as growth factor levels in patients after stent implantation. Methods. In this prospective, randomised, double-blind, placebo-controlled study 60 patients undergoing stent implantation received palcebo or L-arginine (200 mg/kg infused intravenously over 4 hours, 12 and 3 hours before PCI, and 500 mg over 10 minutes prior to stent implantation, followed by oral supplementation of 6 g/day for 14 days after PCI). Quantitative coronary angiography (QCA) and intracoronary ultrasonography (ICUS) were performed at baseline and after a seven-month follow-up period. Serum concentration of L-arginine was measured at baseline, before PCI, 24 hours after PCI, and 7 as well as 14 days after PCI. The transforming growth factor-beta (TGF-á), vascular endothelial growth factor (VEGF) and endothelin levels were assessed before PCI, and 24 hours as well as 14 days after the procedure. Results. No significant differences in the QCA or ICUS parameters were found between patients receiving L-arginine or placebo. 24 hours after stent implantation patietns who received placebo had significantly a higher increase in the endothelin serum concentration and a lower rise in the VEGF level than the patients who received L-arginine...

    Streszczenie polskie: Liczba przypadków pozawałowej niewydolności krążenia (pnk) ciągle zwrasta, a obecnie stosowane metody leczenia nie umożliwiają bezpośredniego wpływu na zmienioną zawałem tkankę. Proponowane, nowe podejście terapeutyczne oparte jest na inżynierii komórkowej i terapii genowej bądź też na połączeniu obu tych technik. Do tej pory jako źródło komórek do przeszczepu w miejsce blizny pozawałowej zaproponowano autologiczne komórki szpiku, fibroblasty, mioblasty oraz kardiomiocyty. Opracowana terapia genowa polega na podawaniu konstrukcji zawierających geny powiązane z procesem angiogenezy: VEGF (vascular endothelial growth factor), bFGF/FGF2 (basic fibroblast growth factor), PDGF-BB (platelet-derived growth factor BB) i HIF-1ŕ). Zarówno inżynieria komórkowa, jak i terapia genowa były testowane na modelu zwierzęcym. Większość protokołów przeszło pomyślnie I fazę badań klinicznych, w tej chwili na całym świecie trwają badania fazy II. Uzyskanie miarodajnych wyników pozwoli na zoptymalizowanie terapii i być może dostarczy pacjentom i lekarzom długo oczekiwanego rozwiązania problemu, jakim jest ciągle zwiększająca się ilość przypadków pnk. W przedstawionej pracy podsumowano nowe metody leczenia w pozawałowej niewydolności krążenia.

    Streszczenie angielskie: The number of cases of post-infarction circulatory insufficiency is still on rise and presently empoyed forms of therapy do not directly access to the pathologically mallformed tissues. The proposed, new therapeutic attempt is based on cellular engineering and gene therapy or combination of both. So far, there have been proposed autologous bone marrow cells, fibroblasts, myobasts or kardiomyocytes as the source fo tisssue transplants to the post-infarction scar. Gene therapy is based upon administration of constructs containing genes connected with angiogenic process: VEGF (vascular endothelial growth factor), bFGF/FGF2 (basic fibroblast growth factor), PDGF-BB (Platelet-derived growth factor) and HIF-1 alpha (hypoxia-inducible factor-1 alpha). As well cellular engineering as gene therpay were studied first in animal models. Majority of protocols were successful therefore phase I clinical trials began; at present even pahse II clincial trials have been initiated. Collection of the data will allow to optimize a therapy and perhaps will deliver to patients and physicians a long awaited solution, i.e. successful treatment of increasing cases of post-infarciton circulatory insufficiency. A present review summarizes new methods of treatment of post-infarcted heart.
