    Streszczenie polskie: Ciąża u kobiety chorej na padaczkę stanowi poważny problem medyczny. Celem pracy była analiza stosowanego leczenia p-w padaczkowego u kobiet w ciąży, występujących powikłań oraz analiza stanu noworodków w badanej grupie. Analizą retroskektywną objęto 84 kobiety ciężarne chorujące na padaczkę. Wśród rodzących było 51 pierwiastek i 33 wielorodki, z czego w 23 przypadkach była to druga, a w 10 trzecia i wyższa ciąża. Grupę kontrolną stanowiło 80 wybranych losowo zdrowych kobiet rodzących i hospitalizowanych w tym samym okresie. Średnia wieku badanych kobiet wynosiła 25 lat, zaś średni czas trwania choroby 8,6 lat. U 45 pacjentek występowały napady uogólnione maksymalne, u 39 napady częściowe. U 26 pacjentek stosowano monoterapię, zaś u pozostałych 58 politerapię. Stwierdzono, że podczas ciąży częstotliwość napadów wzrosła u 13 kobiet, zmalała u 19, nie ulegla zmianie u 52 kobiet. Z powikłań podczas ciąży najczęściej obserwowano infekcję dróg moczowych, nadciśnienie indukowane ciążą, gestozę, plamienie. Rozwiązanie w grupie chorych na padaczkę siłami natury nastąpiło w 40 i cięciem cesarskim w 44 przypadkach. Wady rozwojowe stwierdzono u 4 noworodków: u dwóch wady serca i u jednego wadę rozwojową palców; matki tych dzieci przyjmowały Phenydantin. Natomiast w 1 przypadku stwierdzono przepuklinę oponowo-rdzeniową i wqodogłowie, a matka tego dziecka stosowała w leczeniu Convulex. W grupie kontrolnej u noworodków nie stwierdzono wad rozwojowych. Przeprowadzona analiza pozwala zaobserwować, iż w grupie kobiet w ciąży chorujących na padaczkę częściej niż w grupie kontrolnej występują powikłania podczas ciąży...

    Streszczenie angielskie: Pregnancy in woman with epilepsy arouses several serious medical problems and always belongs to the group of high obstetric risks. The aim of the present clinical study was the evaluation of the antiepileptic treatment efficiency during pregnancy, including risk factor, effects on pregnancy and delivery in epileptic patients. The study group consisted of 84 epileptic pregnant women which delivered between 1992-1998 in Obstetric Departments of University Medical School of Lublin. A randomised group 80 healthy pregnant women constitued the control group. The mean age of the analysed patients was 25 years. 51 epileptic patients were pregnant for the first time, 23 patients for the second time and 10 patients for the third time or more. The mean time of the disease was 8.6 years. In our study group: 45 (53.8 p.c.) patients experienced primary generalized tonic-clonic seizures and 39 (46.6 p.c.) patients experienced patial seizures. 26 patients were treated with monotherapy and the rest with polytherapy methods. The estimation of the seizures frequency during pregnancy in 52 (61.9 p.c.) patients did not change, in 13 (15.4 p.c.) patients increased. Among obstetric complications: urinary tract infections, hypertonia (EPH-gestosis) were observed. In 4 newborn congenital defects have been noted. Mothers of three of them were treated with Phendydantin (heart lesion, developmental anomaly of fingers). The fourth mother used Convulex (meningoarchided hernia, hydrocephalus).

    Streszczenie angielskie: Premature rupture the membranes (PROM) is considered and additional risk factor for pregnancy outcome. The aim of this report to evaluate the effects of reterm PROM on perinatal management, neonatal well-beineg and risk of neonatal and maternal infection as compared to preterm pregnancy not complicated by PROM. Perinatal management and outcome were analyzed in 68 patients (74 neonates) who delivered preterm with PROM-group I, and 141 women (154 neonates) who delivered preterm without PROM-group II. Mode of delivery was not significantly different between groups. Antibiotic therapy was instituted in 31 p.c. of patients without PROM and in 90 p.c. of patients with PROM. Despite this mode of management, the incidence of infection in neonates of mothers without PROM was lower than in those of mothers with PROM (53 p.c. vs 66 p.c.). No statistical differences were observed between the groups with respect to gestational age at delivery. Despite this observation, neonates from group I had significantly lower birthweight and lower Apgar scores as compared to group II neonates. Laboratory measures of infection (WBC, CRP) were significantly higher in group II compared to group I. Preterm PROM is a significant additonal risk factor for the neonate. It appears that management standards must be proposed in order to reduce the incidence of infection in premature neonates born from PROM-complicated pregnancies.
