    Streszczenie angielskie: Background: The aim of our research was to evaluate the influence of general anesthesia with controlled nitroglycerin-induced hypotension on splanchnic mucosal perfusion using gastric tonometry. Material/Methods: In six patients (1 female, 5 males, median age 46 years, range 32 - 62) undergoing elective resection of meningiomas, measurements of gastric mucosal perfusion were taken by nasogastric tonometry. Controlled hypotension was initiated after craniotomy and terminated prior to hemostasis procedures. The mean arterial blood pressure (MAP) was maintained 25 - 30 p.c. below initial parameters during controlled hypotension. The adequacy of perfusion was defined by the intramucosal pH (pHi) and the difference between arterial and intramucosal pH (pHGAP). All parameters were analyzed before the induction of controlled hypotension, after the 1st, 2nd, and 3rd hour, and 1 hour after the termination of controlled hypotension. Results: The MAP decreased during controlled hypotension by ca. 26.5 p.c. (min. 21.1 p.c.; max 31.6 p.c.). The lower MAP was accompanied by a statistically significant increase in heart rate. A significant increase from baseline in regional prCO2 was seen after teh 1st, 2nd, and 3rd hour of controlled hypotension. Intramucosal pH values decreased significantly during the same period, from 7.428 ń 0.032 to 7.372 ń 0.015. Despite these statistically significant differences during controlled hypotension, the pH, values were always greater than 7.35, and the pHGAP values were lower than 0.05, which were recognized as being within the normal range. Conclusions: Controlled hypotension allows accurate evaluation of the blood flow quality at the level of microcirculation. Mild controlled hypotension slightly alters splanchnic mucosal perfusion.

    Streszczenie angielskie: Background: Leptin plays an important role in controlling safety and mainatining energy balance. Acromegaly is characterized by decreased fat, which increases after the disease is cured. Out objective was to investigate serum leptin in acromegaly in terms of disease activity, body fat content, insulin and glucose levels, and selecated anthropometric variables. Mateirl/Methods: We examined 40 patietns with acromegaly and 200 sex- adn age-matched controls for the levles of serum GH, IGF-I, leptin, glucose, and insulin, and for body composition by DEXA, BMI and WHR. In 10 cases the acute ef fect on serum leptin of a somatostatin analogue, lanreotide, was studied. Results: We observed lower elptin in patients with active acromegaly than in cured patients and controls. Body fat was higher in cured than active patients. In the patients, we found significant correlations (p 0.05) between leptin and percent body fat (r = 0.77), leptin and body fat mass (r = 0.74), leptin and fasting insulin (r = 0.62), leptin and fasting insulin/glucose ratio (r = 0.97), leptin adn BMI (r = 0.44), leptin and height (r = -0.47). In the controls there was a significant correlation (p 0.05) o0nly between leptin and WHR (r = -0.45). A paradoxical decrease of the leptin after lanreotide was observed in ;7 out of 10 patients with active acromegaly. Conclusions: Changes in leptin release in acromegaly are related to differences in body fat content and mass, and in insulin resistance. Leptin in acromegaly is not influenced directly by GH or IGF-I secretion. The acute effect of medical treatment of acromeagly by a somatostatin analogue on leptin levels differes from teh effect of a radical cure following pituitary adenoma surgery.
