Streszczenie polskie:
W przebiegu przewlekłych kłebuszkowych zapaleń nerek (KZN) może dochodzić do zaburzeń w systemie czynnika insulinopodobnego (IGF). Celem pracy była ocena stężenia IGF-I, białka wiążącego IGF (GFBP-3) oraz hormonu wzrostu (GH) w surowicy dzieci chorych na przewlekłe KZN. Badaniami objęto 60 dzieci chorych: 11 na KZN (podgrupa Ia), 49 na KZN i zespół nerczycowy (ZN) (podgrupa Ib) oraz 15 dzieci zdrowych, stanowiących grupę kontrolną. Oceniono wyniki badań biochemicznych krwi, wielkość białkomoczu, masę ciałą, wzrost oraz wskaźnik masy ciała (BMI) badanych. Stwierdzono istotnie wyższe stężenie GH, IGF-I i IGFBP-3 w całej grupie oraz w obu podgrupach (Ia i Ib) w porównaniu z grupą kontrolną, natomiast nie wykazano różnic stężenia badanych parametrów między podgrupami chorych. Nie stwierdzono istotnych korelacji między wartościami GH, IGF-I, IGFBP-3 a stężeniem białka całkowitego, albumin, cholesterolu w surowicy, wielkością dobowego białkomoczu ani zależności stężenia GH, IGF-I, IGFBP-3 od płci, wieku dziecka, czasu trwania choroby, parametrów rozwoju fizycznego. Wzrost stężenia IGF-I i IGFBP-3 w surowicy dzieci chorych na przewlekłe KZN można wiązać z wyższą aktywnością GH u tych chorych, a także z ich prawidłowym stanem odżywienia.

In the course of chronic glomerulonephritis (CGN) dysfunctions in the system of the insulin-like growth factor (IGF) are possible. The aim of this study was to estimate the IGF-I, IGF binding protein-3 (IGFBP-3) and growth hormone (GH) concnetration in children with CGN. The tests were applied to the 60 children with CGN: 11 with CGN only (subgroup Ia) and 49 with CGN and nephrotic syndrome (subgroup Ib); and to the control group of 15 healthy children. The indicator of biochemical tests of blood, proteinuria, body-mass, height and BMI were analyzed. It was found that concnetrations of GH, IGF-I, IGFBP-3 in the group I were significantly higher than in the control group, but there were no significant differences between the subgroups. No significant correlations between the values of GH, IGF-I, IGFBP-3 and the values of total protein, albumine, cholesterol concnetrations in blood and 24h proteinuria were discerned. There was also no correlation between the values of GH, IGF-I, IGFBP-3 and the sex, age, disease duration and parameters, of children's physical development. The higher concentration of IGF-I, IGFBP-3 in children with CGN might be related to the higher GH activity in these children, as well as to their good nutritions state.

In some individuals infected with HCV, the anti-HCV is not detectable and HCV RNA is the only marker of the infection. Since the formation of certain antibodies is MHC restricted, we evaluated whether there are differences in the HLA DRB1 allele frequency between anti-HCV negative and positive hepatitis patients, and whether the DRB1 frequency difers between patients with self-limited and chronic infection. Material: 167 HCVRNA positive (37 anti-HCV negative to end of follow-up) patients, including 42 with acute hepatitis and 125 chronic hepatitis patients; control group of 286 healthy individuals. After the follow-up for at leats 30 months the patients were divided into 2 groups: those with self-limited and those with chronic hepatitis. Other reasons of hepatitis and obvious immune defects were excluded. Methods: HCV RNA by RT PCR, antiHCV by the third generation EIA; DRB1 by INNOLiPA DRB1. Results: We found no difference in the frequency of DRB1 alleles between the healthy individuals and HCV infected patients nor between the anti-HCV negative and positive patients. However, the frequency of DRB1*15 and DRB1*11 observed in the patients with self-limited infection was significantly higher than that in the patients with chronic infection. This difference was independent of anti-HCV delectability.
