    Streszczenie angielskie: Idiopathic pulmonary fibrosis (IPF) is the most common interstitial lung disease and is usually associated with a poor prognosis. IPF patients have a significant impairment in health-related quality of life (HRQL), both in terms of physical and psychological functioning. A substantial number of dyspnea scales are available to clinicians for clinical and research use. The objective of the present study was to investigate how some commonly used dyspnea scales correlate with measurements of HRQL in IPF patients. A group of 30 patients with IPF who attended a specialized outpatient respiratory clinic were interviewed during a stable period of the disease. HRQL was assessed by the SF-36 questionnaire. Dyspnea was measured by the baseline dyspnea index (BDI), Modified Medical Research Council Scale (MRC), Oxygen Cost Diagram (OCD), clinical element of the Clinical, Roengenographic and Physiologica score (CRP), and a new dyspnea scale (NS) based on estimates of required oxygen consumption for usual activities. Patients showed impariments in all HRQL domains except the Pain Index. All dyspnea scales showed significant correlations with both physical and mental HRQL domains. The highest Spearman's coefficients between dyspnea ratings and SF-36 components were obtained using the BDI. Dyspnea ratings correlate with HRQL measurements in IPF patients. Dyspnea scales can be used as a measure of HRQL in special circumstances.

    Streszczenie angielskie: Xenotransplantation represents a promising solution to the ever increasing shortage of donor organs in allotransplantation. However, due to different and stronger models of rejection, successful xenotransplantation will require different organ-protective regimes from those used in allogenic transplantation today. Since one can simply increase the dosage of the drugs used, immunomodulation or tolerance induction of the recipient would be the most desirable approach. Transfusion of donor leukocytes has been shown to downregulate recipient responses or even induce peripheral tolerance in small animal models. Since the infusion of donor cells represents a relatively simple approach, as one can purify and compose the inoculum exactly before infusion, we studied whether this approach can be successfully employed in a preclinical swine to nonhuman primate model of peripheral tolerance induction/immunomodulation. In our model, baboons underwent sequential column adsorption and complement blockade. The animals received only initial immunosuppression with cyclophosphamide. No further immunosuppression was given. Subsequently all animals received 1-3x10**10 porcine splenocytes i.v. Development and maintainance of chimerism was analyzed by sequential flow cytometric and PCR analyses. Other parameters studied included effects of the preparatory induction protocol. We could show that a low level of chimerism is maintained in these animals for up to 1,5 years, despite the fact that they received no additional immunosuppression after the initial one.     Streszczenie angielskie: Background: Autistic Disorder is an early-onset developmental disorder with severe lifelong impact on social functioning, communication, and behavior. There is currently no marker or cure. The pathophysiology and etiology are obscure. Evidence for abnormal gamma-aminobutyric acid (GABA) function in Autistic Disorders is limited. A few case-reports and small studies have reported differences in GABA levels in plasma, platelets, and urine, compared to controls. Further studies on abnormalities of GABA function in Autistic Disorder are warranted. Material/methods: Plasma GABA levels were measured using a new and sensitive technique, based on gas chromatography/mass spectrometry, in a small group of youngsters with Autistic Disorder and Attention-Deficit/Hyperactivity Disorder. Participants were outpatients between ages 5-15, satisfying modern criteria for these disorders. Results: Elevated plasma GABA levels were found in youngsters with Autistic Disorder. Psychotropic medications did not seem to affect plasma GABA levels in this study. Plasma GABA levels decreased with age. Conclusions: Elevated plasma GABA levels may be a biochemical marker of Autistic Disorder. This study supports the hypothesis that GABAergic mechansms play a role in the etiology or pathophysiology of Autistic Disorder. However, the hypothesis remains unspecified owing to lack of research. Future studies on the clinical associations of seizure disorders, mood disorders, and catatonia in autistic people may provide the necessary data to formulate a coherent theory of GABA dysfunction in Autistic Disorder. More trials of medication with known or suspected effects on GABA function are qarranted.

    Streszczenie angielskie: Although it is generally accepted that free radicals are involved in the neurodegenerative process occurring in the dopaminergic neurons of the nigro-striatal system in Parkinson's disease, the exact mechanims of neurodegeneration in vivo is still unknown. We propose that the degeneration of dopaminergic nigrostriatal system in this condition may depend on: (a) existence of free dopamine which oxidizes to aminochrome as a consequence of: (i) overproduction of dopamine, (ii) inhibition and/or low expression of synaptic vesicle catecholamine transporter, (iii) inhibition or low expression of monoamine oxidases; (b) one-electron reduction of aminochrome to leukoaminochrome o-semiquinone radical, which induces neurotoxicity, due to inhibition of DT-diaphorase or the existence of a polymorphism wiht a point mutation (C- T) in the cDNA 609 expressing an inactive DT-diaphorase. We suggest that DT-diaphorase plays an neuroprotective role in dopaminergic neurons, which is supported by the following observations: (i) Cu-toxicity is dependent on DT-diaphorase inhibition with diocoumarol in RCSN-3 cells derived from the substantia nigra, (ii) the cytotoxic effect of monoamine oxidase-A inhbitor amiflamine in RCSN-3 cells is increased by 2.4-fold (p 0.001) in the presence of the inhibitor of DT-diaphorase, dicoumarol, (iii) concomitant intracerebral administration of manganese (Mn3+) together with the DT-diaphorase inhibitor dicoumarol into the left medial forebrain bundle produced a behavioral pattern characterized by contralateral rotational behavior when the rats were stimulated...

    Streszczenie polskie: Wstęp: Nerwiakowłókniak jest łagodnym, wolno rosnącym nowotworem nwerwów obwodowych, składającym się głównie z komórek Schwanna, fibroblastów, oraz komórek przypominających komórki onerwia. Może przybierać postać pojedynczych lub mnogich guzów i wywodzić sie z jakiegokolwiek nerwu. Z wyglądu nerwiakowłókniak jest twardą, szarobiałą, nieotorbioną masą o wielkości od kilku milimetrów do kilku centymetrów. Występują dwa typy nerwiakowłókniaków - pojedyncze i w kształcie splotów. Pojedyncze nerwiakowłókniaki są dobrze odgraniczone i posiadają gumowatą konsystencję. Nerwiaki w kształcie splotów tworzą wieloogniskowe śluzowate zmiany opisywane często jako "skupisko robaków" i występują zazwyczaj w przypadkach choroby von Recklinghausena (Neurofobromatoza typu 1; NF1). Opis przypadku: 45-letnia biała kobieta, z zawodu nauczycielka, zgłosiła się z pojedynczym guzem koloru cielistego w prawym dole podkolanowym, który rozwijał się od 6 lat. W czasie kilku miesięcy poprzedzających hospitalizację zaobserwowała intensywny wzrost guza. Zmiana była bezbolesna i nie powodowała świądu. Wyniki wszelkich badań fizykalnych były prawidłowe z wyjątkiem obecności miękkiej różowawej masy na powierzchni zgięcia prawego stawu kolanowego o wymiarach 10x5x3 cm z owrzodzeniem w częsci środkowej. Wyniki wszystkich badań dodatkowych utrzymywały się w granicach normy. W wyniku przeprowadzonego później badania patologicznego ustalono diagnozę nerwiakowłokniaka.     Streszczenie angielskie: Background: Massive pulmonary embolism (MPE) is a potentially fatal event that can occur at any age. A possible form of presentation is through a syncope, whereas the appearance of an atrioventricular (AV) block is exceptional. Although thrombolysis therapy is recognized to be efficacious in pulmonary embolism that courses with shock or cardiorespiratory instability, improving the situation, there has been no report to data of an AV block from an MPE that disappears after administration of thrombolysis. Case report: We describe a 35-year-old woman who developed a high-grade AV block from an MPE that progressed to shock. In the presented case, the AV block and shock symptoms disappeared after the administration of alteplase. Conclusions: Pulmonary embolism should be considered in the differential diagnosis of atrioventricular block. Evidently, the prognosis of the block will depend on the evolution of the MPE. Early thrombolytic therapy of the PE abates shock symptoms and may restore heart rhythm.
