Background: The purpose of this study was to investigate microvascular perfusion in insulin-dependent diabetic patients at various locations on the foot, and to determine which part of the foot is most sensitive to microangiopathic changes. All the parameters of postocclusive reactive hyperemia calculated from multichannel laser Doppler recordings were also evaluated to find the most valuable measure for diabetes examination. Material/Methods: Our study involved 65 subjects divided into four subgroups: male and female controls, and male and female IDDM patients without overt complications. The measurements were performed with a multichannel laser Doppler perfusion monitor using surface probes located in the distal parts of the lower limbs. The occlusion test was performed using a cuff located on the limb above the knee. Multivariate discriminatory analysis was used to evaluate the data. Results: The most valuable data were obtained by recordings from the laser-Doppler probes located on the hallex and the base of the little toe. The maximum hyperemic response for both sex subgroups was significantly lower in the diabetic patients. The time to peak flow was higher in male diabetics. The half-time for hyperemia was significantly longer in the male diabetic patients. Cocnlusions: The females showed smaller changes in foot perfusion than the males, probably due to protection by estrogens. The best locations for perfusion measurement are the most distal, especially the hallex and the base of the little toe. The most valuable parameters of postocclusive hyperemia were maximum response, time to peak flow, and half-time of hyperemia.

Optical methods from the infrared range, especially Laser-Doppler flowmetry and near infrared spectroscopy offer a new non-invasive, real-time technique for monitoring of blood perfusion and oxygenation in living tissue. In spite of some instrumental problems e.g. relative calibration, unknown sampling measurement depth, these methods are already used in clincial studies. In the paper the methods principle, technical development and selected clinical applications have been described.

The article presents the review of the Hall-effect applications in measurement technique, especially the research in this field carried out some time ago at the Institute of Biocybernetics and Biomedical Engineering PAS. The idea and constructions of many Hall-effect devices e.g. teslometers, electric power transducers, mechano-electric transducers for displacement and vibration measurements, seismograph and acceleration transducer are described in the article. Special attention was paid to biomedical applications of Hall-generators like mechanocardiograms and tremor recording as well as to the present status of the Hall-effect applications in measurements and automatic control.

Celem pracy była szczegółowa ocena występowania wczesnych zaburzeń mikrokrążenia w zakresie stóp u osób z nowo wykrytą cukrzycą typu 1. Wcześniejsze nasze obserwacje wykazały, że zastosowanie metody laserowo-dopplerowskiej pozwala na monitorowanie zmian mikrokrążenia powodowanych przez świeżo wykrytą cukrzycę. Badaniem objęto 50 pacjentów: 26 pacjentów ze świeżo wykrytą cukrzycą typu 1 oraz 24 zdrowe osoby bez cukrzycy w wywiadzie rodzinnym (kontrola). Przeanalizowano 7 parametrów opisujących reakcję przekrwienną po okluzji. Do badań zastosowano urządzenie laserowo-dopplerowskie firmy Oxford Optronix (W. Brytania) umożliwiające jednoczesne pomiary ukrwienia tkanki przy użyciu zestawu 12 sond powierzchniowych. U chorych z nowo wykrytą cukrzycą typu 1 w porównaniu do grupy kontrolnej stwierdzono w najbardziej dystalnej części kończyny pacjenta istotne statystycznie (p 0,05) zmniejszenie wartości zera biologicznego odniesionego do przepływu spoczynkowego BZ/RF (wartość sygnału podczas okluzji). Parametr ten wydaje się być najbardziej reprezentatywnym wskaźnikiem umożlwiającym monitorowanie wczesnych zmian mikrokrążenia powodowanych przez cukrzycę. U osób z nowo wykrytą cukrzycą typu 1 zastosowanie metody laserowo-dopplerowskiej umożliwia wykrycie zaburzeń mikrokrążenia w zakresie stóp.
