    Streszczenie angielskie: Classical and ultrastructural neuropathology of prion diseases are generally well described. Here we report that alternations of myelinated fibres in hamsters infected either with polioencephalopathic strains of scrapie or panencephalopathic strains of CJD (Echigo-1) are virtually identical and differ only quantitatively. In contrast, mice infected with the panencephalopathic Fujisaki strain of CJD exhibited much more elaborate changes of myelinated fibres.

    Streszczenie angielskie: We report hare the microglial (macrophage) and astrocytic reaction in several models of transmissible spongiform encephalopathies (TSEs) or prion diseases. With the low power electron microscopy it was readily apparent that myelinated vacuoles were surrounded by cells and their processes. The latter belonged either to hyperplastic reactive astrocytes or to macrophages. Typically, reactive astrocytes exhibited cytoplasm filled with innumerable glial filaments and, occasionally, other organelles (like cilia) and abundant tortuous intercellular junctions of adhesive plaque junction type. Desmosome-like junctions connecting astrocytic elements were also seen. As described earlier, astrocytic process were occasionally interdigitated with oligodendroglial cells and their processes. Two types of macrophages were readily described. The majority of them exhibited electron-dense cytoplasm and numerous "empty" vacuoles (digestive chambers) containing cellular debris. Occasional vacuoles were surrounded by a thin collar reminiscent of "lyre-like inclusions" of the second type of macrophages. Several mylinated fibres were clearly engulfed by the cytoplasm of a macrophage containing unusual annulate lamellae.

    Streszczenie angielskie: As in other neurodegenerative diseases such as Alzheimer's disease, neurons in prion diseases or transmisible spongiform encephalopathies (TSEs) die via programmed cell death of which the apoptotic process is relatively well characterized. A subcellular alteration linked to apoptosis is the formation of autophagic vacuoles, which we and others demonstrated in CJD- and scrapie-affected rodent brains. Autophagy may co-exist with apoptosis or may precede it and the process may be induced by apoptosis stimuli. Here, we extend these observations using different model of scrapie and CJD. Both scrapie models (the 265K and 22C-H) demonstrated autophagic vacuoles with the same frequency; hence, they will be described together. While the following changes had been observed simultaneously in different areas of the same sample, this description is organised as if it followed a sequence of events. First, a part of the neuronal cytoplasm was sequestrated by concentric arrays of membrane; that part of the cytoplasm closed by membranes appeared relatively normal but its density often appeared increased. Next, electron density of the central dramatically increased. Then, membranes proliferated within the cytoplasm in a labyrinth-like manner and an area sequestrated by these membranes enlarged and became more complex structure consisting of vacuoles, electron-dense area and areas of normally-looking cytoplasm connected with convoluted membranes. Finally, a large area of the cytoplasm was transformed into a collection of autophagic vascuoles of different sizes. Virtually identical alternations, albeit with much lower frequency, were seen in terminally ill CJD-affected hansters.

    Streszczenie angielskie: Prion diseases are a group of etiologically heterogenous diseases. In addition to familial cases linked to mutations of PRNP open reading frame they include also cases of unknown etiology. One of the susceptibility factors to sporadic as well as iatrogenic prion diseases are PRNP polymorphisms. In the present study, we analyzed sequences of the PRNP gene codon 219 of 16 Polish CJD cases and we found heterozygous GAG to GAT changes on the sense strand and only wild type sequences on an antisense strand. The RFLP technique was used to verify this divergence and only wild type sequences were revealed.

    Streszczenie polskie: Choroby peroksysomalne są heterogenną grupą genetycznie uwarunkowanych chorób o niezwykle złożonym tle i słabo poznanej patogenezie. Choroby peroksysomalne podzielone są na dwie kategorie: - wywołane zaburzeniami biogenezy peroksysomów, powodującymi brak lub niedostatek normalnie funkcjonujących organelli; - powodowane defektem jednej lub wielu peroksyn bez uszkodzenia struktury peroksysomu. Do pierwszej grupy należy zespół Zellwegera (ang. Zellweger syndrome, ZS) oraz jego łagodniejsze warianty - adrenoleukodystrofia noworodków (ang. neonatal adrenoleukodystrophy, NALD) i dziecięca postać choroby Refsuma (ang. infantile Refsume disease, IRD). Chorobom tym towarzyszą te same zmiany biochemiczne i morfologiczne o różnym nasileniu. Do drugiej grupy zalicza się: - adrenoleukodystrofię sprzężoną z chromosomem X (klasyfikowaną dawniej jako choroba Schildera); - izolowane defekty degradacji VLCFA; - klasyczną chorobę Refsuma; - hiperoksalurię typu I. W pracy tej dokonujemy podsumowania badań molekularnych, obrazu klinicznego i neuropatologicznego całej grupy chorób.

    Streszczenie polskie: Przedstawiamy współczesne kryteria diagnostyczne wariantu choroby Creutzfeldta-Jakoba (vCJD), będącego wynikiem pasażu encefalopatii gąbczastej bydła (BSE) na człowieka. vCJD występuje u ludzi młodych (średnia wieku - 29 lat) i przebiega długo (kilkanaście miesięcy). Początkowo manifestuje się objawami psychotycznymi (lęk, agresja, zaburzenia zachowania, halucynacje), a następnie neurologicznymi (bolesne dysestezje kończyn, ruchy mimowolne, objawy piramidowo-pozapiramidowe). Otępienie dołącza się w poźniejszej fazie choroby. Charakterystyczny jest hiperintensywny sygnał w poduszce wzgórza w projekcji T2 MRI.

    Streszczenie angielskie: Apoptosis is a process of great significance in cell biology. It plays a crucial role in both physiological and pathological conditions. An example is a tumour growth that is based on a subtle balance between cell division and cell death. Previously necrosis was considered to be the major type of cell death in tumours. Many recent investigations have focused on apoptosis, a phenomenon of great importance, and it is this type of cell death that is frequently "chosen" by a moribund cell. The objects of our interest were central nervous system (CNS) tumours, in which we estimated the number of apoptotic cells and sought for any correlation between the intensity of apoptosis and other markers of proliferation. Therefore, we studied CD34, the marker of angiogenesis, and Ki67, the marker of cell proliferation. We investigated 19 medulloblastomas and 15 ependymomas, among which 6 were anaplastic. We used in-situ labelling of DNA fragments to detect apoptosis in paraffin-embedded tissues. The mean value of the apoptotic ratio (AR) for all examined brain tumours was 0.012 with a standard deviation (SD) of 0.032, for medulloblastomas 0.021 (SD 0.04), for ependymomas 0.001 (SD 0.002) and for anaplastic ependymomas 0.004 (SD 0.003).

    Streszczenie angielskie: We report here current concepts of the central nervous system tumours as promulgated by the recent WHO 2000 classification. The latter classification introduced several new concepts and outlined ner entities - for example, chordoid glioma and cerebellar liponeurocytoma. Besides, molecular biology plays the pivotal role for current understanding of tumour development. The discovery of two paths leading to the formation of glioblastoma and mixed gliomas are the best examples of the latter statement.

    Streszczenie polskie: Od dawna wiadomo, że migrena występuje rodzinnie, jednak dopiero doniesienia ostatnich lat dostarczyły danych o molekularnym podłożu tej choroby. W pracy tej omówiono genetykę rodzinnej migreny hemiplegicznej, migreny z aurą oraz rodzinnego klasterowego bólu głowy. Wyniki omawianych badań sugerują, że migrena jest, pod względem genetycznym, heterogenną grupą jednostek chorobowych.

    Streszczenie angielskie: In has been known for decades that migraine shows a strong familiar aggregation. Recent studies provide data on molecular basis of this common neurological disease. This paper reviews genetics of familial hemiplegic migraine, migraine with aura, and familial cluster headache. According to these data migraine seems to be a group of polygenetic multifactorial disorders.

    Streszczenie angielskie: Langerhans cell histiocytosis (LCH) is a rare neoplastic disease of specific dentritic cells which belong to the monocyte-macrophage system. The association of LCH with autoimmune disease is extremely rare and to our knowledge its coexistence with systemic lupus erythematous (SLE) has not been described so far. We report a case of LCH affecting liver, spleen and abdomen lymph nodes, which developed in an adult female six years after diagnosis of SLE treated for a long time with prednisone. Histology showed infiltration of characteristic Langerhans cells with folded, grooved or lobulated nuclei with fine chromatin. In the background there were eosinophils, lymphocytes and CD-68-positive histiocytes. The neoplastic cells were S100p-immunopositive, but stained negatively for CD1a-probably as the result of overfixation of consulted material. CD-68 was present mostly in macrophages. Ultrastructurally, the tumour cells presented structures consistent with Birbeck granules. Clonal origin of neoplastic cells was shown using the HUMARA-PCR assay. The disease was refractory to treatment with high doses of prednisone and vincristine but complete response was achieved after treatment with caldribine combined with cyclophosphamide.

    Streszczenie angielskie: Gaucher-like cells have occasionally been described in various haematological malignancies including Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma (MM) and chronic myelogenous leukemia (CML). A special type of this phenomenon is crystal-storing histocytosis or the so-called pseudo-pseudo Gaucher cells (PPGC) in which crystalline protein storage in macrophages is induced by paraproteinemia. Here we describe a 54-year-old man with an initial suspicion of Gaucher disease and monoclonal IgA gammopathy in whom a correct diagnosis of lymphoplasmacytic lymphoma (LPL) with massive infiltration of bone marrow and spleen by PPGC was confirmed by immunological, ultrastructural and molecular characterisation. The activity of leukocyte beta-glucocerebrosidase was only slightly elevated (7.3 nmol/mg protein/1h) which ruled out the diagnosis of classic Gaucher's disease. The patient received two courses of CHOP without improvement and anti-CD20 monoclonal antibody (rituximab) with only temporary stabilisation. Subsequently, he underwent splenectomy because of prolonged severe pancytopenia and a suspicion of hypersplenism. After splenectomy significant haematological improvement was observed. Following anti-CD20 therapy, changes in immunoprofile and morphology of tumour cells were evident. Before treatment the population of LPL was more divergent, with expression of LCA, CD20, CD38 and CD138. However, after the treatment, there were more mature plasma cells which no longer expressed CD20 antigen-this picture was more consistent with the diagnosis of plasma cell myeloma...
