    Streszczenie polskie: Opisano przypadek pierwotnej gruźlicy migdałka i ucha środkowego u 6-letniej dziewczynki. Zastosowano leczenie operacyjne. Badanie histopatologiczne migdałka i ziarniny z ucha wykazało obecność ziarniny gruźliczopodobnej. Również próba Rt23 była dodatnia. Dziecko skierowano do dalszego leczenia w Instytucie Gruźlicy i Chorób Płuc w Rabce.

    Streszczenie angielskie: The authors report a rare case of 6-year-old girl with primary tuberculosis of the adenoids and middle ear. This child was operated on. Histopathological examination indicated pseudotuberculous granulation tissue; it was also Rt23 positive. Further medical treatment was continued at the Institute for Tuberculosis and the Lung Disease in Rabka.

    Streszczenie angielskie: Bacground. Literature regarding transcathether closure of a single atrial septal defect (ASD) is very rich, however, there are only a few reports showing the possibility of the closure of multiple defects with a single device. Aim. To analyse feasibility and long-term results of closure of the multiple ASD with a single device. Methods. Of 128 patients treated with a transcatheter method, 26 (20 p.c.) had ultiple ASD. Patients were selected for invasive treatment on the basis of the results of transthoracic echocardiography (children) or transoesophageal echocardiography (adults). A significant left-to-right shunt with volume overload of the right ventricle and at least 6 mm rim of the defect were required to undergo transcatheter closure of ASD. Results. The procedure was performed with a single devive in 25 patients (Amplatzer occluder in 22, Starflex in 2, and Cardioseal in 1 patient) whereas in one patint with two distant and hemodynamically significant defects two Amplatzer occluders were implanted. A complete closure of the shunt was achieved in 59 p.c. (13/22) of patients after 24 hours, in 64 p.c. (14/22) - after one month, in 80 p.c. (16/20) - after three months, in 86 p.c. (13/15) - after one year and in 100 p.c. (7/7) - after two years of follow-up. There were no complications during or after the procedures. The follow-up period ranged from one to 48 (mean 19) months. Conclusions. Multiple secundum type atrial septal defects can be efectively closed with a single device. Strategy of transcatheter closure depends on the size and distance between the defects and streching capabilities of the septum.

    Streszczenie angielskie: Ventricular septal rupture is a severe complication of myocardial infarction. Experience in the transcatheter treatment in this condition is still limited. We present a 63-year-old patient with post-infarction ventricular septal defect (VSD) who developed cardiogenic shock prior to the elective surgical revascularisation. urgent cardiac catheterisation with the implantation of three coronary stents and VSD closure with the Amplatzeratrial septal occluder were performed. A uneventful course of the one year follow-up confirmed good results of this compex interventional treatment.

    Streszczenie angielskie: In the patients with complex cardiac anomalies after various palliative operations there are often difficulites to qualify them to the definite procedure. To facilitate the decision making we sometimes can perform the balloon occlusion test. Our experience in 6 such cases is presented.

    Streszczenie angielskie: 2-Deamino-2-methyl-N10-propargyl-5,8-dideazafolic acid (ICI 198583) is a potent inhibitor of thymidylate synthase. Its analgue, Nŕ-[4-[N-[(3,4-dihydro-2-methyl-4-oxo-6-quinazoliny)methyl]-N-propargylamino]phenylacetyl]-L]glutamic acid, containing p-aminohenylacetic acd residue substituting p-aminobenzoic acid residue, was synthesized. The new analogue exhibited a moderately potent thyidylate synthase inhibition, of linear mixed type vs. the cofactor, N1,10-methylenetetrahydrofolate. The Ki value of 0.34 ćM, determined with a purified recombinant rat hepatoma enzyme, was about 30-fold higher than that reported for inhibition of thymidylate synthase from mouse leukemia L1210 cells by ICI 198583 (Hughes et al., 1990, J. Med. Chem. 33, 3060). Growth of mouse leukemia L5178Y cells was inhibited by the analogue (IC50 = 1.25 mM) 180-fold weaker than by ICI 198583 (IC50 = 6.9 ćM).
