    Streszczenie angielskie: Background. The reduction of plasma LDL-cholesterol (LDL-Ch) by LDL-apheresis has been suggested to improve endothelial function and increase myocardial blood flow. Aim. To assess the LDL-apheresis-induced lipid changes on endothelial function and exercise tolerance. Methods. After one month of treatment with diet and statins 7 patients with total cholesterol (TCh) 6 mmol/l scheduled for coronary revascularisation underwent LDL-apheresis as well as simvastatin (20-40 mg/day) and diet therapy. LDL-apheresis was introduced 2-3 days before revascularisation and was continued (with 7-14 day intervals) for up to 3 months. The control 8 patients underwent coronary revascularisation and received simvastatin (20-40 mg/day) as well as diet therapy. The exercise test as well as the evaluation of vascular endothelial growth factor (VEGF) which is involved in atheromatous plaque progression and von Willebrand factor (vWF) (the endothelial damage marker) were performed before, immediately, 6 and 12 months afte LDL-apheresis. Results. LDL-apheresis decreased T-Ch by 49 p.c., LDL-Ch by 58 p.c., and triglyceride by 62 p.c. (p 0.01). The susceptibility of LDL fraction to oxidation was also decreased (p 0.01). The vWF level was significantly lowered from 123 ń 23.2 p.c. to 72 ń 12.7 p.c., and VEGF165 concentration was decreased from 39 ń 8 pg/ml to 15 pg/ml (below the detection level). After 12 months the VEGF and vWF levels remained significantly decreased (22 ń 4 and 105 ń 55, respectively). After 6 and 12 months the exercise tolerance was preserved in the LDL-apheresis group...
