Psoriasis vulgaris is a multifactorial disease; the strongest association was established with the HLA complex. The actual disease-predisposing gene(s) has not been identified yet, but several genes from this region were examined in addition to HLA-C and -B. However, HLA-linked complement component polymorphic genes were not extensively studied. Therefore, we typed 67 psoriatic patients for alleles of the HLA-linked complement components BF, C4A and C4B. Alleles of C3, encoded on another chromosome, were established in parallel as a negative control. Frequencies in patients were compared with thise in unrelated healthy controls, 100 individuals for C4A and C4B, 890 for BF and 4719 for C3. We found no association of BF alleles with disease, similarly to C3. Among C4 alleles, C4B*3 was present in 13,4 p.c. of patients as compared with 1 p.c. of controls (OR, 15.36; 95 p.c. CI, 1.897 - 124.42; P = 0.0009), and C4A*6 was present in 19.4 p.c. of patients versus 7 p.c. of controls (OR, 3.20; 95 p.c. CI, 1.202 - 8.508; P = 0.0155). The high frequency of C4B*3 in psoriatics has not been described so far. These results suggest a contribution of C4 genes themselves or a closely linked gene to the susceptibility to psoriasis.

Początkowo immunoterapia znalazła zastosowanie w chorobach infekcyjnych. Jej duża skuteczność stała się impulsem zachęcającym do wprowadzenia tej metody do leczeniea chorób alergicznych, w tym astmy oskrzelowej atopowej. Obecnie przyjmuje się, że w odróżnieniu od farmakoterapii usuwającej objawy chorobowe, swoista immunoterapia jest jednym leczeniem przyczynowym astmy. Aby osiągnąć skuteczność takiego leczenia musi być spełnionych kilka warunków. Właściwy dobór chorych i zastosowanie odpowiedniej etiologicznie szczepionki przez kilka sezonów przynosi sukces terapeutyczny. W astmie nieatopowej wewnątrzpochodnej a także w astmie atopowej powikłanej infekcjami bakteryjnymi należałoby rozważyć zastosowanie szczepionki bakteryjnej autologicznej albo wieloważnej (standardowej). Ponieważ w trakcie immunoterapii mogą wystąpić groźne dla życia powikłania w postaci reakcji anafilaktycznej, tylko lekarz alergolog może podjąć decyzje o zastosowaniu immunoterapii oraz nadzorować jej przebieg.
Streszczenie angielskie:
When first used, immunotherapy was applied to treatment of infectious diseases. Its high effectinesess gave an impulse to introduce this method in the treatment of allergic diseases, including nronchial asthma. In contrast to pharmacological treatment which treats only the symptoms in allergies, immunotherapy is presently considered to be the only causative treatment. Several conditions have to be fulfilled to obtain the effect of such treatment. Appropriate patients selection, use of the etilogically correct vaccine and continuing the treatment throughout several seasons are the conditions for the therapeutic success. In nonatopic intrinsic asthma or in atopic asthma complicated by bacterial infections the use of autologic or polyvalent (standard) bacterial vaccine should be considered. Because immunotherapy brings a risk of serious side effects such as anaphylactic reaction, only allergy specialist should be entiled to decide of the introduction of such treatment and to supresive its course.
