    Streszczenie polskie: Wprowadzenie. Postawową korzyścią wczesnego żywienia enteralnego w chirurgii jest pobudzenie rozwoju i czynności przewodu pokarmowego. Rola żywienia dojelitowego w chirurgii dorosłych znacznie wzrosła w ostatnich latach, u dzieci wymaga jednak dalszych badań ze względu na konieczność oceny tolerancji, korzyści i kosztów leczenia. Cel pracy. Celem pracy jest ocena tolerancji preparatów fabrycznych ubogo- i bezresztkowych Nutrison i Peptison firmy Ovita Nutricia, stosowanych do żywienia enteralnego u dzieci leczonych w oddziałach chirurgicznych. Materiał i metody. Dwie różne diety przemysłowe - Peptison i Nutrison (Ovita Nutricia) były podawane 30 pacjentom oparzonym, z guzami litymi i po zabiegach operacyjnych w obrębie przewodu pokarmowego, u których wskazania do żywienia standardu szpitalnego były ograniczone. U wszystkich pacjentów kontrolowano masę ciała, stężenia białek w surowicy krwi oraz jonów, a także oceniono tolerancję i akcepatację podawanych mieszanek. Wyniki. W czasie leczenia u 26 z 30 pacjentów masa ciała wzrosła o 10 proc. w stosunku do masy wyjściowej, u 3 chorych wzrosła powyżej 10 proc. w stosunku do masy wyjściowej, a u jednego pacjenta masa ciała zmniejszyła się o 14,8 proc. Stężenie jonów w surowicy krwi u wszystkich dzieci pozostawało w granicach normy. Stężenie albumin w surowicy krwi u 2 pacjentów mieściło się w przedziale 19-25 g/l, u 11 pacjentó wynosiło 26-30 g/l, u 15 pacjentów 31-35 g/l, a u 2 powyżej 35 g/l. U 2 chorych zaobserwowano ...

    Streszczenie angielskie: Background. Basic advantages of the early enteral nutrition in surgery are easy metabolic survey and stimulation of the alimentary tract. However the role of the enteral feeding in general surgery has markedly increased in last decades, its usefulness and tolerance in children needs further studies. Objectives. The aim analysis is to assess the usefulness and tolerance of the Nutrison - low in residue liquid feed and Peptison - complete liquid elemental feed (Ovita Nutricia) in the eneteral feding in paediatric surgical patients. Material and methods. Two modern formulas - Peptison and Nutrison (Ovita Nutricia) were administered to 30 patietns with malignancies, trauma and surgical diseases of the alimentary tract. Weight, serum ionogram, serum albumin, tolerance and patients acceptation of the taste of Peptison and Nutrison were evaluated. Results. In 22 of 30 patients the body weight did not change during the treatment over 10 p.c. of the initial weight, in 3 of them it increased slightly over 10 p.c. In one, the body weight decerased of 14.8 p.c. The serum ionogram did not exceed the normal limits in any patietns. The serum albumin were between 19 and 25 g/l in 2 patietns, between 26-30 g/l in 11, between 31-35 g/l in 15 and above 35 g/l in 2. In 2 patietns the tolerance of both formulas was altered by vomiting and diarrhoea.     Streszczenie angielskie: The purpose of the study was to present a modern classification and discuss the treatment of midfacial fractures in children. From the beginning of 1 January 1998 to 31 October 2000, 147 children were treated for different craniofacial fractures. Among them 28 patients (19 p.c.) had extensive midfacial fractures complicated by impaired vision and/or CNS dysfunction and were treated surgically. The fractures were divided into: zygomaticoorbital (1 pt) and zygomatico-orbito-maxillary (5 pts), isolated orbital wall fractures (14 pts) and naso-orbital dislocations (2 pts), upper facial portion dislocations (2 pts) and fronto-naso-orbital (2 pts) or cranio-orbital fractures (2 pts). Clinical examination revealed mainly dysfunction of facial morphology and aesthetics (enophthalmos, telecanthus), CSF leakage, impairment of vision and restricted eyeball movements. Ophthalmological, neurological and radiological (Waters view, CT, NMR) examinations were performed in all cases. The treatment consisted of bone repositioning, reconstruction with autologus bone grafts and three-dimensional stabilisation using titanium plates. Twenty-three pts were operated on before and 5 pts after 7th day after trauma. The restoration of normal facial morphology was achieved in 26 patients. In 2 cases it was imposisible because of the destruction of soft tissue and an extremely extensive bony trauma. Better reposition of bony fragments, prompt healing and better final results were achieved in patients operated before day 7 after trauma.     Streszczenie angielskie: Soft tissue sarcomas are heterogenous group of malignancies. It seems most useful to divide those tumours and guide the treatment regarding its chemosensitivity. Surgery remains important part of the treatment, but its role is different in each chemo-sensitivity group. In chemosensitive tumours primary surgery should not be mutilating. The pre-treatment offers great chance for secondary conservative complete local control. Also in case of a post-surgical micro-residue chemotherapy may help to achieve complete remission. In tumours of possible chemosensitivity, primary surgery offers best chance for long-term survival, however, patients with unresectable tumours may have a chance to be turned operable after pre-treatment. In case of post-surgical residue, chemotherapy alone usually does not offer the chance for long-term survival. Local re-treatment should be considered. In case of tumours of uproven chemosensitivity surgery is most reliable method of treatment and, also mutilating, is the first treatment. Microscopical residue requires either re-excision or radiotherapy. Only in case of very extensive tumours, the trial of chemotherapy may be tested. Concluding: 70-80 p.c. of long term survivors after combined treatment in 1990-ties compared to 20 p.c. after surgery alone in 1930-40-ties indicate the importance of chemotherapy. Experience of high rate of local relapses from 1980-ties after systemic treatment only (up to 40-50 p.c.) evidences changing, but crucial role of surgery.
