Physiological changes in the QT interval caused by sleep are important in the evaluation of the normal QT interval. Traditionally, Bazett's formula is used for correcting the QT interval for heart rate in childhood. We set out to establish the QT-RR relationship in infants during sleep and concurrently test the performance of four QT correction formulae to determine their comparability and clinical utility. 130 healthy term aborn infants undergoing routine 8-hour polysomnography were enrolled. Continuous EKG limb lead II recording, saturation monitoring, and electroencephalography were conducted. QT intervals and heart rate were measured manually digitally on-screen at hourly intervals. The QT-RR relationship was plotted and QTc calculated using the Bazett, Hodges, Fridericia and Framingham formulae. QTc values for each formula were plotted against age and RR interval. Analysis of variance and regression analysis were used to compare the four formulae and the QT-RR relationship. Mean age was 17.7 weeks (range 4 - 72) and mean weight 6.3 kg (range 2.68 - 12.7). The QT-RR relationship during sleep is curvilinear and appears to be slightly shifted. With increasing heart rate and younger age the Bazett and Hodges formulae overcorrect the QTc whereas the Fridericia and Framingham formulae undercorrect. The Hodges formula correlated best with the RR interval. These formulae are strongly dependent on heart rate. In the setting of a vorderline QTc Bazett prolongation, Hodges formula may be a useful adjunct. Normal infants may have a slightly longer QTc Bazett during sleep.

Celem pracy była ocena wczesnych wyników realloplastyki cementowych panewek stawu biodrowego. Ocenie poddano 67 chorych (53 kobiety i 14 mężczyzn) w wieku od 47 do 78 lat (średnia 72 lata), u których wykonano 68 operacji z powodu aseptycznego obluzowania panewki. Stwierdzono, że odpowiednio wczesne podjęcie decyzji leczenia operacyjnego może uchronić chorego przed rozległym i ryzykownym zabiegiem operacyjnym oraz poprawić wyniki końcowe. W przypadkach masywnych ubytków panewki miednicy, dobre wczesne wyniki przynosi zastosowanie przeszczepów kostnych z użyciem dodatkowych pierścieni i siatek wzmacniających strukturę panewki.

The aim of this paper was to assess early results of revision arthroplasty in aseptic loosening of cemented sockets. A group of 67 patietns (53 females and 14 males) age ranging from 47 to 78 years (average: 72 years) who underwent revision arthroplasty because of aseptic loosening of cemented sockets. Early surgical treatment can spare the patient from a risky procedure and lead to better end results. In cases with massive bone destruction of the pelvis good results can be achieved by using bone grafts and anti-protrusion cages.

This open-label, randomised, controlled study is aimed at assessing the effect of pre-treatment with the metabolic agent trimetazidine on the degree of ischaemia during percutaneous transluminal coronary angioplastry (PTCA). Overall 44 patients with one-vessel coronary artery stenosis ( 70 p.c.) in the medial part of the left anterior descending artery were included. One group (n = 22) was pre-treated with oral trimetazidine. The other group (n = 22) was the control. All patients (n = 44) were administered aspirin and conventional treatment. All patients underwent PTCA; stents were implanted in 11 trimetazidine patients and in seven control patients. The mean ST-segment elevation during all balloon inflations was significantly lower in the trimetazidine group than in the control group (-1.66 ń 1.50 mm vs. 3.29 ń 1.59 mm, p = 0.001). Maximal ST-segment elevations and mean ST elevation values during sequential balloon inflations were also significantly lower weith trimetazidine (p = 0.018). The mean amplitude of the T-wave alterations during all ballon inflations was significantly lower with trimetazidine (3.09 ń 2.39 mm vs. 6.83 ń 4.31 mm; p = 0.001). Similary, the maximal amplitude of the T-wave alterations was 4.50 ń 2.90 mm with trimetazidine vs. 9.25 ń 4.97 mm in control patients (p = 0.0005). Angina and rhythm disturbances were more frequent in the control group. Time from balloon inflation to onset of angina was 50 ń 26.2 s with trimetazidine vs. 32 ń 15.0 s, for control group (p = 0.03). The time to pain relief after deflation was 19.3 ń 11.4 s with trimetazidine vs. 28.2 ń 16.8 s (p = 0.001)...

This review describes the ionic mechanisms involved in the nodal swelling of frog myelinated axons caused by specific marine neurotoxins (ciguatoxins, brevetoxins, Conus consors toxin and equinatoxin-II), analysed using confocal laser scanning microscopy. We have focussed on toxins that either target neuronal voltage-dependent Na+ channels, or that form cationselective pores and indirectly affect the functioning of the Na+-Ca++ exchanger.
