Susceptibility to lung cancer may in part be attributable to inter-individual variability in metabolic activation or detopxification of tobacco carcinogens. The glutathione S-transferase M1 (GSTM1) genetic polymorphism has been extensively studied in this context; two recent meta-analyses of case-control studies suggested an association between GSTM1 deletion and lung cancer. At least 15 studies have been published after these overviews. We undertook a new meta-analysis to summarize the results of 43 published case-control studies including 18000 individuals. A slight excess of risk of lung cancer for individuals with the GSTM1 null genotype was found (odds ratio (OR) = 1.17, 95 p.c. confidence interval (CI) 1.07 - 1.27). No evidence of publication bias was found (P = 0.4), however, it is not easy to estimate the extent of such bias and we cannot rule out some degree of publications bias in our rsults. A pooled analysis of the original dsata of about 9500 subjects involved in 21 case-control studies from the international Collaborative Study on genetic Dusceptibility to Environmental Carcinogens (GSEC) data set was performed to assess the role of gsTM1 genotype as a modifier of the effect of smoking on lung cancer risk with adequate power. Analyses revealed no evidence of increased risk of lung cancer among carriers of the GSTM1 null genotype (age-, gender- and center-adjusted OR = 1.08, 95 p.c. CI 0.98 - 1.18) and no evidence of interaction between GSTM1 genotype and either smoking status or cumulative tobacco consumption.

Przedstawiono 6 przypadków choroby Takayasu rozpoznanej u 5 chorych na podstawie kryteriów ACR z 1990 roku, a u jednej chorej - na podstawie badań USG, tomografii komputerowej i biopsji aorty. Początkowe objawy choroby wystapiły u wszystkich chorych przed 45 rokiem życia; u wszystkich obserwowano opóźnienie od pierwszych objawów do ustalenia rozpoznania. U większości chorych obserwowano duszność, objawy dławicowe, osłabienie, stany podgorączkowe, gorączki. Wszystkie chore w czasie rozpoznawania miały przyspieszony OB i hiper-ŕ2--globulinemię. W badaniu USG wykonanym u pięciu chorych stwierdzono pogrubienie ściany aorty i/lub zwężenie jej odgałęzień. Wykonanie arteriografii nie było konieczne do rozpoznania choroby. Stwierdzono, że największe znaczenie we wczesnej diagnostyce choroby Takayasu ma dokładne badanie przedmiotowe układu krążenia i ocena wskaźników aktywności zapalnej. Podkreślono istotną rolę badania USG w tej chorobie jako badania alternatywnego do arteriografii.
Streszczenie angielskie:
The paper presents 6 cases of the Takayasu disease diagnosed in 5 patients on the basis of the ACR criteria of 1990, and in one patient on the basis of ultrasound scan, computerized tomography and aortic biopsy. The initial symptoms of the disease occurred in all patients before age of 45: in all there was significant delay between the onset of the disease and diagnosis. In majority of patients the following symptoms were observed: dyspnoe, angina-like symptoms, asthenia and fever. In ultrasound scan carried out in five patients there was aortic wall thickening and/or narrowing of the arterial branches. The arteriography was not necessary for the diagnosis. It was found that in early diagnosis of the Takayasu disease the most crucial parts is precise physical examination of the cardiovascular system and assessment of the inflammatory process indices. The important role of the ultrasound scan in this diseasee was stressed, as the test alternative to arteriography.

DNA damage repair, responsible for maintaining the genome integrity, plays a central role in cancer biology. Individual DNA reapir capacity is genetically determined. Inherited defect in nucleotide excision repair (NER) genes leads to three distinct and extreamely rare disorders: xeroderma pigmentosum, associated with high risk of skin cancer, Cockayne syndrome, and trichothiodystrophy. The recently identified common polymorphism in several NER genes may also influence a risk of cancer in general population. The review presents current knowledge about a role of genetic variation of NER genes in cancer predisposition.
