    Streszczenie polskie: Badaniem objęto 68 pacjentów operowanych z powodu otosklerozy. Badania audiometeryczne wykonano przed oraz w 8 - 20 dniu po operacji. Średni ubytek słuchu pacjentów przed zabiegiem operacyjnym wynosił 63,6 dB. Średnia poprawa słuchu w grupie chorych po stapedotomii wynosiła 26,6 dB, zaś po stapedektomii: 29,5 dB. Zmniejszenie rezerwy ślimakowej poniżej 10dB lub jej zamknięcie obserwowano w 74 proc. przypadków. Wczesne powikłania wystąpiły u 4 pacjentów pod postacią zawrotów głowy, zaburzeń równowagi, szumów usznych oraz uczucia rozpierania w uchu operowanym. Wnioski: 1. Leczenie operacyjne otosklerozy wpływa na poprawę lub zachowanie słuchu. 2. W pierwszych dnaich po operacji obserwuje się zaburzenia równowagi o niewielkim nasileniu.

    Streszczenie angielskie: The aim of the study is to assess the results of surgical treatment in otosclerosis patients. The study included 68 patients (51 women and 17 men) aged 19-61 (mean age: 42) operated on for otosclerosis. Material and method: 77 surgical procedures were performed (67 stapedotomies and 10 stapedectomies). The operation on the second ear was performed in 9 cases. Tonal audiometry and stapedius reflex were performed in all the patients. Mean haring loss was determined by applying five frequency; 250 Hz, 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. Audiometric examinations were done before and 8 to 20 days after the operation. Results: Mean hearing loss of the patients was 63,6 dB. Mean hearing improvement in the stapedotomy patients group was 26.6 dB and in the stapedectomy patients group: 29.5 dB. The laack of hearing gain was observed in 4 patients. A decrease of air- bone gap below 10 dB or its closure was shown in 74 p.c. of the cases. In 16 p.c. of the patients the air-bone gap after the surgical procedure ranged from 11 to 20 dB, in 10 p.c. of the cases overdrew 21 dB. Early complications, up to one month after the operation, appeared in 4 patients in the form of vertigo, balance disorders, tinnitus and the feeling pressure in the ear. In one patient, tinnitus in teh operated ear appeared 12 months after the surgery. Conclusion: The surgical procedrue in otosclerosis patients has improved their hearing. In the first few days after the operation, balance disorders were observed.

    Streszczenie angielskie: Background: The evaluation of affected lymph node number, the site of metastatic lymph nodes, and spread of neoplastic infiltration beyond the lymph capsule is considered useful in prognosis. The aim of the study was to estimate the frequency of neck nodal metastases occurrence depending on the site of origin, the grade of larynx cancer progresssion, and clinical condition of neck lymph nodes. Material/Methods: The study comprised 315 patients with larynx cancer who underwent surgery in 1994-1999. Laryngectomy was the most frequent procedure, performed in 254 patients, while partial laryngectomy was performed in 61 patients. There were 630 bilateral neck dissections; type I of modified radical surgery in 27 cases, type II in 45 cases, and type III in 558 cases. Results: Neck nodal metastases were detected most rarely in cases of glotic cancer (16 p.c.), and most frequently in transglottic cancer (56 p.c.). Most frequently, the nodal metastases affected the II level of the lymph node groups regardless of the site. Recurrences to the neck lymph nodes were observed in 3.3 p.c. of the cases where the removed lymph nodes did not reveal any metastatic changes in histopathological examiantions. Conclusion: Postopeartive results of the neck lymph node histopathological examination point to the necessity of lymphadenectomy in the II-V levels. Selective lymphadenectomy is not advisable due to the possibility of metastases to all lvels of the neck lymph nodes regardles of the primary tumor site.
